AIDS in Africa: what drugs do the carers want or need?

Our objectives were: (1) to discover requirements for treatment of patients with AIDS (PWAs) for health-care workers in eight English-speaking African countries; (2) to establish policies for supply of drugs, and develop a method for determining the contents of parcels (PWA-BOXes) for the relief of PWAs. Fifty-seven questionnaires were sent to non-government medical units treating PWAs, supplied by the charity Inter Care. Of these 37 units replied, two had no known PWAs, three were swamped by refugees; therefore, the total number analysed was 32. Only 24 units had access to HIV testing and the mean number of PWAs per unit was 58. The reported complications of AIDS were: diarrhoea 28 units; tuberculosis 27 units; pneumonia 28 units; sexually transmitted diseases 26; candidiasis 28 units; and herpes zoster 20. Lists of drug requirements were received. We present a protocol for calculation of contents of PWA-BOXes in the hope that this will provide guidelines for other workers in this field.

Influence of hydrogen peroxide on adenine uptake by iris, ciliary epithelium and corneal endothelium.

Hydrogen peroxide injected into the anterior chamber of adult rabbits causes a sequence of events that is not seen in young normal rabbits, yet is replicated in young rabbits receiving a catalase inhibitor. The uptake of radiolabeled adenine was measured in corneal endothelial, ciliary epithelial, and iris cells of either normal eyes, eyes injected with hydrogen peroxide or inhibitors of glutathione metabolism. Cellular adenine uptake did not exceed that of the bathing solution, except in corneal endothelia and ciliary epithelium of young animals. Under different experimental conditions intracameral hydrogen peroxide decreased adenine uptake into cells of various tissues compared the same experimental condition where only water injections was made. This occurred even with suppression of catalase activity using 3-aminotriazole. Intravitreal glutathione inhibitors had no effect on adenine uptake. The effects of intracameral hydrogen peroxide seen previously might be explained on the basis of alterations in cellular plasma membrane permeability.

Detection of virus-specific IgA antibodies in serum of kidney transplant patients with recurrent cytomegalovirus infection by enzymeimmuno and radioimmunoassay techniques.

The feasibility of using human cytomegalovirus (CMV)-specific IgA antibody determinations as a signal for early detection of recurrent CMV infections in eight renal transplant recipients was analyzed. Solid phase radioimmunoassay (RIA), enzyme-liked immunosorbent assay (ELISA) and immunoperoxidase assay (IPA) techniques were used for IgA antibody determinations. In parallel, IgG antibodies to CMV were studied by immunoperoxidase assay. A significant rise of CMV-specific IgG antibody titre was observed in all of these patients between 5 and 53 weeks post-transplantation. CMV-specific IgA antibody production was detected close to the time a rise in CMV IgG antibody was observed in seven out of eight patients studied by RIA and ELISA, and in six out of eight patients studied by IPA. In two patients specific CMV IgA antibodies were detected by all three methods before a significant rise of CMV IgG antibody titre was demonstrated. In these patients CMV IgA was detected by RIA earlier than by ELISA and IPA. The potential application of CMV-specific IgA antibody determination for early detection of recurrent CMV infection in renal transplant patients is discussed.

Osteoid osteoma of the distal phalanx of the thumb.

An osteoid osteoma of the distal phalanx of the thumb in a 14-year-old girl is described. Apart from the rare site of the lesion, other unusual features included early closure of the epiphyseal plate, enlargement of the involved segment of the thumb, and hypertrophy of the nailbed. The radiographic appearance was unusual in that it simulated a soft tissue mass eroding the bone from without.
