Spironolactone versus nifedipine in essential hypertension.

In a double-blind, randomized, multicenter study of 194 patients with moderate hypertension, spironolactone and nifedipine were found to reduce blood pressure (BP) to about the same extent and in the same percentage of patients after 45 days of treatment (47 and 50%, respectively). At that point, the patients controlled by either drug continued on their regimen for another 45 days, while patients whose BP was still elevated (diastolic BP greater than 90 mm Hg), received the other drug in addition. After 45 days of combination therapy, 63% of the patients had normal BP, whereas those receiving monotherapy largely remained normotensive (96% in the spironolactone group and 88% in the nifedipine group). The adverse effects were not severe with either group, but the incidence was markedly higher in the nifedipine group.

Cytoprotective effects of Gypsophila saponins towards isolated rat hepatocytes.

Saponins are glycosides widely distributed in the plant kingdom and are found in many foods. The hepatoprotective potential of glucuronogypsogenin (GG) and gypsoside (GY) towards isolated rat hepatocytes treated by three toxic models used at sub-lethal doses: galactosamine (5 x 10(-3) M), CCl4 (5 x 10(-4) M) and erythromycin (5 x 10(-4) M) was investigated. Two schedules were carried out corresponding to curative or preventive treatment. No protection was observed on hepatocytes treated with GY before or after addition of the toxicants. In contrast, a protective action was detected when hepatocytes were pretreated with GG (5 x 10(-5) M) as probe, by the normalisation of LDH leakage and ATP content. It depends on the toxicant: the cytoprotective spectrum is 5 x 10(-5) to 5 x 10(-7) M with galactosamine; 5 x 10(-5) to 5 x 10(-6) M with CCl4; and around 5 x 10(-5) M with erythromycin. Taking into account the importance of LDH as an indicator of membrane damages, GG was assumed to interact with membrane hepatocyte.

Primary nursing in long-term care: is it economically feasible?

1. The demand for nursing home beds exceeds supply, creating a sellers market. Administrators, therefore, do not see the need for making costly changes in the method of nursing care delivery. 2. ++Modified primary nursing, where permanent RN-nursing assistant teams are accountable to a permanent patient assignment, can be budget-neutral. Studies have suggested that primary nursing decreases supply costs, the use of sick leave, and the total number of staff. 3. For primary nursing in long-term care to be feasible, there must be an adequate number of budgeted full time employees, an adequate supply of RNs who are willing to work in long-term care, and adequate salaries to attract competent staff. 4. Although research addressing the costs of implementing primary nursing in long-term care exists, it is largely anecdotal. More controlled studies are needed.

[Role of laser angioplasty in the management of peripheral arteriopathies. Report of 79 cases].

From May 1988 to November 1989, 79 of the 341 patients admitted to our center requiring angioplasty for peripheral artery disease were treated by laser angioplasty (LA) associated with secondary dilatation. Laser angioplasty was performed with and Nd-YAG laser coupled by an optical fiber to 1.8, 2.2 and 3 mm sapphires. The population studied included 64 men and 15 women (mean age: 66 years). Functional outcome before LA was class II in 85% of cases (n = 67) and class III and IV respectively in 10 (13%) and 2 patients. Laser angioplasty was the primary indication in 61 patients (77%) with a femoropopliteal (n = 53), iliac (n = 7) or axillary (n = 1) arterial obstruction. Laser angioplasty was the secondary indication in stenoses which could not be negotiated by a guide or balloon (23%; n = 18). Laser angioplasty led to repermeabilization of arterial occlusions in 82% of cases. The best results were obtained for short occlusions of less than 3 cm (n = 5; 100%) and for femoropopliteal lesions (n = 60; 85%). Stenoses were revascularized in all cases. Complementary dilatation was performed at the same time in all revascularized patients. Forty-seven revascularized patients were followed up 3 to 6 months after LA. Results were good in 36 cases (77%); restenosis occurred in 7 cases (15%); and reocclusion in 4 cases (8%). Thyroglobulin structure and function: recent advances.

Thyroglobulin is a large-size iodoglycoprotein specific to thyroid tissue and is the substrate for the synthesis of thyroid hormones, thyroxine and 3,5,3'-triiodothyronine. Recent studies, which greatly benefited from recombinant DNA methodologies, improved the knowledge of several structural features of this dimeric protein and permitted insights into some structure-function relationships. Analysis-function of the primary structure of the human thyroglobulin monomer revealed several main characteristics: 1) 3 types of internal homologies; 2) extensive homology with the bovine thyroglobulin monomer and known partial sequences in the thyroglobulins of other mammalian species; 3) significant homologies with 2 other non-thyroid proteins (acetylcholinesterase and the invariant chain of the Ia class II histocompatibility antigen); 4) a terminal localization of the hormonogenic sites at both ends of the monomer. Current studies aim at determining conformational characteristics, understanding the molecular mechanisms of thyroid hormone formation and unraveling those interactions which in the thyroid cell and the thyroid follicle will permit this large pro-hormone to synthesize and release a few small thyroid hormone molecules. A more precise knowledge of this molecule in higher vertebrates and during evolution would impart valuable information concerning thyroid pathology, since thyroglobulin has been implicated in some genetic and in autoimmune thyroid diseases.

Cultural relativity and poverty.

The nurse who practices from a perspective of cultural relativity attempts to understand client behaviors within the context of the clients' culture. Viewing customs (behaviors) as a reflection of client beliefs and values can enhance the nurse's effectiveness with clients in poverty. This paper presents a case study in which a culturally relativistic perspective was used to assess and intervene with a family living in poverty.

Epidural blood patch in the treatment of post dural puncture headache: a double blind study.

A randomised double blind controlled trial of 'epidural blood-patch' as treatment for post-dural puncture headache is presented. The method was successful in 11 out of 12 cases on its first application, the twelfth patient being relieved by a second procedure. None of six patients reported benefit from a 'sham' procedure. The rationale for the use of the technique is discussed.

[Structural heterogeneity of human thyroglobulin in relation to its messenger RNA maturation].

One human thyroglobulin cDNA clone present 192 nucleotides deletion which correspond to 64 amino-acid deletion on the protein molecule. We show that this fact figure the exon loss on the mRNA molecule using as template during cDNA synthesis by comparison between mRNA and genomic DNA nucleotide sequences. Different possibility can explain this fact. We retain mainly mistake in mRNA maturation fidelity. This type of event is not an exception and can be an explanation for some pathologies.

Recurrent Streptococcus mutans endocarditis.

Recurrent bacterial endocarditis is associated with several risk factors including parenteral drug abuse, past valvular heart disease, and periodontitis. Recurrence rates vary from 2 to 18 percent for a single recurrence, and 1 to 5 percent for two recurrences. An unusual case study of a patient experiencing three episodes of bacterial endocarditis following dental work is described. The episodes occurred at least one year apart and were characterized by biochemically identical strains of Streptococcus mutans.
