Vaginal semiradical hysterectomy: a new operative procedure for microinvasive carcinoma of the cervix.

Thirty-five patients with stage Ia epidermoid microinvasive carcinoma of the cervix were treated surgically by vaginal semiradical hysterectomy. The primary features of this new surgical procedure are the removal of an approximately 1-cm portion of vaginal cuff, partial resection of the vesico-uterine ligament for isolation of the ureter, and resection of approximately 2 cm of parametrial tissue. The extent of resection is almost the same as that of the abdominal modified radical hysterectomy. Operation time and blood loss were almost the same as those for simple hysterectomy. No postoperative complications were observed. From these results it is concluded that the method is an adequate, safe, and low-risk operative therapy for patients with microinvasive carcinoma of the cervix.

[Disposition and metabolism of 14C-dehydrocorydaline in mice and rats].

Dehydrocorydaline, an alkaloid from Corydalis bulbosa possessing anti-ulceric activity, was labeled by methyl-14C at position 13, and its disposition and metabolism were studied in mice and rats after oral (50 mg/kg) or intravenous (6.8 mg/kg) administration. Blood levels were as low as 0.5 (mice) and 0.06 (rats) microgram eq./ml at maximum, conceivably due to its poor absorption from digestive tracts. Radiometric and autoradiographic studies after oral administration of 14C-dehydrocorydaline revealed that appreciable radioactivity was distributed in rather restricted organs, namely, digestive tracts, liver and kidney. Comparison of distribution with that after intravenous administration suggested that the compound is subject to the significant first pass effect by the liver. The distribution pattern of radioactivity in the liver was heterogeneous regardless to administration routes. Radiometry and autoradiography also revealed that the disposition did not differ significantly from each other in normal rats and those with experimentally induced gastric ulcer. Excretion of radioactivity occurred largely in feces. Excretion via urine and bile was quite minor. Metabolite analysis suggested that dehydrocorydaline was metabolized by O-demethylation and subsequent conjugation with glucuronic acid. However, metabolites were minor relative to the unchanged compound.

[A clinical observation on pyeloplasty in obstruction of the ureteropelvic junction. Special consideration on Anderson-Hynes pyeloplasty].

Thirty-three patients (thirty-five kidneys) underwent pyeloplasty for hydronephrosis due to obstruction of ureteropelvic junction between 1963 and 1982 in our department. Renal function was judged to have improved in 68 and 67% of the patients from the radiogram and radioisotope renogram, respectively, Anderson-Hynes pyeloplasty was performed on 27 kidneys. In these kidneys, the improvement of renal function as judged from the radiogram and radioisotope renogram was 74 and 71%, respectively. Postoperative results were compared according to age, preoperative urinary tract infection (UTI), indwelling periods of nephrostomy and splint catheter, and duration of postoperative UTI. In addition the relationship between indwelling periods of nephrostomy catheter and the duration of postoperative UTI are discussed. Infants under 10 years old had the best improvement. Preoperative UTI and indwelling periods of nephrostomy and splint catheter were not directly related with post operative results. The cases with nephrostomy catheter which could be removed within the 16th day after operation had excellent improvement on UTI and also in renal function.

A coding system for a computer aided dental identification based on the "Canobrain Programme".

Three situations requiring the identification process are considered, that is, (Situation 1) identification of air disaster victims, (Situation 2) identification of an unknown body, and (Situation 3) identification of a missing person. In this paper, a coding system using "Canobrain Programme" in each situation is described. It employs 3 digits and 2 letters in situation 1 or 4 digits in situations 2 and 3 to designate the status of a tooth surface and the nature of the dental materials used in any restoration. Illustrative examples of the trial of this system in situation 1 and 2 are set out.

Morphological analysis of the fascial lamination of the trunk.

For a fuller understanding of the fascial relationships of the visceral organs and their vessels, a general scheme is presented. According to this scheme, there are four basic layers internal as well as external to the trunk musculature. In the abdomen, the internal layers (numbered outwards) consist of (1) the peritoneum, (2) the deep layer of the subperitoneal fascia, (3) the superficial layer of the subperitoneal fascia and (4) the transversalis fascia, while the external layers (numbered inwards) comprise (1) the skin, (2) the superficial layer of the subcutaneous fascia, (3) the deep layer of the subcutaneous fascia and (4) the investing layer of the abdominal fascia. Each interfascial space between layers 2 and 3 gives passage to the main vessels and nerves and will be referred to as the "neurovascular corridor." By the addition of some slight modifications to the basic scheme, the fascial relationships of some organs is illustrated. Photographs of dissections, substantiating the schemes are also included. The schemes and dissections together demonstrate three points: The mammary gland to be situated within the neurovascular corridor, external to the thorax, That the layers of the renal fascia, comprising layers 2 and 3, continue into the pelvic cavity to form the fascia around the ureter and urinary bladder, An outline arrangement of the vascular sheath in the pelvis.

Determination of a new antibacterial agent (AT-2266) and its metabolites in plasma and urine by high-performance liquid chromatography.

A high-performance liquid chromatographic method has been developed which enables accurate determination of a new synthetic antibacterial agent, AT-2266, and its metabolite, M-2, in plasma, and AT-2266 and its five metabolites, M-1, M-2, M-3, M-4 and M-5, in urine. AT-2266 is extracted as ethyl carbamate with chloroform containing 1% ethyl chloroformate and assayed on a liquid chromatograph equipped with an ultraviolet detector at 340 nm. Accurate determinations are possible over a concentration range of 0.1-10 micrograms/ml AT-2266 in plasma, and 1-500 micrograms/ml AT-2266 in urine. The coefficient of variation at the 2 micrograms/ml level of AT-2266 is 1.9% (n = 6). The minimum detectable concentrations of AT-2266 in plasma and urine are 0.01 micrograms/ml and 0.1 micrograms/ml, respectively, and those of other metabolites are similar to those of AT-2266. Plasma levels and urinary excretion of AT-2266 in a man following single oral administration (400 mg) have also been determined.

Aortic fibrous components in exercised rats.

In exercised female rats, the elastin content of the thoracic and abdominal aorta decreased by 4-8% (p less than 0.05). The collagen content in the thoracic aorta, was unchanged but in the abdominal aorta was reduced by 5.2% (p less than 0.05). These results are discussed in connection with physical training.

A new physiological tool for assessing anxiolytic effects in humans: frontal midline theta activity.

The theta activity which appears in the frontal midline area during performance of mental tasks has been designated as "Fm theta". In the present study, the possible use of zopiclone as an anxiolytic agent was investigated using Fm theta, in a comparison with diazepam. Sixteen male university students were given placebo, diazepam 5 mg, zopiclone 5 mg and 10 mg, in a double-blind, cross-over design. EEGs were recorded before and during performance of an arithmetic addition test. This test was repeated twice, prior to and 1 h following drug administration. Diazepam markedly increased Fm theta; however, both zopiclone 10 mg and placebo only slightly affected Fm theta, while zopiclone 5 mg failed to affect Fm theta. Scores of state anxiety were reduced markedly by diazepam and only slightly by zopiclone 10 mg, but were not affected by either zopiclone 5 mg or by placebo. Task performance was reduced markedly and in a dose-dependent manner by zopiclone, and very slightly by diazepam. These results suggest that zopiclone possesses a rather potent hypnotic effect, rather than an anxiolytic effect, and that Fm theta might be a useful tool in predicting the clinical efficacy of anxiolytic drugs in normal humans.
