[Paramedical staff knowledge and practice related to the blood transfusion safety].

Blood transfusion is a complex activity, involving many actors. It is a high risk activity which couldn't be controlled without the use of specific methods. Health care workers beliefs and organisational factors are two major issues for the blood transfusion safety. Our study objectives were to describe the paramedical staff's knowledge and practice regarding blood transfusion safety and to identify factors that are related to them. We carried out a cross sectional study. The information was gathered by using a questionnaire. The latter was developed by foreign teams and adapted to the local context. Two outcome measures were used: the knowledge and practice score and the proportion of true answers. The study showed that only 15% of the interviewed persons have had a score less than 30 (scale range from 0 to 100), i.e. only 15% have had appropriate knowledge and practice with no negative consequences for the patient safety. 13.8% of the study population provided right answers related to the biologic exams required before red cells transfusion and 34% for the abnormal reaction circumstances. These results underscores the importance of strategies improving the quality and the safety of blood transfusion, i.e. the continuous medical education, implementing a blood transfusion information system and the use of transfusion practice guidelines.

[Intra-individual variability of the platelet anti-aggregation effect of aspirin in coronary patients].

UNLABELLED: Several studies have reported a biochemical resistance to aspirin in 5 to 10% of coronary patients. However, the stability of the platelet anti-aggregation effect with aspirin over time remains poorly understood. OBJECTIVE: To study the intra-individual variability at 6 months of the anti-platelet action of aspirin in coronary patients. METHOD: Prospective study including 40 consecutive patients with acute coronary syndrome and taking regular aspirin (250 mg a day). The biochemical impact of aspirin was determined by measuring the time to occlusion (TO) on a collagen/epinephrine cartridge with PFA-100. The determination of the TO was performed 2 months (TO1) and then 8 months (TO2) after starting aspirin. In our population, a resistance to aspirin was defined as a TO < or =125 sec. RESULTS: The median value for TO was generally stable over the two periods, at 158 sec for TO1 and 179 sec for TO2 (p = 0.29). Among the 9 initially resistant patients (22.5%), 4 became sensitive to aspirin without changing the dosage, while only one of the 31 initially sensitive patients became biochemically resistant. CONCLUSION: the existence of a medium term intra-individual variability in the antiplatelet response to aspirin in coronary patients underlines the importance of biochemical surveillance in these high vascular risk patients.

[Malignant otitis externa (2 cases)].

Malignant external otitis is a progressive necrotizing otitis. It's a rare severe and evolutive clinical entities, old diabetics are the most victim. Pseudomonas aeruginosa is the bacteria responsible in the most cases. Prognostic vital can be affected, treatment must be energic, rapid and well adapted. The authors report two old diabetic women presenting a malignant external otitis and discuss etiopathogenic, physiopathologic, diagnosis and treatment of this illness.

[Activity of fourteen antibiotics on Vibrio cholerae el tor].

Two hundred and one strains of Vibrio cholerae el tor were tested by agar diffusion technic against fourteen antimicrobial agents. All the strains tested are inhibited by tetracyclines, gentamycin, tobramycin, kanamycin, chloramphénicol, carbenicillin and the association of trimétomycin but mainly the colistin are the less active. A great majority of strains are inhibited by the ampicillin. The streptomycin but mainly the colistin are the less active.

[Evaluation of Salmonella 1977 to 1982].

Between 1977 and 1982, the National Center of Salmonella of the Institute Pasteur of Tunis has isolated; received and/or identified 1715 Salmonella strains. In typhoid and paratyphoid fevers group Salmonella typhi represent the predominant species with a frequency of 99.6%. In the enteric group, Salmonella wien is the most frequent (50,26%). 11 serotypes appeared in 1982, although 5 serotypes have not been met since 1977.
