[Ectopic production of amylase by a non-producing type of multiple myeloma].

The patient was a 64-year-old woman who was admitted to our hospital because of lumbago. A diagnosis of multiple myeloma (non-producing type) was made, based on (1) the presence of multiple osteolytic lesions, (2) hypercellular marrow with 64.2% plasmacytoid malignant cells, (3) no monoclonal gamma-globulin was detected in the serum and urine, and (4) abnormal monoclonal gamma-globulin was also not detected in the cytoplasm and membranes of these malignant cells. After several courses of chemotherapy, a pleural effusion infiltrated by myeloma cells developed and the patient's serum contained a markedly increased amylase activity of salivary-type. Amylase activity was also detected in vitro in the supernatant of cultured myeloma cells established from the patient's pleural effusion. The presence of alpha-amylase in the myeloma cells, which were derived from pleural effusion, was demonstrated immuno-histochemically. These observations indicates that amylase was ectopically produced by these myeloma cells. Interestingly, 14 out of 20 metaphases in the cells derived from pleural effusion showed translocation of 1p22 near the region of 1p21, where the amylase gene was assigned.

[Studies on immunoreactive somatostatin and gastrin contents of the gastric mucosa in patients with duodenal ulcer--comparison to patients with fundic gland polyposis and normal subjects].

The immunoreactive gastrin (IRG) and somatostatin (IRS) contents in gastric mucosa were measured from the same biopsy specimen of the same patients with duodenal ulcer (DU) at the active stage and healing stage, and compared to those of patients with fundic gland polyposis (FP) and endoscopically normal subjects whose gastric mucosa had only slight atrophic change (Control). The IRS in both the antrum and the gastric body of DU were significantly lower than those of the other two groups, and those showed no difference between the two stages. In all groups, there was a significant positive relation between the IRG and IRS in the antrum. In DU, particularly at the active stage, the relative decrease of the IRS against the IRG was prominent compared to the other two groups. In FP, which has similar background gastric mucosa and ability of acid output to those of DU, it was found that somatostatin was secreted sufficient to control gastrin secretion and acid output. Whereas in DU, secretion of somatostatin was reduced and, particularly at the active stage, it was considered that somatostatin, which could control increased gastrin secretion and increased acid output, was not secreted.

[Idiopathic thrombocytopenic purpura complicated with circulating lupus anticoagulant].

A 37 year-old female was admitted to our hospital because of hypermenorrhea, prolonged bleeding time, thrombocytopenia and the diagnosis of idiopathic thrombocytopenic purpura (ITP) was made. Though activated partial thromboplastin time (APTT) was markedly prolonged, her coagulation factors were within normal ranges. Activities of the circulating lupus anticoagulant (LAC) was suggested. Kaolin clotting time of the platelet poor plasma was used as a sensitive screening test using the mixture of normal and patient's plasma for the detect of LAC. As a result, LAC positive pattern was observed. The treatment with high-dose gammaglobulin brought out a transient increase of the platelet count, but the prolongation of APTT was not corrected. Both the platelet count and the prolongation of APTT were significantly improved after the treatment with betamethasone.

[Effects of verapamil and disopyramide phosphate on the termination, reinduction and long-term prevention of paroxysmal supraventricular tachycardia].

The efficacy of verapamil and disopyramide phosphate for the termination and prevention of paroxysmal supraventricular tachycardia (PSVT) were studied electrophysiologically in 32 patients with inducible sustained PSVT (17 patients received verapamil, 15 patients received disopyramide). Twelve patients had atrioventricular nodal tachycardia, 7 had concealed and 13 had overt Wolff-Parkinson-White syndrome. Intravenous verapamil (0.15 mg/kg) terminated the sustained PSVT in 15 of the 17 patients (88%) by production of AV block in 13 patients, VA block in one, and a ventricular premature beat in one. PSVT could not be induced in any of these 15 patients after they had received verapamil. In the remaining 2 patients, PSVT could not be terminated by the use of verapamil, but the cycle lengths of PSVT were lengthened. Long-term oral dosages of verapamil of 120-240 mg/day were administered in 13 of the 17 patients. All patients except two, whose PSVT was unable to be effected by intravenous verapamil, were well controlled: PSVT disappeared in 7 patients and decreased in 4. Intravenous disopyramide (1.5 mg/kg) terminated induced PSVT in 10 of the 15 patients (67%) by production of VA block. Although PSVT could not be reinitiated in 5 of these 10 patients, non-sustained PSVT was induced in 2 and sustained PSVT was induced in 3 after having received disopyramide. PSVT was induced in all of the 5 patients who failed to respond to disopyramide. The cycle lengths of PSVT after administration of disopyramide remained unchanged.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of maxillary protrusion].

Achievement of best balance and facial harmony is the main goal of orthodontic treatment, and it is especially important to retract the lip in case of maxillary protrusion. A 11-year and 7-months-old girl was first seen with chief complaints of maxillary protrusion. We extracted 4/4 4/4, treated the patients with the Vari-Simplex-Discipline for 43 months, and evaluated changes in facial profiles. We found, that excessive lip protrusion was eliminated and good interdigitation obtained. In addition to, angle ANB and overjet were corrected to ideal conditions.

[Mode of hypertrophy in hypertrophic cardiomyopathy based on the external shape of the left ventricle: a two-dimensional echocardiographic study].

To assess the mode of left ventricular hypertrophy in hypertrophic cardiomyopathy (HCM), two-dimensional echocardiography was performed in 15 patients with asymmetric septal hypertrophy and in 13 normal subjects. The long-axis diameter (LxD) was defined as the distance from the midpoint between the anterior annulus of the aortic valve and the posterior annulus of the mitral valve to the apex in the apical long-axis image. In the parasternal long-axis image, the long axis of the left ventricle (LV) was determined by overlaying the parasternal image on the apical image. The external diameter (ExD) and the internal diameter (InD) of the LV at chordal level were measured in the direction of the short axis. The anterior part (-a) and the posterior part (-p) of both ExD and InD, which were divided by the long axis, were also measured. LxD, ExD, ExD-a, ExD-p and InD-p in HCM were greater than those in the normal subjects. Thus, the interventricular septum (IVS) in HCM was thickened inwardly and outwardly. In contrast, the left ventricular posterior wall (PW) in HCM showed only outward deviation. Thickness of the IVS and PW, and the excursion of the PW in HCM were greater than those in the normals, but the excursion of the IVS in HCM was smaller than those in the normals. We concluded that outward expansion of the IVS and PW occurs in HCM and distinct narrowing of the LV occurs only on the septal side in HCM.(ABSTRACT TRUNCATED AT 250 WORDS)

[Hemodynamic responses to sublingual nifedipine during exercise and recovery in congestive heart failure].

To elucidate the hemodynamic responses to sublingual nifedipine administration during exercise and recovery in congestive heart failure, 16 patients with dilated cardiomyopathy (DCM) and 11 controls were studied using supine bicycle exercise testing. The cardiac index (CI) was measured at rest, at peak exercise and successively during recovery by the thermodilution method. The heart rate (HR), mean blood pressure (mBP) and mean pulmonary arterial pressure (mPAP) were measured every minute. The same exercise load and hemodynamic measurements were repeated about 30 min after the sublingual administration of 10 mg nifedipine. The recovery slope in each parameter was analyzed using an exponential function (Cp = C1ekt). The regression coefficient in each parameter was defined as a CI slope, an HR slope, an mBP slope and an mPAP slope. Before the administration of nifedipine, CI at peak exercise was lower, mPAP during recovery was persistently higher and the CI slope was more blunt in DCM patients, compared with those of the controls. With the administration of nifedipine, CI and HR increased or tended to increase, mBP decreased throughout the exercise and recovery, and mPAP decreased at rest, at peak exercise and one min after exercise in the controls. In DCM patients, however, the CI increased, mBP and mPAP decreased nearly throughout the exercise and recovery, while HR increased throughout the exercise and recovery, except at peak exercise. The CI, HR and mPAP slopes were blunted in the controls; whereas, each slope showed no change in DCM patients. These results suggest that sublingual nifedipine administration can reduce both the preload and afterload, and increase CI throughout exercise and recovery without significant alteration in the recovery course.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of Nd:YAG laser irradiation on acid resistance of defective rat enamel].

As Colchicine is an inhibitor of the function of microtubles, administration of it causes abnormal amelogenesis in the teeth of experimental animals. The purposes of this study were to characterize the defective rat enamel which was formed after a single administration of colchicine, and to examine the effects of Nd:YAG laser with continuous wave on the acid resistance of the defective enamel. The following results were obtained: 1) On about the 10th day after a single subcutaneous injection of colchicine with a dose of 1.5 mg/kg of body weight, the defective enamel was found on the incisor as white bands with rough surface. 2) The defective enamel had less resistance to the acid demineralization than that of normal enamel. 3) After irradiation of Nd:YAG laser with continuous wave, the defective enamel surface became smooth and the small defects on the enamel surface disappeared. 4) After irradiation of Nd:YAG laser, the resistant level of the defective enamel to the acid demineralization increased up to that of normal enamel. The results suggest that Nd:YAG laser irradiation on human teeth with hypomineralized enamel may be effective for the prevention of dental caries.

[Study of tissue drug concentration of fluorinated pyrimidine anticancer drugs--comparison of 5'DFUR and UFT].

In order to verify the tumor-selective toxicity of fluorinated pyrimidine anticancer drugs, we conducted an investigation of the clinical pharmacology of two of these drugs, 5'DFUR and UFT. 5'DFUR was administered to 8 patients and UFT to 8 other patients in respective dosages of 800-1,000 mg/day and 400-600 mg/day an average of 6 days before the patients underwent surgery for cancer of the large bowel, and the concentrations of these drugs in tissue were then measured. No 5'DFUR whatsoever was detected in serum, the lymphnodes, normal large bowel tissue, or cancerous large bowel tissue after administration of this drug. Moreover, levels of the active substance, 5-FU, after administration of 5'-DFUR were low in serum, the lymphnodes, normal large bowel tissue (0.033 +/- 0.024 microgram/g), and cancerous large bowel tissue (0.034 +/- 0.020 microgram/g), and no difference was observed between normal and cancerous large intestine tissue. On the other hand, tegafur was detected in all of the tissues following the administration UFT, and the concentration of 5-FU was significantly high, particularly in cancerous large bowel tissue (0.108 +/- 0.057 microgram/g) compared to normal sites (0.044 +/- 0.048 microgram/g) (p less than 0.05). The above results indicate that UFT is a promising drug for use in chemotherapy for cancer of the large bowel.

[Clinical electrophysiological effects of nicorandil on the conduction system in humans].

The electrophysiological effects of nicorandil on the conduction system in humans were examined. Twelve patients with sick sinus syndrome (SSS), 3 patients with AV block and 3 subjects with normal conduction system were studied. The clinical electrophysiological effects were examined before, and 5 min after intravenous administration of nicorandil 2 mg over 3 min. Nicorandil increased heart rate in 6 patients and decreased it in 4. Wenckebach point was increased in 5 patients and decreased in 4. Maximum sinus node recovery time was prolonged in 4 of 12 patients with SSS, and shortened in 3 of them. It was unchanged in 6 patients without SSS. These changes were not statistically significant. There were no changes in PA, AH and HV intervals and refractory periods the atrium, the AV node and ventricle. Nicorandil had no depressant effects on the conduction system of humans. Thus, this drug can be used easily for the treatment of ischemic heart disease in patients with some conduction disturbances.

[Investigation into the actual condition of physically handicapped children--specially the consciousness in parents].

The purpose of this paper was to investigate the dental situation of children who visited the pedodontics clinic of Nihon University at Dental-Hospital during the period from Jan. 1983 to Dec. 1987. The subject is the awareness in parents of 68 children who carried out the administration of medical care. The results were as follows. 1) Recently we observed that the ratio of physically handicapped children was was on the increase among the first visit patients. 2) The principal diseases of the physically handicapped children were cyclothymic, allergy and blood dyscreasia. 3) The majority of the patients visited for receiving caries treatment. 4) We presumed that the parents of the physically handicapped children showed deep concern for oral diseases.

[The sensitivity of in situ hybridization for detection of human papillomavirus].

We studied a sensitivity of HPV DNA detection by in situ hybridization method using 3H labeled HPV DNA. The materials were CaSki cells and SiHa cells which were derived from as a negative control. The total cellular DNAs extracted from these cell lines were estimated copy numbers of HPV 16 DNA using Southern blot hybridization. In our result, CaSki cell has 400 copies/cell, SiHa cell were appeared to have 1-5 copies/cell. Simultaneously these cells were fixed by periodate-buffered lysine-paraformaldehyde-glutaraldehyde (PLPG) and were detected HPV 16 DNA using in situ hybridization. We detected HPV 16 DNA in CaSki cells and SiHa cells by in situ hybridization also. We concluded that the sensitivity of our in situ hybridization technique is 1-5 copies/cell.

[The diagnostic critical value in DFO low dose loading test on patient with aluminum accumulation compared to each parameter of ROD].

One hundred two hemodialyzed patients were examined to determine the standard level of delta Al value which is the difference of serum Al concentrations between pre and post DFO loading test. We applied low dose of DFO 15 mg/kg in this loading test. Some significant negative correlations were found between delta Al and MCI, sigma GS/D, osteocalcin, free-Hydroxyproline (dialysate) and free-gamma-Carboxyglutamic acid (dialysate). Each correlation rate was -0.58, -0.44, -0.76, -0.57 and -0.51 respectively. In addition tendencies of correlation were found between delta Al and ALP and between delta Al and %QCT (QCT/mean QCT in patient's age x 100). And statistical significant differences were found between (0 less than delta Al less than or equal to 150 micrograms/l) group and (150 micrograms/l less than delta Al) group in each osteobiochemical parameter. These results indicate that 150 micrograms/l is the lower diagnostic standard level of delta Al in 15 mg/kg DFO loading test.

[Busulfan lung exacerbated during steroid therapy: a review of Japanese literature].

A 61-year-old male was diagnosed as chronic myelocytic leukemia (CML) in 1985 and had been treated with busulfan for 4 years and 5 months (total 3,572 mg). Since he had complained dyspnea with abnormal lung shadow on a chest x-ray film in May 31, 1989, he was treated with 3 mg/day of dexamethasone under a diagnosis of busulfan lung. Dyspnea and lung shadow was remarkably improved after the treatment. On July 11, he was admitted our hospital with high fever, hypoxemia and diffuse interstitial shadow on a chest x-p. Although intensive treatment with 10 mg/day of dexamethasone and various antibiotics were done, he died in Aug 6. An autopsy revealed the intra-alveolar fibrosis, dense hyaline deposits in alveoli associated with atypical type II epithelial cells in the lung. Seven cases of busulfan lung have been reported in the Japanese literature. The clinical and pathological aspects of these cases, including our case, were discussed.
