Controlled clinical study on antihypertensive treatment with a diuretic and methyldopa compared with a beta-blocking agent and hydralazine.

Twenty-eight previously untreated patients with essential hypertension were included in a randomized double-blind cross-over study comparing the results of treatment with the established combination of a thiazide and methyldopa (regimen A) with the combination of a beta-receptor blocker and hydralazine (regimen B). Three patients each developed intolerable side-effects on each regimen but they were all treated successfully on the alternative regimen. The remaining 22 patients obtained a significant reduction in BP at rest and during exercise on both treatments, with no significant difference between the two schedules. Heart rate was significantly reduced at rest and during exercise with regimen B, while a significant reduction was also obtained following exercise on regimen A. There was no significant difference between the two regimens as to tolerable side-effects during treatment, which were registered in about 60% of the patients on each scheme. However, 64% of the patients were treated satisfactorily without side-effects on either regimen. It is concluded that the combination of a beta-blocking agent and hydralazine is without obvious advantages compared with the combination of thiazide and methyldopa in obtaining initial BP control in patients with essential hypertension.

Postoperative ventilatory response to carbon dioxide following neurolept anaesthesia.

In order to investigate the duration of respiratory depression following neurolept anaesthesia, 21 patients who had hemilaminectomia performed were subjected to ventilation-carbon dioxide response tests by a rebreathing method in the post-operative period. Up to 3 1/2 h after start of anaesthesia all patients had a decreased ventilatory response to carbon dioxide. After this period some patients showed a normal respiratory response, while others remained depressed for up to 6 h, the period for which the investigation lasted. Most patients in the depressed group were premedicated with pethidine. We conclude that additional narcotics are contraindicated for a period of 3 1/2 h following the start of neurolept anaesthesia for surgical procedures lasting 1-2 h, while clinical control is still necessary before administration of additional narcotics within 6 h, especially if narcotics have been used as premedication.

Combination chemotherapy of an APUDoma. With special reference to the therapeutic value of monitoring hormonal substances.

Investigations are presented on the occurrence of tumour products during combination chemotherapy of a 49-year-old female with an APUDoma metastatic to the liver. Calcitonin was demonstrated in high concentration in the tumour tissue. Serum calcitonin, serum histaminase and 5-HIAA in a 24-hour urine sample increased immediately after the administration of cytotoxic agents, falling subsequently below the pretreatment level. These findings indicate a therapeutic effect with lysis of tumour cells. Continuous determination of the three tumour substances showed an increase in these products before clinical suspicion of progression. Electron microscopic examination during the initial course disclosed the tumour to be an APUDoma. Autopsy failed to disclose a primary site outside the liver. Further autopsy findings were an adenoma of the thyroid and a chromofobe adenoma of the pituitary, thus assigning the patient to type 1 multiple endocrine neoplasia.

Influence of hypoxia and hyperoxia on subcutaneous adipose tissue blood flow in man.

Subcutaneous adipose tissue blood flow (ATBF) was measured by the 133Xe washout method in normal men breathing oxygen at partial pressures from 0.1 to 2 atmospheres. Breathing oxygen at partial pressures of 0.1 and 1 atm. increased and reduced ATBF by 56% and 26%, respectively as compared to air breathing. These effects were highly significant. Breathing of oxygen at 2 atm. partial pressure had no consistent effect on ATBF, although considerable changes--positive or negative--could been seen in some experiments. Oxygen-breathing at 1 atm. partial pressure is associated with a slight hyperventilation and fall in PACO2. This was shown to be without importance for the effect of oxygen on adipose tissue blood flow.

The effect of different oral anticoagulants on diphenylhydantoin (DPH) and tolbutamide metabolism.

The effect of bishydroxycoumarin, phenprocoumon, warfarin and phenindione on the metabolism of diphenylhydantoin (DPH) and tolbutamide has been studied in 54 patients. The half-lives of DPH and tolbutamide in blood following i.v. injections were studied in 33 patients before and after one week of anticoagulant treatment. Bishydroxycoumarin increased the mean half-life values of DPH from 8.8 to 37.4 hours and of tolbutamide from 4.9 to 17.5. Phenprocoumon prolonged DPH half-life from a mean value of 9.9 to 14.0 hours but did not change the tolbutamide half-life. Warfarin and phenindione did not affect DPH or tolbutamide half-lives. Steady state concentration studies in 21 patients showed a rise in serum DPH during bishydroxycoumarin and phenprocoumon treatment but not during treatment with warfarin and phenindione. A rise in serum tolbutamide was noted during treatment with bishydroxycoumarin. These findings suggest that bishydroxycoumarin inhibits the betabolism of DPH and tolbutamide and that phenprocoumon inhibits DPH metabolism. No effect on DPH and tolbutamide metabolism could be demonstrated following administration of warfarin and phenindione.

Multiple endocrine adenomatosis of mixed type.

A case of multiple endocrine adenomatosis (MEA) of mixed type is presented. The syndrome, observed in a 65 year-old female, consisted of multiple neurofibroadenomatosis, medullary thyroid carcinoma, multiple adenomata of the parathyroids, adrenal cortical adenoma and small cell anaplastic bronchogenic carcinoma. Thus, it was composed of type 1 as well as of type 2 MEA. On the basis of another seven cases, collected from the literature, the MEA syndrome of mixed type is reviewed with special reference to the phylogenetic origin of the cells of the APUD system.

Prophylactic, postinfectious and neonatal vaccination against canine distemper in mink.

Mink were vaccinated against canine distemper and challenged with the Snyder Hill strain of canine distemper virus. Protection was acquired if vaccination took place more than a few days before challenge. If vaccination took place even early during incubation mortality was higher than in the unvaccinated controls. Full protection of kits may be achieved even at an age of 16 to 20 days provided the kits had not passively acquired immunity from their mothers. Kits surviving distemper because of passively acquired immunity may not actively acquire immunity during infection and may die at a later exposure.

A psychiatric-psychological study of XYY males found in a general male population.

The psychological test results of five XYY males from in a population study showed an intellectual level within the normal range but with a mean full scale IQ and educational level lower than expected. The cognitive as well as emotional function was characterized by immaturity, manifested in passivity, unreflectiveness and emotional liability, in three resulting in uncontrolled aggressive outbursts. Conflict material also appeared immaturely resolved, centering around unfulfilled needs of contact and insecure masculine identification. The defense mechanisms used were generally rather weak, but only in one subject did the anxiety level seem to be excessively low. All five males differed to a certain extent from their siblings; three of them were hyperactive, restless, hot-tempered and impulsive at school and four of them had difficulties at school. Three learned a trade, but only one stayed in his trade, and one was applying for disablement pension on account of personality deviation. Two of the five had a criminal record. It is concluded that the presence and degree of the above-mentioned characteristics of XYY males varied. It is evident that environmental factors play as great a role for the development of personality and behaviour in males with karyotype 47, XYY as in males with a normal chromosome constitution.

A new preparative method for microspectrophotometric DNA-measurement in megakaryocytes, revealing a significant group of cells with a DNA content of 64 N.

Microspectrophotometric investigations of Feulgen-stained rat bone marrow megakaryocytes, comparing the traditional smear method and a new 1 G sedimentation and 40 degrees C evaporation (S. E.) method, have shown that the latter reveals a significant group of megakaryocytes (ca. 30 percent) with a DNA content of 64 N. This group is totally absent when the smear method is used.
