Chloroquine neuromyotoxicity. Clinical and pathologic perspective.

Six cases of toxic myopathy and/or neuropathy with chloroquine and/or hydroxychloroquine therapy are described. Two patients had unique clinical and pathologic evidence of cardiomyopathy secondary to chloroquine or hydroxychloroquine therapy. One patient had polyneuropathy secondary to chloroquine toxicity. This may be the first documentation of several features of chloroquine/hydroxychloroquine toxicity: morphologic changes in human peripheral nerve in chloroquine toxicity; chloroquine/hydroxychloroquine cardiomyopathy diagnosed by endomyocardial biopsy; and hydroxychloroquine myotoxicity. Chloroquine is a neuromyotoxin that affects nerves and cardiac and skeletal muscles. Discontinuation of chloroquine and hydroxychloroquine resulted in marked improvement in most cases. The reversibility of the symptoms emphasizes the importance of recognizing potential signs of nerve, muscle, and cardiac toxicity in patients being treated with chloroquine or hydroxychloroquine.

Occurrence of magnetic material in teleosts.

Fishes representing the main groups of teleosts have been investigated for magnetic material by susceptibility measurements. All the investigated species contain magnetic material. Bone samples from the skull and the vertebral column contain magnetic particles which yield a saturation magnetization in the range 10(-4) emu g-1 to 10(-3) emu g-1 in a sample. The localization of the magnetization is diffuse within the tissues connected to the bone. There are no significant differences between the amounts of magnetic material that are found in migrating or more stationary species.

The oral glucose tolerance test in men under 55 years of age with intermittent claudication.

Blood glucose at sixty, ninety, and one hundred twenty minutes after a standardized oral glucose load was compared in healthy middle-aged men and patients with intermittent claudication. No difference was found when the authors compared blood glucose at zero and sixty minutes. At both ninety and one hundred twenty minutes, however, they found a relative hypoglycemia in patients with peripheral arteriosclerotic disease in an early stage.

Lower extremity blood flow in intermittent claudication--the role of oral glucose tolerance test.

Tobacco smoking is a risk factor for peripheral arterial disease. During oral glucose tolerance tests (OGTT) in a population study the author and co-workers have earlier shown that smokers have higher blood glucose values early postload and lower values at two hours compared with nonsmokers. Eighty-three patients, all with intermittent claudication but with a normal OGTT, have now been studied according to their response to an oral glucose load. The blood flow resistance during reactive hyperemia, as measured with a calf plethysmograph, was compared between subjects in the quartile of patients with the highest forty-five-minute blood glucose and those in the lowest quartile. The blood flow resistance was significantly higher in the group with a high forty-five-minute blood glucose, 13.3 +/- 1.34 vs 9.5 +/- 0.65 (mean +/- SEM), p less than 0.02. Blood pressure and blood lipids were similar in the two groups. It is suggested that an exaggerated early response in the OGTT might be an independent risk factor for peripheral arterial disease.

Motilin response to a glucose load aberrant in smokers.

Smoking is known to influence the blood glucose response to an oral glucose load, possibly by altering the gut motility. Because the peptide hormone motilin might be involved in the regulation of the gastric emptying pattern, we studied 12 heavy smokers and 9 non-smokers after an overnight fast and tobacco abstinence. After an oral glucose load non-smokers showed falling serum motilin levels, as expected, whereas the smokers had initially increasing values. We conclude that smoking affects the gastrointestinal tract, not only during acute nicotine exposure but also chronically.

Gastric emptying in smokers.

We investigated the gastric emptying of a small solid meal in 21 healthy middle-aged men. Nine heavy smokers and 12 non-smokers were studied with a scintigraphic technique, using 99Tc-tin colloid as a tracer. All subjects had been instructed to abstain from smoking for at least 10 h before and during the examination. Smokers had on an average a shorter gastric emptying half-time (24.2 +/- 8.5 min) than non-smokers (40.6 +/- 14.1 min) (p less than 0.01). We conclude that habitual smoking has not only acute effects on gastric emptying but also a more long-standing influence.

Neutron brachytherapy is better than conventional radiotherapy in advanced cervical cancer.

Californium-252 (252Cf), a fast-neutron emitting radioisotope, was used for neutron brachytherapy (NT) of 82 patients with advanced (stage III and IV) cervical cancer. The results were compared with caesium-137 brachytherapy; both isotopes were given in combination with high-dose fractionated pelvic radiotherapy. In patients with stage IIIB disease (ie, tumour to the pelvic side-wall), 252Cf intracavitary therapy resulted in 54% 5-year survival compared with 12% in the caesium group. To be effective, Cf-NT must be given before external beam (photon) therapy.

Feasibility study: results of treatment of primary and recurrent adenocarcinoma of the corpus uteri with californium-252.

A trial of Cf-252 for the radiotherapy of primary or recurrent corpus adenocarcinoma was carried out at the University of Kentucky. The patients with primary tumors were of advanced age, poor general medical condition, and had multiple chronic medical illness, poorly differentiated tumors and/or metastatic Stage IVB disease. Fourteen patients with primary tumors were treated and all achieved complete local tumor control. The five year actuarial disease-free survival rate by the Kaplan-Meier method was 100% for Stage I-III disease, although 36% died of other, usually medical, causes. For eight patients treated with recurrent tumors, long term tumor control was 40% four year actuarial disease survival. Local control was 100% for vault recurrences of 3 cm diameter size, but only 6/8 (75%) cleared their pelvic tumors completely. Those that did not had tumors of massive size, distant metastatic disease and adenosquamous or poorly-differentiated (G3) pattern. One additional cut across, infected tumor was controlled for 26 months. Corpus carcinoma was highly sensitive to Cf-252 neutron radiation therapy.

Clinical trial of 252Cf neutron brachytherapy vs. conventional radiotherapy for advanced cervical cancer.

252Cf, a neutron emitting radioactive transplutonium isotope, was tested for its efficacy against advanced bulky Stage III-IV cancers of the cervix in a clinical trial at the University of Kentucky Medical Center. Eighty-two patients were treated during 1976-1979 and followed for 5-year survival and tumor control. Three different treatment methods went on sequentially and concurrently, that is, (a) conventional whole pelvis photon with delayed 137Cs implants, (b) conventional photon therapy with delayed 252Cf implants, and (c) 252Cf implants ("early") preceding photon therapy. There were 12% 5-year survival for Stage IIIB cancers by conventional therapy, and 15% by delayed 252Cf implant therapy. For early 252Cf implant therapy there were 54% 5-year survivals with 4% complications and 65% 5-year local control, but distant metastases became a prominent delayed failure pattern.

A multicentre prospective randomized controlled trial of induction of labour with an automatic closed-loop feedback controlled oxytocin infusion system.

A randomized prospective controlled trial of an automatic feed-back controlled oxytocin infusion system (AIS) for the induction of labour at term was carried out in three hospitals. In primiparae, a four-fold reduction in the total dose of oxytocin infused, for the same induction-delivery interval, was achieved with the AIS compared with standard labour ward protocols relying on midwife control of oxytocin infusion rate. In multiparae, the total dose of oxytocin infused was almost halved when the AIS was used, while the mean duration of labour was increased by 1.6 h to 7.5 h. There were no differences in the mode of delivery or Apgar scores. It is concluded that the AIS is a safe technique for induction of labour. Its use has demonstrated that term labour can be induced efficiently with a mean oxytocin infusion rate between 2.5 and 3 mU/min (max 8.8 mU/min) and a total oxytocin dose infused of less than 5 U.

The role of focus group interviews in designing a smoking prevention program.

The purpose of this study was to gather data that would be applied to the design of an adolescent smoking prevention program. Focus group interviews, a form of qualitative research used in social marketing, were used to gather information from the adolescent participants to ensure that the program would reflect the needs and interests of the target population. The participants consisted of sixth graders from urban and rural areas, and black and white populations of low socioeconomic status. A total of eight focus groups was conducted with eight participants in each focus group. Activities such as auctions, roleplays, and guided discussions were carried out to learn about participant likes, dislikes, values, ideas, and abilities to turn down cigarettes. Following substantiation by a quantitative evaluation, this information was incorporated into the program and materials to ensure the maximum effectiveness of the intervention.

Histochemical study of the distribution of carbonic anhydrase in the cat brain.

The distribution of carbonic anhydrase (CA) in cerebrum, cerebellum and medulla oblongata of the cat brain has been examined by a histochemical method. Neuron cell bodies and dendrites are stained in some locations. Many axons are distinctly stained and different intensities of staining can be seen even in adjacent axons. One of the most intensely stained structures is the capillary endothelium and stained capillaries are found in all parts examined. Glial cells are intensely stained in agreement with biochemical and earlier histochemical works. Myelin sheaths are never stained, possibly due to enzyme loss during embedding. The localization of the enzyme shows regional differences. In this respect, the medulla oblongata has been examined in more detail. A small area close to the ventral surface, medial to the roots of the hypoglossal nerve, is characterized by a high CA staining of the neuropil. The cell membrane of some large neurones and the capillary endothelium in this area were also stained. With regard to position and CA content, this area corresponds well to the characteristics of the medullary chemosensitive area as defined by previous experimental studies.

Pattern evoked potentials (PEPs) in Parkinson's disease.

Controversy exists in the literature concerning whether Parkinson's disease (PD) results in prolongation of pattern evoked potential (PEP) responses. PEPs were obtained in 20 patients with PD. The latencies of the first major positive potential (P2) in response to independent left and right eye stimulation were evaluated. There was no statistically significant difference (group or individual) when patients with PD were compared with age-corrected controls. There was no difference in results in comparing patients with mild, moderate, and severe PD. These results contradict previous reports of markedly abnormal PEPs.
