[Role of ureteroscopy in the treatment of lithiasis of the pelvic ureter. Apropos of 52 cases].

Between July 1990 and December 1995, 52 patients with lower ureteral calculi were treated by rigid ureteroscopy in the "B" Urology department of Avicenne Hospital. The use of ureteroscopy facilitated Dormia extraction (58%) or ultrasonic lithotripsy fragmentation (17%) or removal of the stone with the two procedures (21%). The overall success rate was 92.3% with a 2% retreatment rate. Complications were rare (3 of 52: 5.8 percent). The mean hospital stay was 3.3 days. Based on our findings, we conclude that rigid ureteroscopy for lower ureteral stone removal is effective with low morbidity and shorter hospital stay.

[An unusual complication of anterograde endopyelotomy: ureteral intussusception (a case report)].

Endopyelotomy has recently found a new place in the treatment of stenosis of the pyeloureteral junction, and it can represent a treatment of choice. We report an unusual case of antegrade ureteral intussusception after antegrade endopyelotomy in a 30 year-old young woman with a solitary kidney, which caused proximal ureteral obstruction. These intussusceptions were probably caused by double CH endoprosthesis. These iatrogenic injuries required surgical resection with calicoureterostomy.

[Adenocarcinoma of the kidney associated with polycystic kidney treated by conservative surgery (apropos of a case)].

The authors report a case of renal cell carcinoma discovered incidentally in a 52-year-old woman with hepatorenal and pancreatic polycystic disease. Treatment consisted of partial nephrectomy with no adjuvant therapy. After a 3-year follow-up, the patient is still alive with normal renal function and no signs of recurrence. The pathogenesis and therapeutic modalities are discussed in the light of a review of the literature.

[Ureter injuries: therapeutic approach and the role of ureteroscopy. Apropos of 18 cases].

From June 1990 to June 1996, 18 patients with ureteric injuries were treated in our institution. All ureteric lesions were diagnosed post-operatively and repaired early (2 cases) or after a delay (16 cases). 4 patients underwent exclusive endourological treatment, 10 patients underwent repair by open surgery without preliminary endourological procedure, and in the remaining 4 patients, the two methods were combined. In this group, ureteroscopy, which facilitates intraoperative identification of the ureteric segment below the lesion, allowed end-to-end ureteric anastomosis, which, in the light of our results, constitutes the reference surgical technique.

[Urogenital tuberculosis. Apropos of 40 cases].

The authors report a series of 40 cases of genitourinary tuberculosis diagnosed and treated in the department of urology "B" of Avicenne hospital over a 7-year period. The objective of this study is to define the various diagnostic and therapeutic aspects of this disease. The patients were predominantly males (62.5%) with a mean age of 40 years. 25% of cases reported a history of extra-urinary tuberculosis. The very polymorphous clinical presentation is dominated by signs of cystitis (45%). Intravenous urography is frequently suggestive of the diagnosis based on the appearance and multiplicity of the lesions. The radiological lesions most frequently encountered were silent kidney (19 cases) and small tuberculous bladder (11 cases). The definitive diagnosis was established by pathological examination in 38 cases (biopsies, operative specimens, prostatic resection chips) and/or by demonstration of AFB in 2 cases (urine, pus). Tuberculostatic treatment was administered to all patients, either alone (5 cases) or, more usually, in combination with surgical and/or endo-urological treatment (35 cases), reflecting the magnitude and severity of the destructive and scar lesions.

[Fournier's gangrene. Analysis of 32 cases].

A series of 32 treated cases of Fournier's gangrene is analysed. All patients were male, their age ranged from 19 to 89 years. In 16 cases (50%) the aetiology of gangrene was urethral (37.5%), anorectal (12.5%) and in the 16 remaining cases (50%) there was no identifiable cause. Mortality was high (28%) despite broad spectrum antibiotics and aggressive surgical debridement. This mortality was essentially associated to advanced age, debilitated state, delay in diagnosis and toxi infectious context. The average hospital stay was 26.5 days.

[A rare cause of retroperitoneal fibrosis: actinomycosis].

The authors report a case of retroperitoneal fibrosis secondary to actinomycosis with consequences on the urinary tract. They insist on the difficulty of diagnostic, the antibiotic's sensibility and the evolution towards fibrosis which can lead to renal function deterioration.

[Internal drainage by double J ureteral stents. 91 cases].

Urinary drainage by indwelling double J ureteral stent is well documented in the urologic literature. We used these stents in 91 patients. The majority of stents were placed endoscopically (68%). Indications were: -Ureteral obstruction (39 cases) such as tuberculous ureteral strictures, obstruction due to urolithiasis and pelvic malignancies. -Upper urinary tract surgery (29 cases) mainly pyeloplasty, pyelolithotomy, ureterovaginal fistula repair and ureteroneocystostomy. -Adjunct to endourologic treatment (16 cases) such as ureteroscopy and endopyelotomy. -Preparation for extracorporeal lithotripsy (7 cases). The complication rate associated with placement of double J stents was minimal (6.6%). The major complication was migration (3 cases). The average drainage time was 5.8 weeks. In view of these results we conclude that double J stent is safe, effective and has minimal complications.

[Diagnostic and therapeutic aspects of suburethral diverticuli. Apropos of 9 cases and review of the literature].

Nine female patients with a urethral diverticulum were admitted to our center over a six period: The clinical symptomatology was not specific. The diagnosis has been performed by voiding and retrograde cysto-uretrogram. The surgical treatment consisted in the excision of the diverticula, with the patient in the supine position. No perioperative morbidity was observed.

[Role of ultrasound lithotripsy in the treatment of lithiasis of the lower urinary tract. Apropos of 38 cases].

Between February 1989 and February 1994, 38 patients with lower urinary tract lithiasis were treated by ultrasonic lithotripsy. Mean size of the stone was 23 x 19 mm, extreme being 5 mm-50 mm (over 20 mm in 27 patients). Transurethral lithotripsy has been performed in 36 patients, and percutaneous cystolithotripsy in 2 patients. Some desintegration was complete in 37 cases, and the only failure was a case in which the stone was too hard. Morbidity was zero. Ultrasonic lithotripsy for urethral and bladder stones appears to be a safe, effective and simple method of treatment.

[Ureterovaginal fistulas: treatment with preservation of the distal ureteral segment. Apropos of 3 cases].

The authors report on the treatment of 3 ureterovaginal fistulas using a method preserving the segment of ureter below the fistula. In one case, the fistula was successfully managed by ureteroscopic placement of ureteral stent. Two other cases were managed with ipsilateral ureteroureterostomy, where the segment of ureter below the fistula was identified using ureteroscopic perforation of this segment.

[Sclerotherapy with Betadine for simple cysts of the kidney. Apropos of 7 cases].

The authors report on 7 cases of benign, symptomatic renal cysts treated by percutaneous sclerosis using Betadine solution. The mean size of the cysts was 8 cm and we obtained progressive collapse of all of them. This technique was a simple and effective methode with good results. Indeed, 85% of the cysts disappeard, and only one patient retained a small asymptomatic residual cavity.

[Syndrome of the ovarian vein. Apropos of 3 cases].

About three cases of ovarian vein syndrome and a review of the literature, on overall study of this rare entity is done addressing its different pathogenie, clinical, radiological and therapeutic aspects. The authors stress the importance of ureteroscopy with biopsy in order to confirm the diagnosis of the ovarian vein syndrome.

Continent urostomy with hydraulic ileal valve in 136 patients: 13 years of experience.

The hydraulic ileal valve, which we developed in 1975, ensures continence by adapting to 5 different urinary reservoirs. The valve is made by isolating a 14 cm. long intestinal loop with the mesentery. The isolated ileal segment then is folded inward on itself throughout its entire length. We performed 136 continent urostomies with this hydraulic valve. An ileocecal reservoir was used in 122 patients, ileum in 8, sigmoid in 1, rectum in 1 and bladder (continent cystostomy) in 4. Indications for continent urostomy were bladder tumor in 55 patients, complex vesicovaginal fistulas in 5, neurogenic bladder in 13, vesical exstrophy in 12 and miscellaneous reasons in 5. Of the patients 103 (75 per cent) were continent immediately. Continence was obtained after repair of the valve in 24 patients (17.6 per cent). Therefore, 127 patients over-all were continent. Mean followup of our patients was 38 months (range 3 to 154 months). Continence remained excellent with self-catheterization performed easily in 88.3 per cent of the patients. Over-all, all of our continent urostomies were well tolerated biologically and radiologically.

[Continent urostomy using a hydraulic ileal valve. Apropos of 160 cases].

We report our experience of continent urostomy about 160 cases. This study described the continence mechanism by the hydraulic valve. This one is made by isolating of 14 cm long ileal loop which is invaginated. It was adapted to six reservoirs: 142 ileocecums, 9 ileal pouchs, 4 bladders, 3 sigmoids, 1 rectum and 1 colonic transverse segment. The follow-up was few months to 14 years. 145 patients are continent, the continent urostomy was well tolerated by the upper urinary tract. The complications of the valve and their management are analysed.

[The continent urostomy. 12 years' experience with the continent ileocecal bladder].

The ileo-caecal continent urostomy was first described in 1975. It is a combination between an ileo-caecal reservoir and an ileal hydraulic valve. From 1973 to 1986 we performed 107 such operations (41 for vesicovaginal fistula, 40 for bladder tumor, 26 for bladder exstrophy, neurogenic bladder and miscellaneous). Continence was obtained in 100 patients (80 at the first operation and 20 after operation). The follow up was 3 to 146 months (average 37) in 82 patients 74 of whom had conserved good continence. In cases with failure of continence, repair was obtained by reinvagination of the same valve or by creating a new valve. Only 2 cases of metabolic disorders in the form of acute pyelonephritis are reported. The continent urostomy provides a better quality of life for patients requiring a urinary diversion. The psychosocial and economic impact is very important.
