Plasma cell acid phosphatase as an adjunct in the differential diagnosis of monoclonal immunoglobulinemias.

Plasma cell acid phosphatase has been studied in 51 patients, 30 of whom were affected with multiple myeloma (MM) and 21 with nonmyelomatous monoclonal immunoglobulinemias (NMMI). Scoring of results displayed a highly significant difference between the median of the MM and NMMI group (343 vs. 204). The use of plasma cell acid phosphatase as a further criterion in the differential diagnosis of monoclonal immunoglobulinemiasis emphasized.

Monocytic differentiation of cluster cells in agar culture of normal human bone marrow. A cytochemical investigation.

A cytochemical investigation was carried out on the cells that make up the clusters forming in agar cultures of normal human bone marrow after 3, 4, and 5 days of incubation. All the clusters were found to comprise young monocytes that were PAS, peroxidase, Sudan Black, and alpha-naphthyl and naphthol-ASD-acetate esterase positive. Lysozyme activity, investigated by the cytobacterial test, was absent in all cases. After 3 days of incubation a large number of pairs of lymphomonocytes or monocytes with the same cytochemical characteristics were observed. When a method of double esterase incubation (alpha-naphthyl or naphthol-ASD-acetate and naphthol-ASD-chloroacetate) was performed on the same slide, no cells of the granulocytic line or intermediate cells between these and the monocytes were observed in any of the clusters. These results indicate that there is direct monocytic differentiation of the colony-forming cells cultured. The origins and stages observed in the maturation of the monocytes are discussed.

Cytochemical study of acute promyelocytic leukaemia.

Five cases of acute promyelocytic leukaemia (A.P.L.) are investigated for peroxidase, PAS, toluidine blue, astra blue, alpha-naphthyl esterase, double esterase incubation (naphthol-AS plus naphthol-AS-D chloroesterase), and cellular lysozyme activity. These cytochemical investigations may contribute further characterization of the morphologic type.

Guaianolides and eudesmanolides from Centaurea ornata.

Four sesquiterpene lactones and three other compounds have been isolated from the EtOH-H2O-soluble fraction of a CHCl3 extract of Centaurea ornata. A new grosshemin derivative 3 is described, and the stereochemistry of natural 11-epi-dihydroreynosin [2] is revised.

[Pericardial involvement as initial manifestation of multiple myeloma].

Pericardial involvement is a rare complication of multiple myeloma, caused by amyloidosis, infections, or plasmacell infiltration, usually at late or terminal disease stage. We report a patient with pericarditis coming from a department of Cardiology where a preceding (15 years before) diagnosis of breast cancer and present bloody pericardial effusion with probably malignant cells permitted at first to orientate towards metastatic pericardial involvement in breast cancer. Laboratory findings (pancytopenia, hypogammaglobulinemia, proteinuria) suggested to perform bone marrow aspirate, serum and concentrated urine immunoelectrophoresis, measurement of 24-h urine protein excretion, and further cytologic and immunocytochemical assay of pericardial fluid. Acquired data allowed to diagnose light chain multiple myeloma with pericardial involvement caused by plasmacell infiltration. We diagnosed this complication, representing first and main clinical feature of multiple myeloma, owing to a complete clinical and laboratory evaluation and repetition of cytologic and immunocytochemical assay of pericardial fluid.

[Tissue expression of carbohydrate antigen 19-9 (CA 19-9) in adenomatous lesions of the colorectum].

Serum levels of tumor markers almost not detectable in precancerous states or early cancer, the behavior of the tumor associated antigen CA 19-9 in colorectal carcinogenesis was analysed. In order to investigate the adenoma-carcinoma sequence, tissue antigenic expression was measured in adenomas, characterized by different size and histology, and compared with that found in normal colonic mucosa. Tissue content of CA 19-9 was determined by an immunoperoxidase technique (ABC-POD) in 88 colonic polyps, and correlated to the degree of histological dysplasia, and dimension of adenoma. Tissue content of CA 19-9 was also evaluated in non-adenomatous mucosa obtained by endoscopic biopsy. Among the 88 polyps, 50 showed the tubular histological type, while 31 resulted tubulovillous and 7 villous. High degree of dysplasia was present in 7 adenomas (7.9%) and focal carcinoma was observed in 6 (6.8%). Positivity for CA 19-9 was registered in 60.2% of adenomas. No correlation was found between tissue-CA 19-9 and degree of dysplasia, size of adenoma and villous component. However, a statistically significant correlation was observed between expression and cellular distribution of the antigen (chi 2 = 98.07, p < 0.00001). Our data confirmed CA 19-9 expression in adenomas, but it is unlikely this tissue antigen proves to be a reliable marker of adenoma-carcinoma sequence.

[The preoperative staging of rectal neoplasms: the clinical exam and diagnostic imaging].

The management of rectal cancer remains an important clinical problem. Although there was been great progress in surgical management, the survival of patients with locally advanced disease has not improved significantly during the past decades. Preoperative staging and evaluation of the risk of recurrence may help in the choice of operation. It is difficult for clinicians to quantify reliably with digital examination the degree of fixation of the tumor, and they usually cannot distinguish nodal metastases except in advanced cases. The more frequent overstaging of small tumors within one quadrant of the rectum is a major drawback of digital examination. Computed tomography and magnetic resonance seems to underestimate the extension of rectal tumors, but both can be helpful in selecting patients with advanced tumors for whom preoperative adjuvant treatment is being considered. Endoluminal ultrasound is superior in staging tumors confined to the rectal wall, but is not the ideal tool for staging: the results are examiner dependent, the field of vision in depth is limited, and stricturing tumors cannot be passed by the ultrasound transducer. Imaging diagnostic attendibility confirms the preeminent role of intraoperative exploration in the assessment of neoplastic diffusion in order to plan a correct surgical treatment.

[Computers in surgery].

The introduction of computers in medicine, particularly for what surgery is concerned, has many implications related to its optimal utilization. Possible applications as well as advantages and limits of such recording system in a surgical ward are examined. Emphasis is placed on the clinical data management model and the ways the different structures of the system are related. The computer processing of these data provides valid material for either clinical and surgical research or statistical studies.

Breast involvement in malignant blood diseases. A report of four cases and review of the literature.

The breast involvement in malignant blood diseases is a very rare manifestation. Four cases observed by the Author in the last 12 years, consisting of one male with Multiple Myeloma (MM) stage III A, and three females with Non-Hodgkin lymphoma (NHL), in IA-E, IIIA-E and IV A stage respectively are described. The presence of breast plasmacytoma, lymphoblastic or lympho-plasmocytic type lymphoma observed in 3 of 4 cases has been rarely reported.
