[Malignant tumor-like abdominal lymphoma in Whipple's disease].

A 43-year-old man had severe upper abdominal pain and weight loss of 8 kg for over three months. He underwent a laparotomy because, computed tomography having revealed numerous mesenteric and para-aortic lymph nodes, a malignant lymphoma was suspected. Histological examination of a mesenteric lymph node demonstrated exclusively extraintestinal Whipple's disease. The symptoms completely disappeared after the administration of 1.2 mega U penicillin G and 1 g streptomycin daily for 14 days, followed by twice daily 160 mg trimethoprim and 800 mg sulphamethoxazole.

[Functional and morphological changes in the stomach in irradiation of the para-aortic region].

Six assays were conducted in order to determine some functional and morphological parameters of the gastric mucosa in patients who underwent Co60 irradiations of the para-aortic area. Slight as well as marked morphological alterations were observed whereas the functional alterations were found to be marked in all cases. Basal gastric secretion and serum gastrin level showed a continuous reduction, however, in case of severe gastritis the stimulated secretion was increased. A possible correlation with increased tissue histamine levels is discussed.

Dahl salt-sensitive rats and human essential hypertension.

At Brookhaven we have two new strains of rats with opposing genetic predispositions to hypertension: one strain [inbred Dahl salt-sensitive (Dahl-S)] develops severe hypertension on a high-salt diet, while the other [inbred Dahl salt-resistant (Dahl-R)] responds only mildly, if at all. The striking observation is that, unlike other salt-sensitive hypertension-prone rats, inbred Dahl-S rats remain normotensive on a low-salt diet. The observation that hypertension in the inbred Dahl-S, but not the inbred Dahl-R, rat develops only after exposure to a high-salt diet indicates that the development of hypertension in the inbred Dahl-S strain is not spontaneous and that the genetic substrate is a critical determinant of the hazard of excess salt in the diet.

Pathophysiological aspects of dacarbazine-induced human liver damage.

Hepatic Veno-occlusive Disease (VOD) and Budd Chiari syndrome (BCS) have been reported as fatal complications of Dacarbazine therapy. Peripheral eosinophilia and eosinophilic infiltrates after repeated Dacarbazine therapy in some cases were interpreted as evidence of an allergic drug reaction. Since no mention of an allergic drug reaction is to be found in other reports on veno-occlusive disease or Budd Chiari syndrome due to Dacarbazine, the pathophysiological mechanism of these fatal complications of Dacarbazine remain to be elucidated. We describe a case of VOD due to Dacarbazine which showed peripheral eosinophilia after the first cycle of Dacarbazine treatment with no prior possibility of sensibilization, along with signs of a toxic drug reaction. This observation suggests that peripheral eosinophilia, so far interpreted as a sign of an allergic reaction induced by Dacarbazine, can be observed together with obvious signs of toxic drug reaction, thus providing further evidence of a toxic mechanism of Dacarbazine-induced VOD or BCS.

[Clinical aspects and pathology of primary tubal cancer].

Primary carcinoma of the fallopian tube is the least common of the malignant tumors of the female genital tract. Ten cases of primary fallopian tube carcinoma treated at the Women's Department of the University Hospital Mannheim between 1965 and 1983 are described. Four patients survived five years, of whom three had a stage I tumor. The most important factor affecting survival appeared to be the extent of the tumor at the time of diagnosis.

[Cortico-cancellous grafts in infected defect-pseudarthrosis].

Biomechanical studies of stabilizing effect of cortico-cancellous-grafts showed that the resistance of internal fixation is not endangered acutely under a one-foot weight bearing situation. The stability of defectosteosynthesis was significantly raised not only when a lateral joist was left but also when a medial cortico-cancellous-graft was fixed. 29 patients with chronical defectosteitis were treated by internal fixation and cortico-cancellous-grafts on the opposite side to improve mechanical stability and to place biological material for bridging.

[Carcinoma in situ of the urethra].

A case of carcinoma in situ of the male urethra is reported. Incidence, morphology, etiology and clinical significance of this tumorous lesion are discussed. Between transitional cell carcinoma of the urinary bladder and urethral carcinoma in situ there is a relative high coincidence. This requires diagnostic and therapeutical procedures to prevent the development of an invasive carcinoma of the urethra.

Differential development of salt-induced and renal hypertension in Dahl hypertension-sensitive rats after neonatal sympathectomy.

Rats with a genetic susceptibility to salt hypertension were given repeated neonatal injections of guanethidine. Vascular reactivity and tissue catecholamine concentrations indicated that a peripheral sympathectomy had been produced. Chemically sympathectomized rats had lower blood pressure than controls while fed a diet containing 0.4% NaCl. Furthermore, the dramatic rise in blood pressure exhibited by control rats fed a diet containing 8.0% NaCl was completely absent in sympathectomized rats similarly fed. The absence of salt-induced hypertension was observed regardless of whether the animals were anesthetized with ether or pentobarbital or had the blood pressures determined in an unanesthetized state. Finally, two-kidney Goldblatt hypertension did develop in sympathectomized rats, but to a level below intact rats similarly treated.
