Diagnosis of pancreatic abscess via percutaneous aspiration.

UNLABELLED: The pre-operative diagnosis of a pancreatic abscess was not considered in a comprehensive review in 1972. However, advances in technology (Ultrasound--US, Computed Tomography--CT) has allowed guided percutaneous needle aspiration (PNA) of suspected pancreatic lesions. The purpose of this study was to evaluate the safety and diagnostic ability of PNA to differentiate acute pancreatic inflammatory masses from pancreatic abscess (PA). Thirteen patients underwent PNA after US or CT revealed an acute pancreatic inflammatory mass (12/13 cystic). One patient underwent a second aspiration. Clinical features T degrees--101.3 degrees F mean (13/13), leukocytosis 14,400 cu/mm (11/13). Aspirated material was gram-stained and examined for bacteria and leukocytes and cultured. RESULTS: PNA was accomplished successfully in all patients. Aspirate revealed bacteria in nine and pancreatic abscess was confirmed at surgery (8) or post-mortem exam (1). Four of five patients in whom no bacteria were visualized had medical resolution, the fifth had continued T degree and underwent a second aspiration which diagnosed a PA. PA contained moderate to large number of PML via aspiration. CONCLUSIONS: PNA provides a potentially important and safe diagnosis adjunct to earlier accurate differential diagnosis of pancreatic inflammatory masses from pancreatic abscess.

Effect of microwave and conventional baking on yellow cakes.

The quality of yellow butter cakes was adversely affected by microwave cookery. Conventional cakes consistently received the highest sensory ratings. Cakes baked on a carousel in the microwave oven were evaluated as being equal or superior to cakes baked in the microwave without a carousel. Sensory evaluations for appearance and flavor were significantly higher, and values for shear resistance were significantly lower, indicating greater tenderness, for cakes baked in the microwave oven with a carousel compared to those manually turned. Although cakes baked in a conventional oven were superior in appearance, tenderness, mouth feel, flavor, and texture, cakes baked in the microwave oven were considered satisfactory.

Robustness in real life: a study of clinical laboratory data.

Previous studies have evaluated the performance of robust estimators of location using computer-generated distributions. This paper describes how distributions commonly seen in biomedical research differ from the simulated distributions, and compares the performance of six families of estimators using samples from four biomedical distributions. The biomedical distributions can be asymmetric, and have shorter tails and other anomalies, such as digit preferences, which are not modeled in simulation studies. The four biomedical distributions studied differ markedly from the models previously studied, and these differences strongly affect the robust estimators. In such situations the trimmed mean provides an improvement over the sample mean, and its performance is more stable than that of estimators whose psi functions have steeply-descending tails. The central value and dispersion of each of the estimators are evaluated.

The effect of methionine on the progression of hepatocellular carcinoma induced by ethionine.

Female Wistar rats were fed a semisynthetic diet containing 0.3% DL-ethionine for 52 weeks and, most of them bearing hepatocellular carcinomas, were switched to a basal diet for one week and then received 80 mg DL-methionine per rat (cir. 25 mg/100 g body wt) by gavage once per day, five times a week, over a period of 13 weeks. Methionine treatment significantly affected progression of hepatic tumors induced by DL-ethionine administration. The frequency of rats with hepatocellular carcinomas in the methionine treated group was significantly lower than that in control groups. It therefore appears that methionine treatment resulted in a remission of the malignant process.

Therapeutic percutaneous aspiration of pancreatic pseudocysts.

The therapeutic efficacy and safety of percutaneous aspiration of chronic pancreatic pseudocysts was evaluated. Eight patients underwent aspiration a total of ten times. Permanent resolution was obtained in two patients and a third nonsurgical candidate was offered an alternative therapeutic modality. This procedure is simple, rapid, and safe and could become the initial approach to selected patients with a chronic pancreatic pseudocyst.

Naloxone suppresses food/water consumption in the deprived cat.

Intraperitoneal administration of the opiate antagonist, naloxone hydrochloride, resulted in decreased food and water consumption in drug-naive cats. In a cross-over Latin Square design, food consumed by six cats in a one hour period following 23 hours of deprivation, was decreased significantly below control (p less than 0.05) in linear relation to increasing dose (1 mg/kg and 10 mg/kg) of naloxone. Non-linear and time/order effects were not significant. Water consumption was decreased below control in a linear relation to increasing dose (1 and 10 mg/kg) for 5 of 6 cats at the 0.05 significance level. Non-linear and time/order effects on water consumption were not statistically significant for the same 5 cats. These results, and behavioral signs (i.e., vomiting, persistant vocalization, heavy salivation, mydriasis, moderate catatonia, and hissing) occasionally exhibited by four of the six cats in a 1-hr period following injection of the high dose, suggest a malaise-effect of naloxone.
