[The multisegmental electron irradiation of the chest wall following ablative therapy of breast cancer].

PURPOSE: The paper describes the clinical aspects of postoperative irradiation of breast cancer using electron arc technique at the chest wall. The value of the treatment is assessed by long-term results concerning the rate of local recurrences and side effects. The advantages of the procedure are described with regard to physical and technical aspects. PATIENTS AND METHODS: After mastectomy, 117 patients underwent and adjuvant radiotherapy of the chest wall by multisegmental electron rotation technique with energies between 4 and 14 MeV. The number and angular range of the segments as well as the energy of the electrons depended on the peculiar thickness of the chest wall. 25 times 2 Gy were applied within five weeks. The treatment planning is based on a patient model constructed from five CT-slices at least. RESULTS: In four cases (3.4%) a local relapse and in one case (0.9%) a relapse in supraclavicular lymphatics was found. The recurrences appeared after an average period of 32 months. The mean observation time was 70 months. In 37 women (32%) distant metastases were seen after 29 months on an average. The five-year survival rate is 73.5%. CONCLUSION: In comparison to usual techniques using photons the described method shows a more homogeneous dose distribution to the chest wall and a reduction of lung radiation exposure. The clinical result of this dose adaptation to the treatment volume is a lower rate of local relapses and of side effects, inclusive late reactions. It can be suggested that the efficiency of the procedure will be ameliorated in future using special electron compensators. This will result in a more favorable adaptation of dose distribution to the anatomical situation in regard to the cranio-caudal profile.

[The role of radiotherapy in endocrine orbitopathy].

The treatment of Graves' ophthalmopathy is symptomatic because the etiology remains unknown though evidence supports an aberrant immune response. Orbital radiotherapy, combined with corticosteroids, is indicated for rapidly progressive ophthalmopathy and troublesome soft tissue symptoms. In case of sight-threatening manifestations operative decompression is the method of choice. However, radiotherapy seems to be a safe and effective method as a good to excellent response in 60-70% of patients can be expected. The treatment results of 15 patients with a long term follow-up are presented.

[Five simultaneous neoplasms].

One case of five independent tumors is reported: Fibrosarcoma of the right thigh, bilateral carcinoma of breast, papillary carcinoma of thyroid, and basalioma below the left clavicula. A synopsis of literature containing the rare cases of more than three primary tumors is presented. The difficulties concerning localization, origin and registration of multiple primary malignancies and their differentiation to recurrencies and metastases are discussed.

[Value of pharyngography in the diagnostic staging program of pharyngeal tumors].

The role of barium pharyngograms in the pre- and post-therapeutic evaluation of pharyngeal tumors is described. The intralumenal part of the tumor is well shown by high spatial and contrast resolution of the pharyngeal mucosa. Barium pharyngogram is the only imaging method which can record the course of the swallowing act at the pharyngeal level. Post-operatively, barium pharyngography reliably demonstrates altered pharyngeal morphology; fibrotic stenoses may be distinguished from narrowing due to recurrence. The result of primary radiotherapy is easy to document.

Endoscopy

[Double-contrast pharyngography in surgically treated and/or irradiated oro- and hypopharyngeal malignant tumors].

The use of a double contrast technique in the oropharynx and hypopharynx is described and its diagnostic value following treatment is defined. By means of the double contrast pharyngogram, it is possible to demonstrate the pharyngeal mucosa and, post-operatively, the anastomosis. It is the only imaging method which demonstrates the course of swallowing and passage through the pharynx. By paying attention to physiological changes following treatment and taking account of early post-therapeutic appearances, it is possible to form a judgement concerning the peripharyngeal tissue. CT, however, is the method of choice for demonstrating an extraluminal tumour. In view of the ease of the method, double contrast pharyngography is the ideal radiological procedure for close follow-up of patients with malignant tumours of the pharynx.

Barium Sulfate

[Results of muscle biopsies in diphtheritic polyneuropathy. Light- and electron-microscopic examinations of muscle fibers, intramuscular nerves, motor endplates, and intramuscular vessels].

Muscle biopsies from the lower extremities of four patients with severe tetraplegic form of diphtheritic polyneuropathy were examined by modern techniques including histochemistry, electron microscopy and morphometric procedures. Until now comparable studies have not been published. The biopsies were removed during the acute stage of the polyneuropathy. We found scattered small angulated muscle fibers beside a more generalized slight atrophy predominantly of type 2 B fibers and targetoid-phenomenons or cores in type 1 fibers. Beside this neurogenic pattern there also were, corresponding with the results of electromyography, primary myogenic alterations with different degenerative phenomenons, suspicious of toxic origin as in cardiac muscle. The intramuscular vessels showed no abnormalities except some perivascular predominantly mononuclear cellular reactions with a remarkable number of cerebriform lymphoid cells, probably T-lymphocytes. No specific pathological alterations could be detected in 11 intramuscular nerves and two motor endplates. This may reflect the more proximal demyelination of human peripheral neurons by the diphtheria toxin as found in experimental diphtheria of the rabbit in contrast to the more distal type of the guinea pig.

Adolescent
