Successful therapy of sarcoidosis-associated thrombocytopenia refractory to corticosteroids by a single course of human gammaglobulins.

Thrombocytopenia developed in a 21-year-old patient with sarcoidosis when corticosteroid therapy was discontinued. Platelet-associated IgGs were elevated. High-dose cortisone did not affect the thrombocytopenia. However, platelet counts remained normal for more than 1.5 years following intravenous treatment with a single course of human gammaglobulins.

Association between Legionella pneumophila and amoebae in water.

The presence of amoebae and Legionella pneumophila in ground-water, drinking water supplies and whirlpools was investigated. Volumes of 10 to 1,000 ml were concentrated by membrane filtration. L. pneumophila was detected on buffered charcoal yeast extract (BCYE) agar, and amoebae by inverting filters on nonnutrient agar plates seeded with Escherichia coli that were incubated at 37 C for up to 12 days. In 65% of the samples positive for L. pneumophila amoebae were also detected. L. pneumophila and amoebae were detected together in 38% of warm drinking water samples. The highest isolation temperature for amoebae was 57 C, but fewer amoebae were detected above than below 50 C. In cold drinking water, amoebae were found in 88% of samples. The presence of L. pneumophila and amoebae in whirlpool waters (42%) presents a risk for man. Fresh environmental isolates of an Acanthamoeba species and L. pneumophila serogroup 4 were used for laboratory experiments. The amoebae supported intracellular multiplication of L. pneumophila in Chang's medium and autoclaved tap water, as shown by colony-forming unit (CFU) counts, direct fluorescent antibody test and Gimenez staining. Results confirmed that interaction between L. pneumophila and amoebae could occur in nature, and that the latter could act as hosts for legionellae and support their growth.

A sensitive screening method of detecting anti-granulocyte antibodies employing radiolabeled staphylococcal protein A.

We describe an improved method of detecting anti-granulocyte antibodies utilizing radiolabeled staphylococcal protein A (SPA). The results of this SPA assay were compared to data obtained with leukoagglutination tests and granulocyte indirect immunofluorescence techniques. We have shown that the SPA assay is highly sensitive and reproducible. In addition, absorption studies confirmed that the assay is specific for granulocytes. The SPA assay is performed in microtiter plates, and requires significantly fewer granulocytes and less test sera than previously described techniques. Also, we have shown that granulocytes prepared for this assay can be separated and stored for up to 48 h. Therefore, the SPA assay described herein is particularly useful for screening of sera and is one of the most sensitive assays available for detecting anti-granulocyte antibodies.

A monoclonal antibody to hemoglobin F.

A monoclonal antibody BMU7-17 which recognizes an antigenic site unique to the cyanogen bromide fragment, CB2 (residues 56 through 133) of the gamma-chain of human hemoglobin has been produced using cell hybridization techniques. The antibody does not crossreact with hemoglobin A, the isolated alpha- or beta-chains, or with hemoglobin of various mammals. Affinity chromatography on Staphylococcal protein A-Sepharose 4B and radial immuno-diffusion revealed the antibody to be of the mouse IgG 1 class. BMU7-17 identifies F cells in adult and in cord blood specimens, as demonstrated by an indirect immunofluorescence assay. The dissociation constant of the antibody as determined using tritiated hemoglobin F with a double antibody radioimmunoassay has a Kd = 1.2 X 10(-9) moles by Scatchard plot analysis.

Outcome of bone marrow transplantation in patients with extramedullary involvement of acute leukemia.

Infiltration of extrahemopoietic tissue with leukemic cells was evaluated as a prognostic indicator in 18 patients with acute leukemia undergoing bone marrow transplantation. When compared to 107 patients who did not have extramedullary leukemia at any time prior to marrow grafting, the patients with leukemic invasion into organs outside the hemopoietic system had a significant increase of leukemic recurrence and a significant decrease in survival after marrow transplantation. Extramedullary leukemia may be a negative prognostic indicator for bone marrow transplantation candidates.

Visual detection of granulocyte surface antigens using the avidin-biotin complex.

A visual test for detection of granulocyte surface markers using the avidin-biotin complex (ABC) has been developed. That this assay is highly specific, reproducible, and sensitive was determined by studying the expression of HLA antigens on granulocytes with monoclonal antibodies. Further, using granulocyte specific alloantisera, the results of the ABC test compared well to data from leukoagglutination assays and indirect immunofluorescence tests. The assay is particularly advantageous in that granulocytes can be stored, only small amounts of cells and sera are needed, and heterogeneous cell populations can easily be studied.

[Gastric mucosal dysplasias: what is their clinical significance (author's transl)].

Using criteria of proliferation kinetics, karyometry, histology and histochemistry, the attempt at a definition of normal and dysplastic surface epithelium of the gastric mucosa is made. Instead of using such terms as surface carcinoma, intra-epithelial carcinoma and carcinoma-in-situ, the term dysplasia III of the surface epithelium is suggested, because serial biopsies of gastric mucosa over periods ranging from one month to two years have demonstrated reversibility or persistence of such changes without transition into early carcinoma. Results of such studies in 8 262 patients demonstrate that even proliferation-kinetic, karyometric and histochemical methods do not make it possible to judge from single tests the prospective potency of certain changes of the gastric surface mucosa as "precancerous" or even "intra-epithelial carcinoma".
