WS9326A, a novel tachykinin antagonist isolated from Streptomyces violaceusniger no. 9326. II. Biological and pharmacological properties of WS9326A and tetrahydro-WS9326A (FK224)

WS9326A binds competitively to [3H]substance P (NK-1 receptor) binding sites on guinea-pig lung membranes (IC50 = 3.6 x 10(-6) M), and acts as a tachykinin antagonist in various functional assays. WS9326A inhibited tracheal constrictions produced by exogenously added substance P and neurokinin A, with IC50 values of 9.7 x 10(-6) M and 3.5 x 10(-6) M, respectively. WS9326A inhibited neurokinin A-induced bronchoconstriction in a dose dependent manner when administered to guinea-pigs intravenously together with neurokinin A, and was also effective in preventing capsaicin-induced bronchoconstriction, which is known to be caused by release of endogenous tachykinins (substance P and neurokinin A). FK224 (tetrahydro-WS9326A; catalytic hydrogenation of WS9326A gave FK224) was more potent than WS9326A in the [3H]substance P receptor binding assay using guinea-pig lung membrane (IC50 = 1.0 x 10(-7) M).

WS-7338, new endothelin receptor antagonists isolated from Streptomyces sp. No. 7338. I. Taxonomy, fermentation, isolation, physico-chemical properties and biological activities.

WS-7338 A, B, C and D, novel endothelin receptor antagonists, have been isolated from fermentation broth of Streptomyces sp. No. 7338. These antagonists were purified from the culture mycelium by extraction with acetone, followed by carbon column chromatography and HPLC. Among them, WS-7338 B showed good activity in an endothelin receptor binding assay with an IC50 of 2.7 x 10(-7) M.

WS-7338, new endothelin receptor antagonists isolated from Streptomyces sp. No. 7338. II. Biological characterization and pharmacological characterization of WS-7338 B.

WS-7338, produced by Streptomyces sp. No. 7338, was found to be a competitive and specific antagonist against ET-1 receptors in in vitro studies and WS-7338 B is also active in vivo. Furthermore, WS-7338 B was a specific antagonist for vascular ET-1 receptors (ETA receptors) and significantly prevented the accumulation of intracellular inositol 1,4,5-triphosphate (IP3) in endothelin treated rat aorta tissues.

Measurement of serum IgE antibodies against Japanese cedar pollen (Cryptomeria japonica) in Japanese monkeys (Macaca fuscata) with pollinosis.

IgE antibodies against allergens of Japanese cedar (Cryptomeria japonica, CJ) pollen in the serum of seven Japanese monkeys (Macaca fuscata) with pollinosis were measured by fluorometric indirect enzyme-linked immunosorbent assay (ELISA). All of the monkeys were found to have specific IgE to the crude pollen antigen. The specific IgE levels were well correlated with those determined by the Pharmacia CAP system. IgE antibodies were then assayed with two kinds of purified allergens (Cry j I and Cry j II) by the ELISA. We found that five monkeys had specific IgE to both allergens, although the other two had IgE only to Cry j I or Cry j II; there is different immune responsiveness to the two major allergens in the monkeys.

[Repeated arterial infusion chemotherapy for inoperable hepatocellular carcinoma using implantable drug delivery system].

We performed a clinical evaluation of repeated arterial infusion chemotherapy using an implantable drug delivery system for 41 patients with inoperable hepatocellular carcinoma (HCC). About half of our patients could not undergo transcatheter arterial embolization (TAE) because of extreme tumor extension and/or accompanying advanced cirrhosis. In most patients we implanted a 5 Fr. catheter non-surgically and connected it to an implanted injection port through a subcutaneous tunnel. The treatment schedule was weekly or biweekly intrahepatic one-shot administration of mitomycin C, adriamycin, 5-fluorouracil and epirubicin. The response rate (CR + PR) was 24.4%. The median survival period was 401.1 days. The 6 month, 1-year and 2-year survival rates were 73%, 48% and 24%, respectively. There were no severe side effects nor complications. The implantable drug delivery system will contribute not only to improved therapeutic efficacy for inoperable HCC but also improve the quality of life for patients.

Object losses and resolutions of women in middle adulthood.

Women in middle adulthood frequently experience loss of objects during this life stage, and thus their psychiatric disorders are often related to object losses. This paper presents three cases of women in middle adulthood whose onset of illness was precipitated by object loss and discusses the psychodynamics, nature of their object loss and mourning process. All three cases presented symptoms of depressive anxiety, paranoid reaction and hypochondriacal anxiety, however, the onset of their illness was related to losses of objects. In addition, it was found that they had not resolved grief towards other losses in the past which preceded the object loss that precipitated the illness. Furthermore, the patients found new objects after completing their mourning process. However, these were not new objects, but rather a rediscovery of objects which they had lost in the past. Such findings are thought to be useful, not only in the treatment of women in middle adulthood, but also in elucidating psychiatric disorders which are precipitated by loss of objects.

Resolution of developmental tasks pertaining to prolongation of adolescence.

The establishment of ego identity is a developmental task of primary importance in adolescence. Adolescence is considered to be a period of psychosocial moratorium for the establishment of ego identity and incorporation into adult society. Some youths are known to consciously or unconsciously extend this period of psychosocial moratorium in order to escape incorporation, prolonging the resolution of developmental tasks associated with adolescence. This paper discusses the psychodynamics of the prolongation of adolescence through presentation of the therapeutic consultation of a male patient in his early 30's prolonging establishment of ego identity, or in particular, that of work identity. Analysis in this case revealed that the prolongation of adolescent mentality was deeply related to the wound of narcissism peculiar to adolescence. Additional comments are given on the therapeutic mechanism of therapeutic consultation.

Therapy of a case of psychotic personality organization manifesting gender identity disorder.

The author relates his experience with a case of psychotic personality organization manifesting transsexualism spanning about 10 years. This paper presents the case and discusses the psychodynamics together with the course of therapy. The patient was a case of primary transsexualism who fell into hallucinatory-delusional states each time he seriously tried to become a woman. His transsexual wish was identification with his idealized grandmother image, for defense of his helpless and vulnerable self, and an attempt to reestablish his self which was facing a crisis of disintegration due to disorganization of his ego. Over about 4 years of psychoanalytic psychotherapy, the conflict between "becoming a woman" and "taking over as head of the house" was an integral issue. The psychotherapy was brought to a tentative conclusion by the patient giving up the idea of "becoming a woman" and deciding to "take over as head of the house".

[A case of advanced breast cancer responding to low-dose 5'-DFUR].

A 69-year-old female patient with locally advanced breast cancer with multiple bone metastases (T4cN2-M1) was treated with 5'-DFUR after failure of tamoxifen treatment. The 5'-DFUR dose was limited to 600 mg per day, half the usual dose, because of its gastroenterological side effects. The primary tumor and metastatic axillary lymph nodes regressed three months after the start of treatment, and the complete remission of lesions was obtained in 16 months. Reduction of bone metastasis was confirmed by the disappearance of osteolytic lesion in the skull roentgenogram and the decrease of abnormal accumulation on the bone scintigram. Serum levels of CEA and ST-439 markedly reduced during the treatment as the disease regressed.

[A ruptured aneurysm at the peripheral collateral circulation of the anterior choroidal artery in a patient with moyamoya disease: a case report].

This 42-year-old man experienced a sudden onset of occipital headache. Neurological examination revealed a moderately disturbed consciousness and a moderate left hemiparesis. CT scan disclosed a hugh hematoma in the right temporo parietal lobe without intraventricular hemorrhage. A cerebral angiography demonstrated typical findings of moyamoya disease and a small saccular aneurysm at the peripheral portion of the right anterior choroidal artery, which was dilated at the collateral circulation to the parietal lobe. The hematoma was removed at once by a craniotomy. He became alert but mild hemiparesis persisted. MRI disclosed a small signal-void lesion lateral to the trigone of the right lateral ventricle. The angiography repeated three weeks after the removal of the hematoma showed the unchanged size of the aneurysm. Direct surgery for the aneurysm was performed via the right parietal transcortical approach. The aneurysm was reached under the guidance of the intraoperative angiography. Trapping of the parent artery and the excision of the aneurysm were performed. On the basis of the presence of an internal elastic lamina at the neck of the aneurysm, the surgical specimen was histologically verified to be a true aneurysm. Since the collateral circulation was well preserved during surgery, no worsening of the neurological manifestation was observed. In view of the unfavorable prognosis for a moyamoya patient with this type of the aneurysm, which often results in a massive ventricular or intracerebral hemorrhage, surgery directed to the aneurysm itself should be considered.

[Studies of clinical usefulness of new tumor markers of ovarian cancer, CA 54/61 and CA 602--CA 602 assay reagent kit, performance its normal value and correlations with other tumor markers].

We made a preclinical study of a newly developed tumor maker, CA 602, and its clinical study using serum samples available from 58 institutions located throughout Japan. In the preclinical study, a CA 602 assay kit was investigated for the reproducibility and precision of assay results; and in the clinical study, the kit was investigated for the normal value of the marker, for variations in assay result with age, menstrual cycle and term of pregnancy, and for correlations of assay results with those of other tumor markers. The tests of the kit for simultaneous reproducibility and interval reproducibility of assay results, and the results of analytical recovery and dilution tests were all favorable; the kit proved to be reliable in both precision and reproducibility. For the study, 2 cutoff levels were set: mean + 2 SD of healthy subjects, i.e., 63 U/ml, and the level which permits the maximal efficiency of differential diagnosis of benign from malignant ovarian tumors, i.e., 90 U/ml. The assay results showed that CA 602 levels were low in women aged 50 and over; the levels were high in the first half of pregnancy, and also high in the menstrual period to the early follicular phase. The assay results of CA 602 also proved to be intimately correlated with those of CA 125, which suggested that the 2 markers might be analogous to each other. CA 602 proving to be of high reproducibility even in the range of concentrations below the cutoff value, the measurement with the marker appeared to be of high precision, capable of detecting even the slightest variations in the antigen. CA 602 therefore appears of great value in the early detection of recurrent ovarian cancers.

[Cervical esophago-gastrostomy via the posterior mediastinal route for carcinoma of the thoracic esophagus].

From 1981 to 1990, 297 cases of carcinoma of the thoracic esophagus were resected in our department. Among these cases, 225 cases (84.3%) were reconstructed cervical esophagogastrostomy via the posterior mediastinal route. In this paper, these 225 cases were analysed to evaluate the usefulness of this procedure which was developed by S. Abo in 1975. Operative mortality was 4.4% Anastomotic leakage occurred in 24.4% of the cases, but the majority of the cases were treated successfully without any drainage and 83.3% of the cases could start their diet within one month after operation. The volume of oral intake by the patients after discharge increased satisfactorily (1,500-1,800 kcal/day), but the body weight decreased gradually. So the ambulatory enteral nutritional support was started and proved useful for maintaining their body weight. Alkaline reflux evaluated using 24-hour pH-monitoring system was observed in about half of the cases but the degree of the alkaline shift was not strong. Some complaints (heart burn in 6.3%, regurgitation in 21.9%, feeling of fullness after eating in 40.6%, stenotic sensation in 20.7%) were present but not serious.

[A case of minimal-sized adenocarcinoma in a fistula-in-ano].

The patient was 47 year-old man with a simple intermuscular fistula-in-ano. Fistulectomy revealed that there was a small mucinous adenocarcinoma. The tumor was small and localized in the wall of the fistula and the muscular invasion was not recognized. The mucin contained a little O-acylated sainomucin, and the origin of the tumor was considered as anal gland cell. This case appears as one evidence that the cancer in fistula-in-ano originates from the anal gland cell in the fistula.
