Current management of mediastinal tumors.

The incidence of mediastinal tumors is increasing throughout North America. The benign variants are usually managed by relatively simple surgical procedures. Malignant mediastinal tumors can be among the most challenging of medical and surgical conditions and may require the expertise of medical, radiation, and surgical oncologists. Thymomas, germ-cell tumors, and lymphomas comprise the majority of the malignant mediastinal neoplasms. Some variants are managed entirely medically while others are extirpated initially. Many require multimodality therapies, either according to an accepted protocol or, more challengingly, off protocol.

[Prevalence of health problems in the Gazel (EDF-GDF) cohort: regional distribution and disparities].

This study deals with regional differences in annual prevalence of 44 declared diseases or clinical disorders in 1989 in GAZEL cohort participants from "Electricité de France-Gaz de France" company. The studied sample consisted of 20,325 people, from 35 to 50 years old, living in France, who answered to a mail questionnaire in 1989. The collected data from one year were analysed. An overall "regional effect" was searched about every listed disease in the questionnaire. For this purpose, a logistic model, controlling for age and sex, was used. For comparing prevalence of a disease in one region with average prevalence, an odds ratio (OR) was calculated: it was defined here as the ratio of disease odd in the region to the average French odd. The regional effect was studied in greater detail for five diseases, making allowance for extra factors of adjustment (tobacco and alcohol consumption, Quetelet index, salary category, number of declared diseases). The regional effect remained significative for four diseases: hypertension with OR from 1.26 to 1.68 in northern regions, and from 0.63 to 0.75 in southeastern ones; osteoarthrosis with OR lower than 1 in western regions (0.60 to 0.79), and larger in the South (1.24 to 1.46); hyperthyroidism, goiter with OR elevated in Midi-Pyrénées (2.91) and Lorraine (1.82) [corrected]; renal stones with OR high in the South (1.85 in Provence Riviera). These results can be compared to known observations about geographical differences in mortality. They also suggest persistence of hazardous areas for some diseases.

Galactosylceramide lipidosis (Krabbe's disease) and deciduous dental tissues. A case report.

The deciduous teeth of a 5-year-old girl affected by a protracted type of infantile globoid-cell leukodystrophy or Krabbe's disease, a galactosylceramide lipidosis, were examined with the scanning and transmission electron microscopes. The diagnosis was established on the basis of galactosylceramidase activity (0.04 mu katal/kg proteins versus 0.86 +/- 0.49 in the normal situation) and abnormalities in the white matter and trouble in myelinization assessed by magnetic resonance imaging. In spite of the fact that enamel, dentin and pulp are extraneural tissues, they were also affected by the disease. Ultrastructural observations of dental mineralized tissues revealed enamel hypoplasia as well as abnormal proteinaceous lamina and hypomineralized lines or bands. Inside the mantle dentin, dendritic inclusions of amorphous material were seen. Lysosomal storage inclusions were observed in all the cells of the dental pulp. The myelin sheaths of dental peripheral nerves displayed severe degenerative changes. Proliferation of dentinoclasts contributed to stimulate root resorption.

[CO2/O2 anesthesia for the castration of male piglets (preliminary results)].

Introduction of anaesthesia with CO2/O2 (60% to 40%) is possible within 90 and 120 seconds. There are moderate to excessive excitations occurring as part of state II of anaesthesia. Anaesthesia (during castration) in CO2/O2-atmosphere produces excellent analgesia and relaxation. The duration of castration surgery is much shorter under CO2-anaesthesia than without anaesthesia. Blood cortisol levels are significantly higher after castration without CO2-anaesthesia. About 5 minutes after CO2/O2-anaesthesia and castration surgery, piglets are already awake and standing.

HER-2/neu: a differentiation marker in adenocarcinoma of the esophagus.

Adenocarcinoma of the esophagus is an aggressive malignancy which is increasing in frequency. The HER-2/neu oncogene product is a putative differentiation marker which exhibits decreased expression in colon carcinoma, while there is overexpression in breast and ovarian cancers. We analyzed the relationship of cell differentiation to HER-2/neu expression in esophageal adenocarcinoma using immunohistochemistry on formalin-fixed, paraffin-embedded material from 14 patients whose tissue contained normal, dysplastic, and malignant features. HER-2/neu expression was detected in 1 of 14 biopsy specimens and 2 of 9 resection specimens. The oncoprotein staining was greatest in normal tissue, less in dysplastic tissue, and not detected in malignant tissue. Our findings, similar to what is seen in the colon, suggest that the HER-2/neu oncogene product is a differentiation marker which is lost in esophageal adenocarcinoma.

Sinonasal cancer and occupational exposure to formaldehyde and other substances.

A case-control study of cancer of the nose and paranasal sinuses was conducted in France to determine whether occupational exposure to formaldehyde was associated with an increased risk of sinonasal cancer. Exposures to 14 other substances or groups of substances were also studied (wood dust, leather dust, textile dust, flour dust, sugar dust, coal/coke dust, nickel compounds, chromium compounds, chromium VI, welding fumes, soldering fumes, cutting oils, paints and lacquers, glues and adhesives). Cases (n = 207) and controls (n = 409) were interviewed to obtain detailed information on job history and other potential risk factors for sinonasal cancer. In addition, a questionnaire specially designed for this study was used to help assess exposures to formaldehyde and other substances of interest. The questionnaires were translated into history of occupational exposure by an expert in industrial hygiene, without knowledge of case-control status. Several exposure variables (lifetime average level, duration, cumulative level) were used to describe the risk related to exposure to formaldehyde. Potential confounding factors (occupational and non-occupational) were examined and adjusted for when necessary. No significant association was found between exposure to formaldehyde and squamous-cell carcinomas of the sinonasal cavities. Because of the strong association between exposure to wood dust and nasal adenocarcinoma, it was not possible to assess an independent effect of formaldehyde on this type of cancer. However, among males exposed to medium or high levels of wood dust, the risk of adenocarcinoma associated with formaldehyde was significantly elevated for the highest exposure categories for average level (OR = 5.3, 95% confidence interval = 1.3-22.2), cumulative level (OR = 6.9, 95% CI = 1.7-28.2) and duration of exposure (OR = 6.9, 95% CI = 1.7-27.8). Although a residual confounding effect of wood dust could not be excluded, this study suggests that exposure to both formaldehyde and wood dust may increase the risk of nasal adenocarcinoma, by comparison with the risk due to wood dust alone. This study also indicated an increased risk among males who had been exposed to glues and adhesives, for all histologic types, which was not explained by a confounding effect of paints and lacquers, wood dust or formaldehyde. No other significant association was observed.

Suramin-induced mucopolysaccharidosis in rat incisor.

Two weeks after a single injection of suramin, the secretory and post-secretory ameloblasts of the rat incisor were filled with large lysosome-like vacuoles. At the light-microscope level, these vacuoles were positively stained with Alcian blue when MgCl2 was used at a critical electrolyte concentration varying between 0.1 and 0.3 M, whereas no staining appeared when MgCl2 varied between 0.7 and 0.9 M. Hyaluronidase digestion markedly reduced but did not totally abolish the staining, indicating that glycosaminoglycans were accumulated inside these vacuoles. Examination of these cells with the electron microscope revealed a polymorphic population of large vesicles, filled to various degrees with cetylpyridinium chloride (CPC)-positive and malachite green aldehyde (MGA)-positive material. The same pattern was observed in secretory odontoblasts but to a lesser extent. In the extracellular matrix, suramin-induced alterations appeared as large defects occurring during enamel formation. In predentin and dentin, the number and/or size of electron-dense aggregates resulting from CPC and MGA fixation, were enhanced in the suramin-injected rats. These aggregates were largely reduced or suppressed respectively by hyaluronidase digestion and chloroform/methanol treatment of the sections. The accumulation of glycosaminoglycans and phospholipids reported here inside ameloblasts and odontoblasts and in predentin and dentin supports the occurrence of suramin-induced mucopolysaccharidosis and lipidosis in this experimental animal model.

Delayed myocardial laceration after intrapericardial pneumonectomy.

Curative resection for large central bronchogenic tumors may require radical or intrapericardial pneumonectomy. Myocardial herniation through the pericardial defect is a rare early postoperative complication. Prevention of cardiac herniation and subsequent hemodynamic compromise is always necessary after intrapericardial pneumonectomy.

Emergency department-based surveillance for syphilis during an outbreak in Philadelphia.

STUDY OBJECTIVE: To identify missed opportunities for syphilis treatment during an outbreak. DESIGN: Prospective prevalence survey. SETTING: Urban hospital emergency department. PARTICIPANTS: Nine hundred sixty-one persons aged 15 to 44 years seeking medical attention in the ED who were not suspected of having any sexually transmitted disease (STD) at the time of their visit. INTERVENTION: Serologic testing for syphilis and public health follow-up as needed. MEASUREMENTS AND MAIN RESULTS: Twenty-one non-STD patients (2%) had untreated early syphilis, and 22 (2%) had positive serology but were lost to follow-up. Among 271 STD patients seen in the ED during the same period, 15 cases (6%) were detected. We estimate that 80 or more additional untreated early syphilis cases would have been identified had all 15- to 44-year-old patients entering the ED been tested. The cost of screening was $251 per case detected. CONCLUSION: Patients not suspected of having any STD account for most early syphilis cases among all ED patients. Screening and on-site treatment for syphilis should be offered to all young adults seeking medical attention in the ED during syphilis outbreaks.

Job exposure matrices in industry.

Job exposure matrices (JEM) are designed to link information on occupation with information on exposure to specific workplace hazards. In spite of some limitations, JEM are particularly useful in large retrospective epidemiological studies. The development of JEM designed for a company or an industrial sector have a more specific field of application than JEM used in population-based studies, and can therefore be based on a more detailed classification of occupations and better exposure information. This article reviews on-going research related to the design of JEM in support of industry-based studies. The review covers design aspects related to the structure of JEM, and specially the four main axes that may produce differences on exposure characteristics: agent (exposure), job, time and place. Attention has also been given to the different sources of information on exposure, the characteristics of the exposure and the validation of exposure estimates.

Design of a job exposure matrix on electric and magnetic fields: selection of an efficient job classification for workers in thermoelectric power production plants.

Occupational exposure to 50 Hz electric and magnetic fields (EMF) was measured among 184 workers in thermoelectric power production plants using an individual portable dosimeter. A job exposure matrix (JEM) is elaborated from these data to be used in an epidemiological study on the potential carcinogenic effects of EMF. To reduce the range of exposure misclassification in the study, groups of workers with high exposure homogeneity must be identified. Classifying the workers by type of plant yielded homogeneous exposure groups, especially for workers in non-nuclear power plants. Workers in nuclear plants had higher mean exposure to magnetic fields, but the homogeneity of exposure was smaller. The exposure also differed between occupations, but the occupational title did not produce a uniform increase of exposure homogeneity within subgroups. It is concluded that the place of work is the most important determinant of exposure to magnetic fields for workers in thermoelectric power plants to be included in the JEM.
