Fate of residual fragments after successful extracorporeal shock wave lithotripsy.

PURPOSE: The aim of the present study was to investigate the reason residual fragments from upper urinary tract calculi failed to clear after successful extracorporeal shock wave lithotripsy (ESWL). METHODS: Risk factors were analyzed in 161 patients with residual fragments (< or = 4 mm) that had remained for more than 3 months after ESWL. The factors examined in the present study were gender, a history of urolithiasis, the number, location and size of stones, hydronephrosis 3 months after ESWL and bacteriuria before ESWL. The mean follow-up period was 20.0 months (range 6-69 months). RESULTS: The overall stone-free rate was 14.3%. The stone-free rate in patients with multiple stones or hydronephrosis 3 months after ESWL was significantly lower than that in patients without these conditions (P < 0.05 and P < 0.01, respectively). The cumulative non-clearance rate in patients with hydronephrosis was significantly higher than in patients without this condition (P < 0.05). Results of Cox's proportional hazards model indicated that hydronephrosis was the most important and only significant factor for failure to clear of the seven factors investigated (P < 0.05). CONCLUSION: Hydronephrosis was most highly correlated with the fate of residual fragments after ESWL.

Vesicular monoamine transporter 1 is responsible for storage of 5-hydroxytryptamine in rat pinealocytes.

Vesicular monoamine transporters (VMATs) are involved in chemical transduction in monoaminergic neurons and various endocrine cells through the storage of monoamines in secretory vesicles. Mammalian pinealocytes contain more 5-hydroxytryptamine (5-HT) than any other cells and are expected to contain VMAT, although no information is available so far. Upon the addition of ATP, radiolabeled 5-HT was taken up by a particulate fraction prepared from cultured rat pinealocytes. The 5-HT uptake was inhibited significantly by bafilomycin A1 (an inhibitor of vacuolar H+-ATPase), 3,5-di-tert-butyl-4-hydroxybenzylidenemalononitrile (a proton conductor), or reserpine (an inhibitor of VMAT). RT-PCR analysis suggested that VMAT type 1 (VMAT1), but not type 2, is expressed. Antibodies against VMAT1 recognized a single polypeptide with an apparent molecular mass of approximately 55 kDa, and specifically immunostained pinealocytes. VMAT1 immunoreactivity was high in the vesicular structures in the varicosities of long branching processes and was associated with 5-HT, but not with synaptophysin, a marker protein for microvesicles. The 5-HT immunoreactivity in the long branching processes disappeared upon incubation with reserpine. These results indicate that 5-HT, at least in part, is stored in vesicles other than microvesicles in pinealocytes through a mechanism similar to that of various secretory vesicles.

Vascular endothelial growth factor activates MAP kinase and enhances collagen synthesis in human mesangial cells.

UNLABELLED: Vascular endothelial growth factor activates MAP kinase and enhances collagen synthesis in human mesangial cells. BACKGROUND: Vascular endothelial growth factor (VEGF) is an endothelial mitogen that is constitutively expressed in normal human glomeruli, but its role in the kidney is still unclear. In this study, we examined the effects of VEGF on human mesangial cells (HMCs). Methods and Results. Reverse transcription-polymerase chain reaction analysis demonstrated the presence of VEGF receptor mRNA (flt-1 and KDR) in HMCs. The treatment of HMCs with VEGF did not cause a change in 3H-thymidine incorporation or cell numbers. In contrast, VEGF caused a dose- and time-dependent increase in collagen synthesis, with threefold to fivefold increases in both cell-associated and secreted collagen synthesis seen after treatment with 200 ng/ml VEGF. The effects of VEGF were attenuated by treatment of HMCs with the tyrosine kinase inhibitor herbimycin A or the MEK inhibitor PD 98059, but not with the protein kinase C (PKC) inhibitor chelerythrine. VEGF treatment also caused a marked increase in p42/p44 mitogen-activated protein kinase (MAPK) activity, but had no significant effect on HMC superoxide production. Finally, an increase in collagen synthesis was also seen in rat mesangial cells treated with VEGF. CONCLUSIONS: These results suggest that VEGF is not a mitogenic signal in HMCs, but may be involved in the regulation of the mesangial matrix in humans by a MAPK-dependent mechanism.

Plasma brain natriuretic peptide level as a biochemical marker of morbidity and mortality in patients with asymptomatic or minimally symptomatic left ventricular dysfunction. Comparison with plasma angiotensin II and endothelin-1.

AIMS: To evaluate the level of plasma brain natriuretic peptide as a predictor of morbidity and mortality in patients with asymptomatic or minimally symptomatic left ventricular dysfunction. METHODS: We measured plasma levels of atrial natriuretic peptide, brain natriuretic peptide, norepinephrine, angiotensin II, and endothelin-1 and monitored haemodynamic parameters in 290 consecutive patients with asymptomatic or minimally and newly symptomatic left ventricular dysfunction (functional classes I-II, mean left ventricular ejection fraction=37%). All patients were followed up for a median period of 812 days. The Cox proportional hazards model was used to assess the association of variables with mortality and morbidity. RESULTS: At the end of the follow-up, 24 patients had suffered cardiac death and 25 had been hospitalized for worsening heart failure during the follow-up period. Among 21 variables such as clinical characteristics, treatment, haemodynamics, and neurohumoral factors, high levels of plasma brain natriuretic peptide (P<0.0001), norepinephrine (P=0.042), left ventricular end-diastolic volume index (P=0.0035), and left ventricular end-diastolic pressure (P=0.033) were shown to be independent predictors of mortality and morbidity by stepwise multivariate analysis. Moreover, only a high level of plasma brain natriuretic peptide (P<0.0001) was shown to be an independent predictor of mortality in these patients. CONCLUSIONS: These results indicate that a high plasma brain natriuretic peptide level provides information about mortality and morbidity in patients with asymptomatic or minimally symptomatic left ventricular dysfunction.

Reversal of multidrug resistance by kopsiflorine isolated from Kopsia dasyrachis.

Kopsiflorine, an indole alkaloid of the aspidofractinine-type isolated from Kopsia dasyrachis, was examined for its effect in enhancing drug cytotoxicity in multidrug-resistant tumor cells. The cytotoxicity of vincristine was enhanced in a concentration-dependent manner by kopsiflorine in drug-resistant KB cells (VJ-300). Kopsiflorine alone had no effect on the growth of drug sensitive or resistant cells, but the intracellular accumulation of vincristine was enhanced by kopsiflorine in VJ-300 cells. Kopsiflorine (10 micrograms/ml) significantly inhibited the binding of [3H]azidopine to P-glycoprotein in VJ-300 cells. The results suggest that kopsiflorine interacts directly with P-glycoprotein and inhibits the efflux of antitumor agents in drug-resistant cells.

Nonconvulsive status epilepticus in eyelid myoclonia with absences--evidence of provocation unrelated to photosensitivity.

A 10-year old girl with eyelid myoclonia with absences (EMA) in whom nonconvulsive status epilepticus developed shortly after awakening is described. A video-polygraphic recording during the status showed the characteristic eye-closure provocation of eyelid myoclonia with upward deviation of the eyeballs and brief absences. Ictal EEG showed generalized polyspikes concomitant with eyelid myoclonia, while absences were accompanied by 3.5 Hz polyspike-wave complexes on EEG. This condition occurred even in total darkness as well as even after seizures precipitated by bright sunlight had been eliminated by medication. The present case suggests that the eye closure mechanism could be a more potent precipitating factor than photosensitivity in the pathophysiology of EMA.

Plate Tectonics at 3.8-3.7 Ga: Field Evidence from the Isua Accretionary Complex, Southern West Greenland.

A 1&rcolon;5000 scale mapping was performed in the Isukasia area of the ca. 3.8-Ga Isua supracrustal belt, southern West Greenland. The mapped area is divided into three units bounded by low-angle thrusts: the Northern, Middle, and Southern Units. The Southern Unit, the best exposed, is composed of 14 subunits (horses) with similar lithostratigraphy, bound by layer-parallel thrusts. Duplex structures are widespread in the Isua belt and vary in scale from a few meters to kilometers. Duplexing proceeded from south to north and is well documented in the relationship between link- and roof-thrusts. The reconstructed lithostratigraphy of each horse reveals a simple pattern, in ascending order, of greenstone with low-K tholeiitic composition with or without pillow lava structures, chert/banded iron-formation, and turbidites. The cherts and underlying low-K tholeiites do not contain continent- or arc-derived material. The lithostratigraphy is quite similar to Phanerozoic "oceanic plate stratigraphy," except for the abundance of mafic material in the turbidites. The evidence of duplex structures and oceanic plate stratigraphy indicates that the Isua supracrustal belt is the oldest accretionary complex in the world. The dominantly mafic turbidite composition suggests that the accretionary complex was formed in an intraoceanic environment comparable to the present-day western Pacific Ocean. The duplex polarity suggests that an older accretionary complex should occur to the south of the Isua complex. Moreover, the presence of seawater (documented by a thick, pillow, lava unit at the bottom of oceanic plate stratigraphy) indicates that the surface temperature was less than ca. 100 degrees C in the Early Archean. The oceanic geotherm for the Early Archean lithosphere as a function of age was calculated based on a model of transient half-space cooling at given parameters of surface and mantle temperatures of 100 degrees and 1450 degrees C, respectively, suggesting that the Archean oceanic lithosphere was rigid. These conclusions-rigidity and lateral plate movement-support the idea that the modern style of plate tectonics was in operation only 0.7-0.8 G.yr. after the formation of the Earth.

Quantitative non-radioactive in situ hybridization study of GAP-43 and SCG10 mRNAs in the cerebral cortex of adult and infant macaque monkeys.

We performed non-radioactive in situ hybridization histochemistry in several areas that include both the association areas and the lower sensory areas of monkey cerebral cortex, and investigated the localization of neurons expressing two growth-associated proteins: GAP-43 and SCG10. Both GAP-43 and SCG10 mRNAs were observed in both pyramidal and non-pyramidal neurons. Prominent hybridization signals for GAP-43 mRNA were observed in layers II-VI of the adult association areas: the prefrontal areas (FD), the temporal (TE) and the parietal (PG) association areas. The signals for GAP-43 mRNA were weak in layers I-III of the adult primary somatosensory area (PB) and primary (OC) and secondary (OB) visual areas, and cells with prominent signals were observed in layers IV-VI. The prominent signals for SCG10 mRNA were observed in layers IV-VI of all areas examined. These results suggest that the expression pattern of GAP-43 mRNA, but not that of SCG10 mRNA, may be related to the functional difference between association and lower sensory areas of adult cortex. In the infant cortex (postnatal days 2, 8 and 31), the signals for both mRNAs were intense in layers II-VI of all areas. Therefore, layer-specific expressions of GAP-43 and SCG10 mRNAs are established after the first postnatal month.

The need for long-term treatment in the mouse lymphoma assay.

The L5178Y tk +/- mouse lymphoma assay (MLA) has been widely used as a genotoxicity test for the detection of mutagens and clastogens. The standard MLA, as well as other mammalian cell gene mutation assays, usually employs a short treatment period (3-6 h). Our previous report, however, suggested that such short treatments may be insufficient for detecting some clastogens and spindle poisons. For the present study, we introduced and evaluated a longer treatment (24 h) in the MLA. We examined 15 chemicals which were evaluated as negative or inconclusive in the short-term study. Cells were exposed to the chemical for 24 h without S9 mix, cultured for 2 days and then thymidine kinase-deficient mutants were selected in 96-well microtiter plates under trifluorothymidine. Eleven chemicals yielded positive responses in the 24 h treatment MLA. They included nucleoside analogs (2'-deoxycoformycin and dideoxycytidine), a base analog (1,3-dimethylxanthine) and spindle poisons (colchicine and vinblastine sulfate), all of which do not directly affect DNA, but bring about mutations and chromosome alterations through nucleoside metabolism and chromosome segregation. Because the mutagenicities of these non-DNA targeting chemicals appear to be cell cycle dependent, treatment extending over more than one cell cycle may be required for their effect. Combining results from the present and previous studies, 31 of 34 (91%) chromosome aberration-positive chemicals exhibited positive responses in the MLA, suggesting that the sensitivity of the MLA with 24 h treatment periods approaches that of the chromosome aberration test.

Evaluation of the mouse lymphoma tk assay (microwell method) as an alternative to the in vitro chromosomal aberration test.

In order to evaluate the utility of the mouse lymphoma assay (MLA) for detecting in vitro clastogens and spindle poisons and to compare it with the in vitro chromosomal aberration test (CA), we conducted an international collaborative study of the MLA that included 45 Japanese laboratories and seven overseas laboratories under the cooperation of the Ministry of Health and Welfare of Japan and the Japanese Pharmaceutical Manufacturer's Association. We examined 40 chemicals; 33 were reportedly positive in the CA but negative in the bacterial reverse mutation assay, six were negative in both assays and one was positive in both. We assayed mutations of the thymidine kinase (TK) locus (tk) of L5178Y tk +/- mouse lymphoma cells using the microwell method. According to our standard protocol, cells were exposed to the chemical for 3 h, cultured for 2 days and TK-deficient mutants were expressed in 96-well plates under trifluorothymidine. Each chemical was coded and tested by two or three laboratories. Among the 34 CA-positive chemicals, positive MLA results were obtained for 20 and negative results were obtained for nine. The remaining five chemicals were inconclusive or equivocal because of discrepant inter-laboratory results or reproduced discrepant results, respectively. Among the six CA-negative chemicals, one was negative in the MLA, two were positive and three were inconclusive. Thus, the MLA could detect only 59% (20/34) of CA-positive chemicals. We concluded that the MLA was not as sensitive as the CA. Some MLA-negative chemicals evoked positive responses in the CA only after long continuous treatment. These might also be genotoxic in the MLA with long continuous treatment. Improvement of the MLA protocol, including alteration of the duration of the treatment, might render the MLA as sensitive as the CA.

A proposal for a simple way to distinguish aneugens from clastogens in the in vitro micronucleus test.

In our previous in vitro micronucleus (MN) study, we showed that aneugens, in addition to inducing micronuclei, induce a higher frequency of polynuclear (PN) and mitotic (M) cells than clastogens. We hypothesized that the frequency of PN and M cells induced can distinguish aneugens from clastogens. To test the hypothesis, we conducted the micronucleus tests with mitomycin C (MMC), N-methyl-N'-nitro-N-nitrosoguanidine (MNNG), vincristine (VINC) and diazepam in a Chinese hamster cell line (CHL) and VINC, benzo[a]pyrene (BP) and 7,12-dimethylbenz[a]anthracene (DMBA) in a subclone of V79 cells (V79-MZ). All chemicals increased the frequency of M cells with statistical significance. All chemicals except diazepam increased the frequency of PN cells with statistical significance. Three of the aneugens (VINC, BP and DMBA) induced >/=200 PN cells/1000 cells while the clastogens (MNNG and MMC) induced 100 PN cells at most. All the aneugens but no clastogens significantly increased the frequency of M cells. We propose that micronucleus test-positive chemicals that induce >/=200 PN cells/1000 cells and significantly increase the frequency of M cells are aneugens and those that induce at most 100 PN cells/1000 cells and do not significantly increase the frequency of M cells in our MN test protocol are clastogens. Diazepam, however, did not induce PN cells, although it increased the frequency of M cells dose dependently. We explain this fact in relation to diazepam's mode of action. Our proposal suggests a quick, easy and practical way to distinguish aneugens from clastogens for screening purposes.

Validation study of the in vitro micronucleus test in a Chinese hamster lung cell line (CHL/IU).

We conducted a collaborative validation study, under the auspices of the Japanese Ministry of Labour, on the in vitro micronucleus test to see if it could be used as an alternative to the in vitro chromosome aberration test for evaluation of chemical safety. We used the Chinese hamster lung cell line (CHL/IU), which is the most widely used system for the latter test in Japan, and evaluated 66 chemicals, including clastogens and polyploidy inducers. The cytochalasin B cytokinesis blocking method, which is commonly used in human lymphocyte culture, was applied to the established cell line, but did not improve the detection of chemically-induced micronuclei in continuously growing cells. The highest micronucleus frequencies were obtained at 48 or 72 h continuous treatments. In short treatments (6 h), a 42 h recovery time yielded the best responses. Concordance between the results of the micronucleus test and the chromosomal aberration test was satisfactorily high (88.7%), and we concluded that the in vitro micronucleus test could be used in place of the chromosomal aberration test as a simple and rapid method for detecting clastogens and aneugens in vitro. We also propose a protocol for the test.

1,2-dotriacontanedioyl-sn-glycero-3-phosphocholine: A cyclic lipid used for phospholipase A2-catalyzed modulation of the liposome surface charge.

An aqueous suspension of liposomes, which were made of cyclic 1, 2-dotriacontanedioyl-sn-glycero-3-phosphocholine (dTPC) and 1, 2-diphytanyl-sn-glycero-3-phosphocholine, was incubatedwith phospholipase A2 (Naja mossambica mossambica) in 0.05 M Tris-HCl, pH 7.7-7.8. At 25 degrees C, approximately 30% of the cyclic lipids was digested into 1-(omega-carboxyhentriacontanoyl)-sn-glycero-3-phosphocholine (lyso-dTPC), the original morphology being preserved but with a decrease in the zeta-potential of the membrane from ca. +2 to -1 mv. At 38 degrees C in 30 min, as much as 45% of the cyclic lipids was attacked by the lipase, which resulted in lowering of the potential to -2.5 to -3 mV. The enzymatic surface-charge modulation was discussed in conjunction with a side-selective attack of the phospholipase on the lipid bilayer.

Serotonin derivative, N-(p-Coumaroyl)serotonin, isolated from safflower (Carthamus tinctorius L.) oil cake augments the proliferation of normal human and mouse fibroblasts in synergy with basic fibroblast growth factor (bFGF) or epidermal growth factor (EGF).

N-(p-Coumaroyl)serotonin (CS) with antioxidative activity is present in safflower oil. We have reported that CS inhibits proinflammatory cytokine generation from human monocytes in vitro. As reactive oxygen species (ROS) affect cell proliferation, in this study the effect of CS on the proliferation of various cell types was examined. CS augments the proliferation of normal human and mouse fibroblast cells. The cells continue to proliferate in the presence of CS and form a transformed cell-like focus without transformation. CS, however, does not augment the proliferation of other cell types, either normal or tumor cells. CS augments the proliferation of fibroblasts in synergy with basic fibroblast growth factor (bFGF) or epidermal growth factor (EGF), but not with acidic FGF(aFGF) or platelet-derived growth factor (PDGF). This study using synthesized derivatives of CS reveals that the growth-promoting activity is not due to antioxidative activity. These findings indicate that CS is a natural compound with unique growth-promoting activity for fibroblasts.

Molecular cloning of a defense-response-related cytochrome P450 gene from tobacco.

Plant defenses against pathogen attack involve a series of inducible responses that contribute to resistance. Tobacco leaves injected with HWC (hyphal wall components prepared from Phytophthora infestans) elicitor showed typical defense responses, including the induction of localized necrosis and the accumulation of pathogenesis-related proteins. In order to elucidate the molecular mechanisms by which plant defense systems are activated, we screened tobacco plants for genes differentially expressed in response to HWC. We performed differential screening by RT-PCR with random primers and obtained PCR products specific to HWC-treated leaf RNA. Northern hybridization using the PCR products as probes confirmed that one transcript was actually induced by HWC treatment. As the deduced amino acid sequence of this clone showed the highest degree of similarity to elicitor-induced soybean cytochrome P450 CYP82A4, it was designated CYP82E1. The expression of CYP82E1 was strongly induced in tobacco by the soybean pathogen Pseudomonas syringae pv. glycinea (nonpathogenic on tobacco), but it was activated only slightly and in a delayed fashion by the tobacco pathogen P. syringae pv. tabaci (pathogenic on tobacco), implying that the product of CYP82E1 may be involved in disease resistance in tobacco.

Accumulation of a fusion protein containing 2S albumin induces novel vesicles in vegetative cells of Arabidopsis.

We have previously reported that precursor-accumulating (PAC) vesicles found exclusively in developing seeds are involved in a transport of seed storage proteins, such as 2S albumin, from the endoplasmic reticulum to protein-storage vacuoles. Here, we constructed chimeric genes that encode fusion proteins consisting of both various lengths of polypeptides derived from pumpkin 2S albumin and a selectable marker enzyme, phosphinothricin acetyltransferase. The chimeric genes were expressed in transgenic Arabidopsis in order to investigate the mechanism of the PAC vesicle formation. A fusion protein expressed by one of the chimeric genes is accumulated as a proprotein-precursor form, and localized in novel vesicles of vegetative cells. The vesicles show distinct features that well much to the PAC vesicles. Despite of the accumulation of the fusion protein, the transgenic Arabidopsis is still sensitive to phosphinothricin. Phosphinothricin acetyltransferase contained in the fusion protein is obviously compartmentalized in the PAC-like vesicles that do not permit the detoxification of this herbicide. These results indicate that the PAC-like vesicle can be induced in vegetative cells by the ectopic expression of the protein that is destined to be compartmentalized into the PAC vesicles.

Oligomeric proteins containing N-terminal targeting signals are imported into peroxisomes in transgenic Arabidopsis.

Employing transgenic Arabidopsis plants, we analyzed the mechanism for the translocation of peroxisomal proteins from the cytosol into the matrix that is mediated by the N-terminal targeting signal. A hybrid Arabidopsis variety was generated which accumulates two kinds of originally bacterial proteins, beta-glucuronidase (GUS) and a GUS chimeric protein designated as CS-delta C42-GUS, that carries the N-terminal targeting signal for glyoxysomal citrate synthase. Because the CS-delta C42-GUS is targeted to peroxisomes but had never been observed to be processed to produce the mature protein, it can be distinguished from the GUS protein by its molecular size. Cell fractionation analyses showed that the native GUS protein, although lacking the targeting signal, was co-localized with the CS-delta C42-GUS protein in the peroxisomes of the hybrid plant. It is suggested that the native GUS protein forms oligomeric structures with the peroxisome-targeted chimeric proteins and can therefore be transported into peroxisomes. Sucrose density gradient centrifugation revealed that the native GUS and the chimeric GUS indeed are present both as a dimer and a tetramer in the Arabidopsis hybrid variety.

Changes in platelet ATP secretion and aggregation during pregnancy and in preeclampsia.

BACKGROUND: Platelet secretion plays an important role in the aggregation of platelets. However, the quantitative relationship between platelet aggregation and secretion of ATP during pregnancy and in pre-eclampsia has yet to be clarified. This study is designed to determine whether platelet count, volume, aggregation, and the amount of secreted ATP change in healthy, nonpregnant women, nonpreeclamptic pregnant women, and preeclamptic pregnant women and whether beta-thromboglobulin (BTG) and platelet factor 4 (PF-4) concentrations alter in nonpreeclamptic and pre-eclamptic women. METHODS: Peripheral blood was collected from 114 women. Nonpreeclamptic pregnant women were divided into four groups (gestational weeks 10, 20, 30, and 35). Platelet aggregation and ATP secretion were investigated with the use of a lumi-aggregometer. BTG and PF-4 concentrations in peripheral blood were determined in 12 pregnant and 11 preeclamptic women. RESULTS: The amount of secreted ATP upon induction by 5 microM ADP increased significantly (P < 0.05-0.01) with gestational age. On the other hand, the amount of secreted ATP induced by 5 microg/mL collagen reached the maximal value from gestational weeks 20 to 35 in nonpreeclamptic women. Significantly more platelet aggregation was induced by the ADP and collagen in nonpreeclamptic women in gestational weeks 20 and 30 than in the gestational weeks 10 or 35 (P < 0.05-0.005). The amount of secreted ATP and platelet count were significantly lower (P < 0.05) in preeclampsia than in normal pregnancy. The BTG and PF-4 concentrations were significantly higher (P < 0.05) in preeclampsia than in normal pregnancy. CONCLUSIONS: The sensitivity of platelets for ATP secretion may intensify with progression of pregnancy. In normal pregnancy, around gestational week 35, the platelets may exhibit weaker ability to aggregate but maintain the capacity to secrete ATP. In preeclampsia, secreted ATP decreased because platelets may be stimulated to undergo a partial secretion.
