The role of selective internal urethrotomy in the management of urethral stricture: a multi-centre evaluation.

The advent of selective internal urethrotomy under direct vision has enabled precision endoscopic surgery to be undertaken on a wide range of urethral strictures. A multi-centre survey of 197 cases involving 322 urethrotomy procedures from 5 urological departments in England is reported. The overall results after a follow-up of up to 4 years suggest that there is no indication for further procedures currently existing in 160 (81%) of those cases subjected to selective internal urethrotomy. The additional injection of triamcinalone acetate into the strictured area prior to urethrotomy is recommended in resistant cases. The procedure of selective internal urethrotomy is, in our opinion; the best primary method for the treatment of urethral stricture, and it is hoped this will reduce the indications for anastomotic or substitution urethroplasty.

Continent ileocaecal urinary reservoir.

In 14 cases, a continent ureteroileocaecal urinary reservoir has been constructed. If a long-term ease of self catheterization can be maintained, some patients requiring an urinary diversion might prefer an appliance-free urinary reservoir to a freely draining conduit and urinary appliance. The indications and place for the continent urinary reservoir still need to be studied, modified and proven during the coming years.

Parameatal urethral cysts: a review of 6 cases.

6 cases of parameatal urethral cysts are reported in 3 males and 3 females. The condition is considered to be rare. The aetiology is thought to be obstruction of the para-urethral ducts possibly due to infection. The treatment of choice is complete excision.

Urodynamic studies in the district general hospital.

The information given by urodynamic investigations has been of practical value in clinical practice and we feel that the time and money spent has been worthwhile. The results are particularly useful in patients with neuropathic bladders, women and stress incontinence in whom detrusor instability is also suspected, young men with "obstructive" symptoms and in men with post-prostatectomy problems. Its value in te large group of women with frequency, urgency and nocturia is debatable given our present lack of satisfactory treatment for these patients.
