Autonomous ambulatory care by nurse practitioners and physician assistants in office-based settings.

Data from the 1997 National Ambulatory Medical Care Survey were used to examine the autonomous provision of ambulatory medical care by nurse practitioners (NPs) and physician assistants (PAs) in physician-managed office-based settings. An estimated 6.81 million office visits involved autonomous care by NPs and PAs, for an overall rate of 2.55 visits per 100 persons. The visit rates were greatest for patients over 64 years of age, females, blacks, and patients from the Northeast. The visits encompassed a broad range of acute and chronic problems, with a greater proportion of non-illness care visits when compared with visits to physicians. While NPs and PAs provided diagnostic services and pharmacotherapy, there was more emphasis on therapeutic or preventive services in their practices than among physicians' practices. Predisposing, enabling, and need factors were differentially associated with visits to NPs and PAs. Utilization of NPs and PAs as autonomous providers in office-based settings appears limited. Public policy and educational initiatives can focus on predisposing, enabling, and need factors to increase access to autonomous practice of NPs and PAs in ambulatory care.

Nursing faculty: one generation away from extinction?

Concern over an impending shortage of qualified nursing faculty sparked a statewide survey of nurses with graduate degrees in one midwestern state. A return rate of 61 per cent yielded a sample of 288. In general, nursing faculty members were older than nurses with graduate degrees employed in other roles. Three fourths of the sample worked full-time in stable positions in which they intended to remain until retirement. Fewer than one third were nurse educators; one-third were hospital employees. Nearly half the sample had nonnursing graduate degrees despite local availability of two nursing master's programs. Noncompetitive academic salaries, desire for clinical practice, and rising expectations in higher education were factors deterring nurses with graduate degrees from pursuing faculty roles. Improved compensation, greater respect, closer proximity to work, more realistic expectations, and improved opportunity to continue clinical practice were viewed as incentives to entice practicing nurses into faculty roles. Recommendations for addressing these important challenges are summarized. The next generation of nurses depends on strategic planning now to build the pool of qualified faculty for the future.

Restructuring registered nurse curricula.

National initiatives in higher education, health reform, and the nursing curriculum revolution are applied to five curricular patterns for registered nurse (RN) upward mobility baccalaureate education. American Association of Higher Education principles for quality baccalaureate education are applied to the curricular models. The patterns described are: community model, health promotion model, nursing diagnosis model, case management model, and caring model. Three educational strategies for prompting paradigm shifts in RNs who return to school are discussed: collaborative learning, portfolios, and self-assessments. These learning strategies are designed to move the adult student through Perry's phases of cognitive development from duality to relativism. The combination of fresh new curriculum patterns and collaborative learning strategies can empower nurses to discover new paradigms with which to transform the profession.

A qualitative retrospective study of coping strategies of newly widowed elderly. Effects of anticipatory grieving on the caregiver.

Findings of a qualitative retrospective study of coping strategies of newly widowed elderly are presented. The focus is on effects of anticipatory grieving on the caregiver before the spouse's death as reported by the widowed partner following the death. Most frequent and troubling problems were reported as loneliness, social isolation, disruption in eating and sleeping patterns and independent decision making. Changes in health status following the death were not as detrimental as expected. Caregivers' health actually improved when those responsibilities were over. Recommendations for hospice care for the caregiver are suggested.

Interactive television presentation style and teaching materials.

Special teaching techniques and presentation style are required for an effective telecourse. This article describes modification of a traditional graduate nursing course for televised delivery to remote students. Teacher appearance and behaviors as well as student activities for telecourses are proposed.

Nurses' educational needs regarding battered women.

A study was conducted of nurses in rural hospitals to determine their knowledge of and educational needs regarding battered women. The results indicated that nurses had very low knowledge level regarding battered women. The vast majority had not studied this important issue either in nursing school or on the job and, as a result, wanted more staff development on the subject of battered women, with a focus on assessment and intervention with battered women.
