Abnormalities of canines eruption.

The aim of this article is to present the personal experience on the treatment of 2 groups of patients presenting the same problem: the recovery of teeth 13 and 23 in the dental arch. The 1(st) group consists of adult patients aged approximately 20-28 years with a retained position of the permanent canines and permanence of the respective deciduous teeth in the dental arch. In the 2(nd) group, the loss of the deciduous teeth had not been followed by the physiological eruption of the permanent teeth. The authors' aim is to demonstrate that similar orthodontic treatments can represent valid solutions for the treatment of pathologies with different etiopathogenesis.

[Electrophysiologic monitoring of the facial nerve during otoneurosurgery].

The use of intraoperative facial electromyography (EMG) during otoneurosurgery facilitates anatomic and functional preservation of the VII nerve recording neural activation evoked both mechanically and electrically. The present work presents a personal experience involving the use of this technique in 27 cases of otoneurosurgery (16 neuromas of the acoustic nerve, 4 tympano-jugular paragangliomas, 5 VII nerve decompressions, 1 vestibular neurectomy, and 1 VII nerve neuroma monitored intraoperatively with Nicolet Nerve Integrity Monitor-1 (NIM-2). The latter is a highly sensitive instrument for intraoperative electromyographic recording. It is equipped with a system for electric stimulation, a display and a speaker for the EMG signal and acoustic alarm. During monitoring basically 3 types of EMG responses can be recorded: "burst", "train" and "pulse". In the present work the various EMG patterns are analyzed in detail as well as the relationship between their emergence and the type of surgical procedure performed. Intraoperative interpretation of the EMG activity makes possible more precise, intricate surgical maneuvers. It is felt that intraoperative monitoring has made it possible to preserve the anatomy and functionality of the facial nerve more frequently than is possible with other unmonitored cases. This becomes most evident when evaluating the long-term results.

[Monitoring of the diabetic pregnancy. Apropos of 34 insulin-treated cases].

This study concerns a series of 34 pregnant diabetic patients, insulin-dependent or requiring insulin, which were followed between January 1st 1980 and May 31st 1986. The blood level of glycolated hemoglobin (HbA1C) decreased constantly, demonstrating the improvement of the balance of the blood sugar, but its level, at the end of the pregnancy, could not be correlated with the presence of a macrosomia (27%) and/or of a neonatal hypoglycemia (24%). On the contrary, the infraclinical hypertrophy of the ventricular septum (HVS) diagnosed systematically by sonogram, and discovered in 6 newborns (nb) could represent a good marker of the harmful role of maternal hyperglycemia, even if moderate. Arterial hypertension, found in 8 cases, has a prognostic value as it is responsible for acute fetal distress (AFD), 2 cases, and for delivery by caesarean section (8 cases). A multidiscipline approach seems to improve the prognosis of pregnancy in diabetic patients, which nevertheless, should still be considered as a high-risk pregnancy.

[Thanatophoric nanism: a case of early ultrasonic diagnosis].

The diagnosis of a thanatophorous nanism is evoked in the presence of an abnormality of the size of the long bones. This study records the symptoms which, on sonograms, permit to recognize this fatal form of nanism among other forms of metaphysis chondrodysplasia, in order to establish a diagnosis before deciding to interrupt the pregnancy.

[Epidemiologic aspects of extrauterine pregnancy and therapeutic approach. Apropos of 200 cases in a continuous series].

The study of 200 files has enabled to define a few epidemiological aspects of ectopic pregnancy: large frequency of this pregnancy with ectopic location, for 42 births, in an average, occurrence in young women, between 25 and 35 years, hypofertile, nulli or primiparous, importance of utero-adnexal infections and their sequelae, found in 50% of the cases, and linked to the increase of sexually transmissible diseases and the frequency of voluntary interruptions of pregnancy, importance also of histological lesions of the isthmo-interstitial segment of the tube. The different therapeutic approaches are presented: the authors explain the reasons for a deliberately conservative therapy (in 82% of the cases) without compromising the maternal prognosis: absence of anatomical abnormality justifying the excision of the tube, or, often bilateral anatomical lesions requiring a subsequent radiological work-up, and a secondary treatment. Finally, desire of another pregnancy in a population which is infertile, most of the time, and of which the mean age is 28 years.

[When should one operate on polyps of the isthmo-interstitial portion of the fallopian tube?].

The authors analyze the diagnosis and the results of the treatment of 43 patients presenting simple polyps of the isthmo-interstitial segment of the tube with at least a 2 years follow-up. The study was carried out between 1980 and 1983. It was a prospective, non-randomized study. They conclude that the medical treatment, which gives excellent results, is advantageous. Microsurgical treatment (reimplantation or isthmo-ostial anastomosis) is only used after failure of the medical treatment.

Familial erythrophagocytic lymphohistiocytosis: adverse prognostic significance of delayed diagnosis.

Familial erythrophagocytic lymphohistiocytosis (FEL) is a rare disorder of the monocyte-macrophage system, for which an autosomal recessive mode of inheritance has been postulated. It is characterized by a dismal prognosis and is peculiar of early infancy. Three new cases of infants affected by FEL are reported. All three patients were diagnosed about three months after the onset of symptoms, and all three died shortly after diagnosis. The need for early diagnosis and prompt, intensive cytotoxic chemotherapy is emphasized.

Comparison of treatment with two different doses of leukocyte interferon alpha in patients with chronic hepatitis C.

BACKGROUND/AIMS: This study was carried in order to investigate whether human leukocyte interferon-alpha administered for 12 months at two different dosages, improves long-term responses in chronic hepatitis C and to see whether pre-treatment gamma-glutamyl transpeptidase values help to predict the clinical response to Interferon. METHODOLOGY: Forty-five patients were treated for 12 months with natural Interferon-alpha: 3 MU (group A: 31 cases); 6 MU (group B: 14 cases). Biochemical and virological responses were monitored during treatment and follow-up. RESULTS: Alanine aminotransferase was normalized in 58.1% (Group A) and 54.5% (Group B) of patients by the end of the treatment. Due to side effects 3 patients had to discontinue treatment. During follow-up, remission was maintained in 30.8% and 45.4% of patients respectively (p = 0.046). After 12 months of therapy, respectively 46.7% and 45.4% of patients with complete biochemical response, cleared virus from serum, as did, among long-term responders, 3/8 and 3/4 evaluated patients. Independently of dosage, a complete response was found more often in patients with normal pre-treatment gamma-glutamyl transpeptidase than in those with pre-treatment abnormal values. CONCLUSIONS: High dosage of IFN alpha was associated with a significantly greater rate of sustained biochemical response and with a better chance of viremia becoming negative. Pre-treatment gamma-glutamyl transpeptidase was able to predict the outcome of the treatment.

Immediate TRAM flap breast reconstruction in an obese patient with relapsing benign phyllodes tumor: report of a case.

The authors present a case of relapsing benign phyllodes cystosarcoma of the breast in an obese female treated with a simple mastectomy and an immediate reconstruction using a bipedicled transverse rectus abdominis musculocutaneous (TRAM) flap. After a brief review of treatment options for phyllodes tumors, they discuss the advantages of TRAM flap breast reconstruction, suggesting that it may also be applied in obese patients.

Titanium bone-integrated implants in extraoral facial prosthetic rehabilitation: surgical planning and long-term follow-up.

The authors report on their clinical experience in the reconstruction of complex facial deformities using titanium osseo-integrated implants for the retention of soft silicone prostheses. They also evaluate the importance of this surgical technique as a viable alternative to traditional reconstructive procedures using autologous grafts, both in patients with severe osteomuscular defects and corrective surgery of unsuccessful reconstruction operations. The patients who underwent implantation operations were studied by CT 3D and 99m Tc SPECT (Single Photon Emission Computerized Tomography) procedures to evaluate osseo-integration at 3 weeks, 3, 6, 12, 24 months. The study demonstrates that the radiation emission peaks three weeks after surgery with the maximum bone remodeling activity, and after the functional loading of the implants, 3 months after surgery. High uptake past the eight month after surgery has never been detected and must be considered abnormal. SPECT offers the possibility of obtaining a three dimensional reconstruction of the photon emission of selected structures. The use of these nuclear medicine methods in addition to traditional-type radiological procedures introduce new possibilities, although still in the clinical experimentation phase, for the long-term follow-up of the inserted implants in craniofacial rehabilitation.
