Haemophiliac hands--a three year follow-up study.

The total absence of literature regarding the affects of haemophilia on the hand prompted this three year study at the Israel National Haemophilia Centre. The hands of all haemophiliacs were examined and X-rayed at the time of their annual check-up. Apart from a number of established Volkmann's contractures in elderly patients and multiple bone cysts of the carpus, surprisingly little pathology was found. When considering the extensive damage done to the musculoskeletal system by this haemarthritic pathogenesis it is astounding how the hand, with its multiple small joints, constant movement and trauma, escapes significant damage.

A new orthopedic fixation method in the treatment of bladder extrophy.

Extreme diastasis of the pubis and malalignment of both ilium and hip joints are always associated with the absence of infraumbilical structures and with the failure of the anterior bladder wall to close. Therefore, for a successful cure of extrophy, the pelvic ring has to be repaired, at the time of or a few days prior to the surgical cure of the bladder. The repair of the pelvis, when executed in poor aseptic conditions, results in difficult nursing problems, and fails due to the lack of convenient fixation. For these reasons, we transformed Bartons tongues into an external fixator which enhanced and improved the controlled contention of the "vertical bilateral posterior osteotomy" of the pelvis. When a two stage procedure is decided on, the modified Bartons tongues do not interfere with the urologic surgery, and in addition, the nursing problems are alleviated. Although we have used our modified Bartons tongues in only three cases, our methodology is of low enough risk for it to be reported at this time.

[Can the course of an acute leukemia be predicted?].

65 unselected patients with acute leukemia, diagnosed in 1971 to 1978, were analysed for prognostic factors. The survival-curves (constructed by means of Kaplan-Meier-estimation) were compared with Logrank and generalized Wilcoxon Test. Response to therapy, age, sex, initial granulocyte and platelet-count, LDH and portion of peripheral blasts were examined. The median survival time of patients with partial or complete remission (35.5 weeks) was almost six times as long as in treated patients without remission. The prognosis got worse in higher age. Women had with 7.5 weeks a shorter survival time compared to men with 23 weeks. The same was true for low initial platelet-count, high peripheral blast-portion and high LDH. The morphologic type and initial granulocyte counts had no prognostic value.

The clinical importance of factor XI deficiency in an Ashkenazi Jewish patient.

Factor XI (PTA) deficiency, a coagulopathy, has been found to be relatively common in Ashkenazi Jews. Clinically it may manifest itself by varying forms of hemorrhage including postoperative bleeding. It is recommended that all Ashkenazi Jews have a partial thromboplastin time (PTT) coagulation test before surgery in order to rule out this coagulopathy, and if the PTT is abnormal a Factor XI assay should be done to confirm the diagnosis and ensure that proper therapy can be provided. A surgical case is presented with its ensuing complications resulting form lack of such information.

Subtalar dislocations.

Severe plantarflexion of the foot associated with adduction and supination resulted in medial subtalar dislocation in three patients. Closed manipulation in two patients and open reduction in one restored normal anatomy of the foot, with no complications in a follow-up of 1 to 2 years.

[Cerebrospinal fluid proteins and epilepsy].

The authors examined 54 epileptic patients with a view to prove the presence of an immunological cerebrospinal fluid syndrome. The result showed that a significant immunoglobulin depression could not be proved by means of a cerebrospinal fluid test.

Sexual responsiveness of college students to rape depictions: inhibitory and disinhibitory effects.

Two experiments were conducted to identify the specific dimensions in portrayals of sexual violence that inhibit or disinhibit the sexual responsiveness of male and female college students. The first experiment replicated earlier findings that normals are less sexually aroused by portrayals of sexual assault than by depictions of mutually consenting sex. In the second experiment, it was shown that portraying the rape victim as experiencing an involuntary orgasm disinhibited subjects' sexual responsiveness and resulted in levels of arousal comparable to those elicited by depictions of mutually consenting sex. Surprisingly, however, it was found that although female subjects were most aroused when the rape victim was portrayed as experiencing an orgasm and no pain, males were most aroused when the victim experienced an orgasm and pain. The relevance of these data to pornography and to the common belief among rapists that their victims derive pleasure from being assaulted is discussed. Misattribution, identification, and power explanations of the findings are also discussed. Finally, it is suggested that arousing stimuli that fuse sexuality and violence may have antisocial effects.

Ethical issues and exposure to rape stimuli: a reply to Sherif.

The three ethical issues raised by Sherif are discussed. In response to the first issue, we recognize the need to measure the effectiveness of our attempts to counteract possible undesirable effects of the research materials. Problems that arise in implementing such an assessment are considered. Also discussed is the possibility that exposure to such materials combined with an effective discrediting communication may have beneficial effects. In addressing the second issue, it is argued that Sherif's criticisms of our alternative data interpretations are unfounded. Finally, Sherif's suggestion to formalize the process of having investigators consider the social policy implications of their research is whole-heartedly supported.

An unusual fracture of the base of the fifth metatarsal bone.

An unusual fracture of the base of the fifth metatarsal bone is described. The type of fracture and the displacement of the proximal fragment was unlike that of most other fractures of this region of the foot. It requires open reduction and internal fixation which was successfully carried out by means of tension wires.
