Seven patients with a malignant tumor were treated for spinal instability with Luque-rod fixation either with or without bone-grafting. The stabilization was successful in all patients, including five who died as a result of the tumor between six and eighteen months postoperatively. Two patients were still alive at an average of twenty-two months after operation. The only complication that was directly related to the procedure was cutting through the lamina of a fixation wire.

The surgical approach has been the predominate mode of caries management for the past 150 years. Dentistry has, however, in recent years moved toward an antibiotic/antimicrobial model of disease management. This approach, however, raises serious questions: (1) do the antibiotic/antimicrobial agents (chlorhexidine, povidone iodine, fluoride, etc) kill all offending organisms?; (2) if so, do the agents preclude the re-entry of the same organisms from external sources?; and (3) if the agents do kill all the offending organisms, do any remaining pathogenic organisms have selective advantage in repopulating the tooth surfaces? To overcome the problems inherent in an antibiotic/antimicrobial approach, probiotic methods are currently under study as means of caries management. This paper discusses probiotic approaches, such as genetically modified Streptococcus mutans and targeted antimicrobials in the management of dental caries. Implications for this approach in the management of other diseases are also presented.

The transverse strength of large amalgam restorations retained by amalgapins of varying diameter and number was evaluated. Restorations with amalgapin retention fractured with greatest frequency parallel to the pulpal floor through the amalgapin at the amalgam-tooth interface. The findings indicate that the geometric configuration and number of amalgapins was of greater importance to the transverse strength of amalgapin-retained restorations than the total cross-sectional area.
The effect on microleakage of two glass-ionomer/resin "sandwich" restorations and an incrementally placed microfilled resin restoration was tested in class 5 preparations extending apical to the cementoenamel junction. There was significantly less leakage detected in the sandwich restorations with microfilled resin placed in one or two increments. These results indicate that a glass-ionomer/microfilled resin sandwich restoration may significantly reduce microleakage in restorations extending below the cementoenamel junction.

The last decade has seen dramatic developments in the management of ventricular arrhythmias, with new ideas about antiarrhythmic drug therapy and a growth in the use of implantable device therapy. This article reviews these changes and aims to provide a straightforward guide to the current management of ventricular arrhythmias.

Fractured or ditched margins are the most common problem associated with amalgam restorations. Unfortunately, continuously replacing dental amalgams takes its toll on a tooth. Good restorative practices demand that we limit the number of restorative procedures to which we subject a tooth. This paper looks at an effective technique for sealing the margins of ditched amalgam restorations. This technique allows the practitioner to repair the ditched area of the amalgam without replacing the amalgam restoration.
