Organ culture of human somatotrophic pituitary adenomas: ultrastructure and growth hormone production.

Ten somatotrophic adenomas removed from acromegalic patients and fragments of the non-tumoural surrounding pituitary were submitted to organ culture for periods of up to one month. Electron microscopic observation shows that these tumours retain their histological differentiation throughout the culture period. The cell morphology of the cultured tumours remains essentially unchanged and in particular the secretory granules keep their initial size (150 and 130 nm). However, the granules disappear gradually so that most of the cells look chromophobic by the 4th week of culture, and numerous lysosomes as well as autophagic figures appear at the same time. The hGH concentration in the culture medium has been measured of 4 adenomas. It is very high (10-fold greater than that from non-tumoural pituitary medium) during the first week (range 200-300 mug/ml). It still remains very high in the same experiments until the second week of culture in one experiment (200 mug/ml). After incubation of cultures with 3-H-leucine 3 H-hGH is obtained in the medium giving evidence of hormone synthesis by adenoma cells in culture. 3-H-hGH represents 40 percent of 3-H-proteins in the culture medium and gives the same elution pattern as standard hGH on Sephadex G 100 chromatography. A certain degree of correlation is observed between morphological and biological results: the greatest hGH production is obtained from explants which maintain the best histological appearance.

[Study of the hypothalamo-pituitary adrenal function in 16 patients after surgery for pituitary tumor (author's transl)].

The hypothalamic pituitary adrenocortical function has been studied in 16 patients operated from pituitary tumors (13 adenomas; 3 craniopharyngiomas). Comparisons have been made between corticotropin and cortisol response to lysine vasopressin, insulin induced-hypoglycemia and metyrapone IV and per os. Among these different stimulating tests, insulin induced hypoglycemia and metyrapone per os seem to give the more accurate informations metyrapone per os being more convenient because harmless. Three different groups of patients have been distinguished : one without adrenocortical deficiency; one with a complete deficiency and a third group with a partial deficiency. Correlations have been studied between the degree of the adrenocortical deficiency, the volume of the tumor and the presence of the absence of other anterior pituitary dysfunctions.

[Two cases of gluten intolerance presenting as dwarfism (author's transl)].

Two patients with dwarfism were found to have intolerance to gluten. The authors describe the clinical, biological and radiological signs which enable the growth disorder to be related to its true cause, when obvious signs of a celiac syndrome are lacking. In fact, a small intestine biopsy is the essential diagnostic procedure. The hormonal changes found in such patients are discussed, and include normal HGH levels, reduced somatomedin activity which becomes normal after a gluten-free diet, and low blood insulin levels in the basal state as in insulin tolerance tests.

[Severe dwarfism with high plasma hGH levels and no somatomedin acitvity: Laron's syndrome (author's transl)].

A case of Laron type dwarfism which is reported describes a clinical picture of isolated previous deficiency in growth hormone. But serum level of this hormone is found to be high both in regard to basic value and after ornithin stimulation. The somatomedin activity of the child serum was studied by two methods: biological assay and binding assay. This activity was found to be non-existent both in regard to basic conditions and after injection of hGH corroborating the diagnosis of dwarfism of Laron type.

[Biosynthesis and hormonal regulation of IGF (insulin-like growth factors, or somatomedins). Experimental and clinical studies (author's transl)].

Rat liver explants in culture release into the culture medium IGF and their carrier which form a complex with an apparent molecular weight of similar to or approximately 40,000. With gel filtration in acetic acid the IGF are separated from their carrier and each can then be measured by radioligand assay. In terms of their hormonal control "in vitro", GH, insulin and cortisol have an independent action on each. Pituitary and various brain tissues in organ culture also produce IGF and their carrier. The "in vitro" regulation of the IGF and their carrier has been investigated through assays of their levels in human serum under normal and pathological conditions, such as hypopituitarism, acromegaly, coeliac disease, Cushing's syndrome and idiopathic short or tall stature.
