Total laparoscopic aortic repair for occlusive and aneurysmal disease: first 95 cases.

OBJECTIVES: To analyze the outcome of our preliminary experience with total laparoscopic aortic repair in patients with occlusive or aneurysmal disease. MATERIAL AND METHODS: From September 2002 to April 2005, we performed 95 consecutive total laparoscopic aortic repair procedures including 72 for aortic occlusive disease (group A) and 23 for abdominal aortic aneurysm (group B). RESULTS: In group A, mean operating time was 216+/-50 min with a mean clamp time of 57+/-21 min and surgical conversion was required in two cases (2.7%). No postoperative death occurred but there were three postoperative complications necessitating re-intervention (retroperitoneal hematoma, embolic ischemia, and early prosthetic infection). Mean duration of hospitalization was 8 days (range, 5-42 days). All grafts were patent at 2 months. In group B, mean operating time was 251+/-57 min with a mean clamp time of 101+/-15 min and surgical conversion was required in seven cases (30%). There was one postoperative death (4.3%) due to pulmonary embolism and one non-fatal complication (retroperitoneal hematoma). Mean duration of hospitalization was 6.4 days (range, 4-12 days). All grafts were patent at 2 months. CONCLUSION: Total laparoscopic repair is feasible and safe for occlusive and aneurysmal aortic disease. Operators must acquire technical skills using simulators.

The effect of feeding or starvation on resource allocation to body components during the reproductive cycle of the sea urchin Sphaerechinus granularis (Lamarck).

To determine the effects of feeding or starvation on resource allocation to body components during the reproductive cycle of Sphaerechinus granularis, sea urchins were placed in laboratory tanks and either fed ad libitum or starved during two different periods of their biological cycle, i.e. the mature stage and the recovery stage. The urchin growth was monitored over the whole experimental period, the gonad, gut, lantern indices and organic matter levels of different organs were determined at the end of the experiment. During the mature stage sea urchins in good nutritional conditions did not increase in size, but allotted energy to gonad production and stored reserves in body wall. Limiting food stopped the gonadal growth without complete regression. During the recovery period food allowed somatic growth, i.e. test growth and the storage of reserves in gonad somatic cells. This somatic production did not occur under food-limited conditions and the resources allotted for survival and maintenance were taken from different body components.

Comparison of embryonic development and metal contamination in several populations of the sea urchin Sphaerechinus granularis (Lamarck) exposed to anthropogenic pollution.

Over the past years the functional deficiencies observed in the sea urchin Sphaerechinus granularis from the Bay of Brest (France) have led us to select several populations within this species to assess water quality in the Bay of Brest. The present study reports on the comparison of the quality of embryonic development and of the level of metal contamination in sea urchin gonads and gut wall in four populations. Three of them lived in the Bay of Brest, and the last one was considered a reference, being settled in the Glénan Archipelago in southern Brittany (France). The sea urchin population living at the mouth of the River Aulne, at the Armorique Point, seemed to be the most affected, for it displayed the highest rate of blockage and the longest delay in embryonic development along with the highest concentrations in heavy metals, i.e., Fe, Cu, Pb, Cd, and Hg, in the gut wall and in the gonads. A comparison with previous data highlighted a degradation of health conditions in the population living at Marloux in the vicinity of Brest harbor. The comparison of the metal levels detected in individuals from these two populations with the data reported in the literature allowed us to conclude for a local contamination of the bay waters by Fe and Hg. In addition, this study also revealed that Zn and Cd concentrations were sometimes higher than the maximum values reported in the literature and evidenced an increase of contamination by Cu and Pb over the period of study; it should be noted, however, that these values were not excess levels. Our investigations also suggest that Zn, Cd, Pb, and Cu levels should be monitored in the future.

[Simplified vibriocidal antibody titer performed on a drop of blood].

We report a simplified technique for vibriocidal antibody test in underprivileged people in tropical area. The test is performed on a drop of blood sampled on a disc of blotting paper. It avoid taking of large quantities of blood in timid underfed people who are often solicited. Its reading is rapid and easy by the naked eye owing to staining viable germs violet by the neotetrazolium succinate. Tested in mice and humans, the simplified method gave data well correlated to those obtained with the standard test on serum for vibriocidal titres higher than (or equal to) 1/40.

[Resistance of Vibrio cholerae 0:1 to the 0/129 compound and multiresistance to antibiotics].

The authors present a study of the in vitro susceptibility to O/129 compound and usual antibiotics of 29 strains of V. cholerae O:1 biotype El Tor isolated during epidemics in miscellaneous countries over the world from 1982 to 1991. Several identical isolates from the same epidemic are represented by one strain. Susceptibility testing by diffusion method and MICs by agar dilution method are used. The data show that the resistance to O/129 compound is often associated with the resistance to usual antibiotics such as trimethoprim, sulphonamides, chloramphenicol, ampicillin and tetracycline. This resistance to the vibrostatic compound leads to a double problem of diagnosis and therapy. The nitrofuranes derivatives and tiliquinol-tilbroquinol association, an intestinal antiseptic, are the most active antimicrobial agents as well on the strains O/129 sensitive as on the strains O/129 resistant.

[Recurrent supraventricular tachycardia. The efficacy of a radio frequency system inserted into the coronary sinus].

The case is reported of a man of 66 with attacks of recurrent supraventricular tachycardia (SVT) which failed to respond to the standard medical treatment and which, because of their frequency, led to mental disturbances. The electrophysiological features of SVT are discussed, and the possibilities of recording potentials from the SA node. A definitive pacemaker was placed in the coronary sinus, and linked to a simple stimulator working at radio frequency (and activated by the patient himself); this led to an almost immediate reduction in the attacks of SVT.
