[Kawasaki disease: a new pediatric nosological entity].

Kawasaki disease (KD) is a new, well characterized pediatric syndrome. In this work the last epidemiologic, diagnostic and clinical data are pointed. Especially the heart-disease and its monitoring and therapy are studied. Our casuistry (5 cases in 4 years) is also presented. The international literature and our clinical experience suggest that KD is more spread than we think and therefore in needs to be studied carefully by the pediatricians.

[Cystic lymphangioma of the spleen (author's transl)].

The spleen is the least common intra-abdominal site for the development of cystic disease. A lymphoangioma of the spleen was first reported in 1885 by Fink; since that time 47 cases of lymphoangioma or lymphoangectasia have been reported. This report concern a case of cystic lymphoangioma of the spleen consisting of a large splenic cyst with associated multiple small subcapsular cysts and lymphangectasia. The diagnosis and treatment of splenic cysts are discussed and a new classification of splenic cysts is proposed.

Child↗

[Mast cells in the laryngeal papillomatosis of the child].

Mast cells are probably involved in the immunitary phenomena thought to be responsible for the evolution of laryngeal papillomatosis in children, particularly as a consequence of the correlation between such cells and eosinophile granulocytes. A morphological study of 9 cases in subjects aged between 2 and 9 yr is reported. The mean number of mast cells in the various phases of the diseases was assessed. There was a significant decrease in mast cell numbers right from the first stages of the lesion to the terminal stage prior to regression.

[Infiltrating lobular carcinoma of the breast, alveolar variant, with stromal reactive osteoclast-like giant cells. Description of a case].

Authors report a case of alveolar variant of infiltrating lobular carcinoma of the breast with stromal osteoclast-like giant cells. Immunohistochemically the carcinoma cells were reactive for EMA and Cytokeratins; osteoclast-like giant cells (OCL-GC) were negative. OCL-GC and histiocytoid mononuclear cells with nuclei similar to those within the OCL-GC present in this case, were strongly immunoreactive with KP-1, a recently developed monoclonal antibody directed against cells of mononuclear phagocytic lineage in routinely processed tissue section and showed immunoreactivity to vimentin. These findings of our case provide evidence against a carcinoma cells derivation and support a monocytic/histiocytic origin for the OCL-GC.

Breast Neoplasms↗

[Soft-tissue malignant melanoma (clear-cell sarcoma). Description of 2 cases with unusual histological features].

Malignant melanoma of soft part. Report of two cases with unusual histological features. Two examples of malignant melanoma of soft parts (clear cell sarcoma) are reported. In the first case the tumor cells showed predominantly eosinophilic cytoplasms; positive staining for S-100 protein has been observed immunohistochemically in the majority of the tumor cells. The second case, in recurrent lesion, showed a prominent "balloon cell" transformation of tumor cells. The clinico-pathologic picture, differential diagnosis and histogenetic problems are also discussed.

Collagenous spherulosis of the breast with atypical epithelial hyperplasia.

Collagenous spherulosis of the breast is a recently described lesion that was previously been seen only in a setting of benign, typical epitheliosis. We report that unusual case of a collagenous spherulosis of the breast in association with severely atypical epithelial hyperplasia in a 45-year-old woman. The patient presented an asymptomatic right lump and underwent a wide breast tissue resection. After a postoperative follow-up of twelve months, the patient remained well with no breast symptoms. Since atypical hyperplasia in the breast is considered predictive of relatively high risk of later carcinoma development, the finding of collagenous spherulosis with atypical hyperplasia should suggest a long-term follow-up. although at present the biological and prognostic significance of this association is unknown.

Breast↗

[Granular cell tumor of the male breast. Case report and review of the literature].

The authors report a case of granular cell tumor of the breast arising in a 38-year old man. Preoperative diagnosis was of breast carcinoma. However, frozen sections showed the benign nature of the tumour. It is important to identify this lesion as, in spite of a clinical and mammographic similarity with carcinoma, its behaviour is quite benign, and a simple tumour excision is the recommended treatment.

Prognostic significance of different biological markers (DNA index, PCNA index, apoptosis, p53, karyotype) in 126 adenocarcinoma gastric biopsies.

BACKGROUND: The different clinical evolution of gastric adenocarcinomas with the same clinico-pathologic characteristics prompted the authors to investigate the prognostic significance of different biological markers. METHODS: One hundred twenty-six preoperative cancer gastric biopsies, selected according the stage evaluated after following gastrectomy, were examined for DNA content by means the static cytometry, proliferating cell nuclear antigen (PCNA), p53 mutation, apoptosis by immunohistochemistry and karyotype using fluorescence in situ hybridization (FISH) techniques. RESULTS: The gastric cancers, ail belonging at stage III, were adenocarcinomas histologically typed as: 26 well differentiated (G1), 45 moderately differentiated (G2), 43 poorly differentiated (G3) and 12 of undifferentiated type with signet-ring cells. The tumours showed a prevalence of diploidy (68%), a mean PCNA-LI of 4.8%, trisomy of chromosome 7 in 40% of the cases, low presence of apoptosis (30%) and p53 mutation (17%). Only apoptosis was significantly correlated to histological diagnosis (p = 0.009). A multivariate analysis showed that the DNA content and PCNA-LI were the only independent prognostic markers for survival (p = 0.005 and p = 0.0002 respectively). CONCLUSIONS: The evaluation of these two biological variables, especially of the PCNA index, on gastric cancer biopsies may be useful in predicting the aggressiveness of each tumor and in identifying patients in need of additional perioperative therapies.
