Urinary trypsin-inhibitor: extraction from pregnancy urine by affinity chromatography.

Affinity chromatography has led to the extraction of a trypsin inhibitor from urines of pregnant women, with a recovery of about 85%. The inhibitor obtained by this method is fairly pure, but heterogeneous, according to several criteria. The heterogeneity is not the result of the extraction procedure, but it seems to pre-exist in the urine itself.

Preliminary study to antituberculosis chemotherapy in a region of sub-Saharian Africa: annual risk of tuberculosis infection calculation and tuberculin survey.

A tuberculin survey was performed in Arua District (577,799 inhabitants, recent return of refugees from Zaire and Sudan) in 1987 prior to the implementation of antituberculosis Chemotherapy and Control Programme. 50 clusters (schools) of 30 pupils each were selected in the 7 Counties by Sistematic Random Sampling. 1110 students 10 year old without BCG vaccination scar were injected with 5 IU of PPD; 1016 of them (more than 6% of the estimated district population of 10 year of age) came back after 72 hours for reading. 125 of them were positives (more than 10 mm of induration). The Infection Rate detected was 1.23% +/- 0.19. Considering the infection rates found by the Ugandan National Surveys done in 1950 (2.6%) and in 1970 (2.3%), and the slight slope of the curve calculated on those values we have underestimated the real Infection Rate in the district. To avoid this bias we suggest to include schools in sampling (it is simple and cost-effective) only if the Student's population is likely to be representative of the general population.

[Urethro-vesical-vas deferens reflux as a cause of acute scrotum].

The Authors reviewed 159 children who underwent surgical therapy for "acute scrotum", and they consider that the torsion of the appendix testis is the most common cause of the syndrome in children. On the other hand the orchi-epididymitis is very rare in paediatric age. About the incidence and etiopathogenesis of orchi-epididymitis is reported the revision of the literature; in particular is described the condition of the urethro-vesical-deferential reflux causing a chemical or infective orchi-epididymitis.

Effects of ubidecarenone in an exercise training program for patients with chronic obstructive pulmonary diseases.

A study was undertaken to determine the usefulness of ubidecarenone in pulmonary rehabilitation in exercise training programs in the management of chronic obstructive pulmonary disease (COPD). The subjects were 20 patients with COPD who had been participating in an exercise training program for at least four weeks. The patients were randomly assigned either to receive 50 mg of oral ubidecarenone daily or to enter a control group during the program. Oxygen consumption, expired volume, and heart rate were measured during exercise tests before and after training. Maximum oxygen consumption increased 13% in the ubidecarenone-treated patients and 7% in the controls, and maximum expired volume increased 10% in each group. The increases were significant in the ubidecarenone group but not in the controls. Heart rate increased 2% in both groups. It is concluded that ubidecarenone deserves further evaluation in exercise training programs for patients with COPD.

[A clinical comparative study of crystalline pure lactulose and powder pure lactitol in portasystemic encephalopathy of cirrhotic patients].

The efficacy and the tolerability of the new crystalline pure lactulose formulation (Laevolac Cristalli) vs lactitol is evaluated in 40 patients suffering from liver cirrhosis and treated for the associated encephalopathy (PSE). Both disaccharides proved to be effective in the maintenance therapy of PSE. With respect to the previous formulation of lactulose, the crystalline one has a significantly lower incidence of side-effects. Pure crystalline lactulose, showing a similar efficacy and rise of side-effects, proved to be better accepted by the subjects of this study.

[Extralobar pulmonary sequestration in children: presentation of a case].

The pulmonary sequestration is an anatomical condition with as isle of pulmonary tissue independent from tracheobronchial tree. This tissue has an independent artery originated from aorta. Anatomically we can have two types of sequestration. The first is the condition of an isle of pulmonary tissue included without any separation in the lobe; this condition in named intra-lobar sequestration. On the other hand we have a second condition with an isle of independent pulmonary tissue surrounded with independent pleura; the last one is named extra-lobar sequestration. In the present article the authors describe a case of extralobar sequestration in a two years old girl. The symptoms presented by the girl were hyperpyrexia, tough and recurrent bronchopulmonary infections. The girl underwent surgery removal of sequestration. The authors describe their experience, the importance of symptomatology and the anatomo-pathologic characteristics of this lesions.

[Conservative treatment of post-traumatic periduodenal hematoma: presentation of a clinical case and review of the literature].

The authors describe a case of post-traumatic hematoma of the duodenum in a two years old child. This is a very rare condition in paediatric age, but with the increasing incidence of traumatic pathology it can be predicted an increase of this condition. The duodenal hematoma can be treated either with surgical approach or with conservative treatment. The authors discuss the characteristics of the pathology, the etiopathogenetic mechanism, and propose the same classification adopted by some french authors. The type of lesion indicates the adequate therapy; in particular in the case report (type 1) a conservative treatment is adopted. The laboratory parametres are significative but none is diagnostic. In the present case the amylasemia is not elevated until two weeks after the trauma. Very important for diagnosis are T.C., roentgenography and echotomography, the last one can be easily used for the control of the resolution of the hematoma. The conservative treatment is based on nasogastric suction and parenteral nutrition; although the role of nonoperative treatment is emphasized, the importance of careful assessment for signs of peritonitis or enlarging tender mass cannot be overlooked.
