Simultaneous infection with non-A, non-B and B viruses in a patient exhibiting different histologic patterns of acute hepatitis.

We present the case of an elderly female patient who developed acute and severe hepatitis with two distinct peaks following blood transfusion. The serological markers and histopathology during the first attack were characteristic for hepatitis non-A, non-B(NANBH), whereas the second peak was due to hepatitis B virus (HBV) infection. Electron microscopy revealed morphologic equivalents for both hepatitis agents.

Loss of Hydrogen from Carbon 5 of d-Glucose during Conversion of d-[5-H,6-C]Glucose to l-Ascorbic Acid in Pelargonium crispum (L.) L'Hér.

Conversion of d-[5-(3)H,6-(14)C]glucose to l-ascorbic acid in detached apices of Pelargonium crispum (L.) L'Hér cv Prince Rupert (lemon geranium) was accompanied by complete loss of tritium in the product. Chemical degradation of d-glucose which was recovered from the labeled apices yielded d-glyceric acid (corresponding to carbons 4, 5, and 6 of glucose) with a (3)H:(14)C ratio of 4 to be compared with 9, the ratio in d-[5-(3)H,6-(14)C]glucose initially. Conversion of d-[6-(3)H,6-(14)C]glucose in the same tissue was accompanied by retention of tritium in l-ascorbic acid with a (3)H:(14)C ratio comparable to that of compounds from the hexose pool. Results indicate that during l-ascorbic acid biosynthesis from glucose in Pelargonium crispum hydrogen at carbon 5 undergoes exchange with the medium, suggesting an epimerization at this carbon atom.

Renal bile acid excretion as a cause of neoplastic lesions in the urinary tract after total portacaval shunt in the normal rat?

Rats with a total portacaval anastomosis (PCA, PC-SS) develop preneoplastic and neoplastic lesions in the urinary tract. In contrast to this, animals with a modified shunt (mPCA) do not develop these lesions. To evaluate the possible role of bile acids excreted with the urine for tumor development, total plasma bile acid concentration and 24 hours urinary bile acid excretion were measured radioimmunologically in rats with total and modified shunts. Additionally the renal 14C-glycocholic acid excretion into the urine was studied after oral administration. Total plasma BA increased from 4.89 +/- 1.0 mumol/l in sham-operated controls to 77.7 +/- 39 mumol/h in PCA and 52.9 +/- 36.7 mumol/l in mPCA rats (p less than 0.001 vs controls, PCA vs mPCA = n.s.). Urinary bile acid excretion rose from 0.2 +/- 0.29 mumol/24 hours in controls to 4.47 +/- 4.49 in PCA and 2.55 +/- 2.22 mumol/24 hours in mPCA rats (p less than 0.001 vs control; PCA vs mPCA = n.s.) 14C-glycocholic acid was excreted within 24 hours into the urine in 13.6 +/- 11.5% in PCA and 26.3 +/- 23.5% of the administered dose in mPCA-rats (controls; 2.98 +/- 0.67%, p less than 0.001; PCA vs mPCA = n.s.). Since renal BA-excretion is similar in both shunted groups, urinary BA does not seem to be of primary significance for cancer development in the urinary tract of totally shunted rats.

[Effect of copper status on reproductive performance and milk yield of female cattle in two copper deficient habitats. Effect of supplemental copper feeding on copper status].

The influence of a nutrition poor in Cu on the copper status of dairy cows was investigated with and without Cu-supplementation through the mineral mixture typical of the territory and enriched with Cu. The roughage produced in the territory investigated contained less than 8.0 mg Cu/kg and did not guarantee a Cu supply of the dairy cows meeting their needs. Feeding the mineral mixture rich in Cu improved the Cu-status of the dairy cows, did, however, in one of the territories not result in a Cu-incorporation in the liver that meets the needs. Possibly the reason for this divergent behaviour is an oversufficient S- and Fe-supply. In contrast to this, the Cu-content of the cerebrum of the cows in both territories showed the effect of the Cu in the mineral mixture.

[Effect of the copper status on the reproduction performance and the milk yield of cows in 2 copper-deficient locations. 2. Effect of copper deficiency on the reproduction performance and the milk yield of cows].

The influence of copper deficiency on the reproduction performance of cows has not often been investigated. In two habitats feeding cows with mineral mixtures rich in copper resulted in a significant reduction of the aperiodically extended sexual cycles. The beginning of the first visible symptoms of heat, the period between gravidities, the duration of gravidity, the birth weight of the calves and the Cu-content of the colostrum remained uninfluenced by Cu-supply. The liver weight increase of the calves from cows without Cu-supplement during gravidity was by 5% lower during the first 90 days of life than that of the offspring of cows with additional intrauterine Cu-supply (p greater than 0.05). The mineral mixture increased the milk yield by 0.4 resp. 0.5 kg and the milk fat content by 0.10 resp. 0.13%. This resulted in an excess production of 32 resp. 62 g milk fat per cow and day of the experiment (p greater than 0,05).

[Iodine metabolism. 2. Effect of iodine deficiency on reproduction and the development of progeny of goats and miniature pigs].

The influence of J-deficiency on reproduction was tested in model experiments with goats and mini pigs, partly through several reproduction cycles including the descendants after intra-uterine depletion as well. The effect of 0.05 mg J/kg dry matter in the concentrated feed (total ration 0.03)--0.04 mg J/kg dry matter) for goats was a diminished success of first inseminations, a higher frequency of abortions, high mortality and poorer development of the kids. All J-deficiency goats and kids suffered from hypertrophy of the thyroid gland, half of all J-deficiency kids were born hairless. Concerning the mini pigs, 0.06 mg J/kg dry matter even after intra-uterine depletion had no detectable influence either on conception or the mortality and development of the piglets. The thyroid gland of all deficiency pigs was enlarged, however. The hypertrophy of the thyroid gland makes it easy to diagnose J-deficiency visually.

[Significance of cellularly bound immunoglobulin G in chronic hepatitis induced by hepatitis B virus (author's transl)].

In vivo binding of immunoglobulin G was determined in hepatocytes of 15 patients, who suffered from hepatitis B virus induced chronic hepatitis. The incidence of hepatocytes containing immunoglobulin G bound in a granular pattern correlated positively with HBcAg within the nuclei and negatively with HBsAg in cytoplasma. The third component of complement could not be demonstrated in the hepatocytes. No correlation was found between the degree of immunoglobulin G fixation and inflammatory activity within the liver. It is suspected that immunoglobulins deposited as granula within the cell are identical with phagocytized immune complexes, which may be composed of HBcAg and anti-HBc.

[Investigation of iodine metabolism. 1. The influence of iodine deficiency of the feed consumption and the development of the live weight of goats and dwarf pigs].

With goats and dwarf pigs two experiments were made with semi-synthetic rations. The influence of insufficient (0.05 resp. 0.06 ppm) and sufficient (0.42 resp. 0.56 ppm) supply with iodine on feed consumption and the development of the live weight during the growth period and gestation was investigated. While iodine deficiency resulted in diminished feed consumption and a growth depression of the goats, a negative influence on feed consumption and the live weight increase of the dwarf pigs could not be detected, feed expenditure per kg weight increase, however, was distinctly higher.

[Quantitative changes of portal and arterial blood supply of the liver following porta-systemic shunting procedures (author's transl)].

Hepatic hemodynamic alterations following various porta-systemic shunting procedures were evaluated in the normal and cirrhotic rat liver using a microsphere embolization method. The aim of the study was to differentiate the hepatic arterial and portal venous contribution in a defined cirrhotic rat liver model following total shunting procedures and to determine residual portal blood supply after selective shunt procedures intended to prevent post shunt liver failure. Experimentally selective shunt procedures such as mesentericocaval shunt, splenocaval shunt, portacaval transposition and even portacaval side/side shunting lead to improved hepatic blood supply compared with total end/side shunting. Improvement of liver blood supply is caused rather by arterial compensation than by residual portal blood perfusion. The amount of portal blood supply is not related to a specific type of selective shunt. Evaluation of hepatic arterial compensatory capacity is a more reliable method of predicting total liver blood supply following any shunt procedure.

[Problems of hepatoportal encephalopathy after portacaval anastomosis in the rat (author's transl)].

Necrosis and injury of ganglia cells as well as alterations of the glia represent the morphological changes in portal-systemic encephalopathy. This is a frequent complication of a spontaneous or surgical porta-caval collateral circulation. Such changes can also be observed when porta-caval end-to-side anastomosis (PCA) was constructed in otherwise healthy rats. Increased incorporation rates of 3H-thymidine into the glia were indication for enhanced proliferating activity. Efforts done to maintain the pancreatic venous blood flow to the liver beside PCA resulted in significant diminuation of brain pathology and function. The latter measured by EEG. This happened although the grade of hyperammonemia was virtually identical in both settings. The interpretation is offered that access of the liver to hepatotrophic substances (i. e. insulin) from the pancreas prevents portal-systemic encephalopathy in PCA which is in agreement with data of the literature. The observation implies that in clinical action exclusively those surgical techniques should be used by which the venous blood supply from the pancreas to the liver is maintained.

[Endotoxinemia in acute pancreatitis (author's transl)].

In a clinical study the question was asked, whether endotoxinemia of intestinal origin accompanying acute pancreatitis would influence decisively the course of this disease. In 21 of 32 patients with acute pancreatitis endotoxinemia could be diagnosed by the Limulus test, in this group 9 patients died. In the group of patients without endotoxinemia however no patient died. Extrapancreatic complications as for instance coagulation defects, renal failure, and pulmonary changes were predominant in the patients with endotoxinemia. From these findings the conclusion is made, that endotoxinemia occurs frequently in acute pancreatitis and influences decisively the course of the disease.

[The supply of wild ruminants with major and trace elements. 1. The cadmium content of winter grazing and the cadmium status of red deer, fallow deer, roes and moulons].

The Cd-determination of 10 different winter grazings for wild ruminants in seven different biotopes showed significant differences of the habitats resulting from the previous Cd-emission of metallurgy smelting non-ferrous metals. The winter grazing of the Tharandt Forest and the Harz mountains contained significantly more Cd than that of the Lower Flaming hills, the western Erzgebirge and the northern agrarian areas. The bark and the tips of conifer twigs, twigs of oak and mountain ash, bilberry and heather plants, i. e. perennial plants resp. parts of plants proved to be particularly rich in Cd. On an average, wild ruminants consume more Cd than domesticated ruminants, which do not consume parts of perennial plants. The rumen contents of all wild ruminants contained, with 0.20 to 0.25 mg/kg dry matter more Cd than could be expected from the content of the grazing. In the Cd-contaminated areas the amount of Cd in the rumen contents too showed the Cd-load. The kidneys, livers and the covering hair of a total of 172 head red deer, roes and mouflons reflected the Cd-load and the dependence of the Cd-level on the age. The Cd-concentration ascertained in the organs of the wild ruminants was compared with that of sheep and cattle. The, on an average, higher Cd-load of the wild ruminants was confirmed. The kidneys of wild ruminants older than five years should, as a general principle, not be eaten and neither should the livers of animals from Cd-contaminated biotopes. The limb defects of mouflons from Cd-contaminated areas are discussed in connection with Cd-induced secondary Cu-deficiency resp. Mn- and Zn-deficiency. A reference is made to further works.

On the pathogenesis of galactosamine hepatitis. Indications of extrahepatocellular mechanisms responsible for liver cell death.

In order to evaluate the pathogenesis of galactosamine hepatitis, the action of galactosamine on mast cells, and alteration in the complement system suring the course of this experimental injury were studied. It has been previously demonstrated that rat livers after colectomy are refractory to galactosamine-induced liver cell necrosis and inflammation. For this reason colectomized animals were used to see whether the biochemical alterations produced by this aminosugar and thought to be responsible for cell death developed. Results showed: 1. galactosamine potently degranulates mast cells in vivo and in vitro, 2. the complement system is a) activated during the course of galactosamine hepatitis, probably by circulating endotoxins, and b) is essential for liver cell death in galactosamine hepatitis, and 3. colectomy does not prevent biochemical changes known to occur during galactosamine metabolism. It is concluded that death of galactosamine-injured liver cells is triggered by extrahepatocellular mechanisms, which lead ultimately to an activated complement system by endotoxins. It is postulated that related mechanism may also occur in viral hepatitis and in fulminant hepatic failure in man.
