In Vitro activity of 6 antimicrobials against propionibacteria isolates from untreated acne papulopustulosa.

In the present study, MIC values of 6 antibiotics were determined for 70 Propionibacterium acnes and 13 P.granulosum strains from 71 untreated acne patients using the agar dilution method. The interpretation of in vitro results is difficult because there are only poor data about the concentrations of antibiotics achievable in the sebaceous gland infundibulum. Based on breakpoint concentrations according to DIN 58,944, no resistance was found against chlortetracycline, doxycycline, minocycline, and chloramphenicol. In contrast, 11% of the P.acnes and 31% of the P.granulosum strains were not susceptible in vitro to erythromycin at a breakpoint concentration of 4 micrograms/ml. The present study shows that these strains are not suppressed even at higher erythromycin concentrations up to 256 micrograms/ml. Data from the literature reporting ineffective treatment of patients carrying erythromycin-resistant P.acnes strains may be explained by our study. Concerning clindamycin, resistance was found in only 6.8% of P.acnes and in 0% of the P.granulosum strains. Since sensitivity was seen in 100% of the strains already at a concentration of 16 micrograms/ml, a therapeutic effect may be possible.

A large terrestrial carbon sink in north america implied by atmospheric and oceanic carbon dioxide data and models

Atmospheric carbon dioxide increased at a rate of 2.8 petagrams of carbon per year (Pg C year-1) during 1988 to 1992 (1 Pg = 10(15) grams). Given estimates of fossil carbon dioxide emissions, and net oceanic uptake, this implies a global terrestrial uptake of 1.0 to 2. 2 Pg C year-1. The spatial distribution of the terrestrial carbon dioxide uptake is estimated by means of the observed spatial patterns of the greatly increased atmospheric carbon dioxide data set available from 1988 onward, together with two atmospheric transport models, two estimates of the sea-air flux, and an estimate of the spatial distribution of fossil carbon dioxide emissions. North America is the best constrained continent, with a mean uptake of 1.7 +/- 0.5 Pg C year-1, mostly south of 51 degrees north. Eurasia-North Africa is relatively weakly constrained, with a mean uptake of 0.1 +/- 0.6 Pg C year-1. The rest of the world's land surface is poorly constrained, with a mean source of 0.2 +/- 0.9 Pg C year-1.

[Modification of stratum corneum quality by glycerin-containing external ointments].

The effects of glycerol are discussed in detail. Glycerol can lead to a transition of crystalline lipid structures within the horny layer lipids into liquid crystalline states. Especially if applied in oil/water-emulsions it improves, the hydration of the horny layers better than urea. In addition, if facilitates the dissolution of desmosomes within the superior layers of the horny layer enhancing desquamation. Furthermore, by using glycerol, the mechanical properties of the skin can be influenced; long term used leads to increased elasticity. The roughness of the horny layer is reduced, which can be explained by the abrasive and/or the hydrating effect. Glycerol used in o/w-emulsions also protects against the influence of tensides or noxious lipophilic agents. Presumably a flux from the base of the epidermis towards its surface triggers the regeneration of the barrier. Finally, glycerol can lead--similarly to urea--to an improvement in active agent penetration, as has been proven for hexylnicotinate.

Predictive washing test for evaluation of individual eczema risk.

It was the aim of our studies to estimate predictively the individual eczema risk for persons due to repetitive contact with washing-active substances, by a barrier function test on clinically healthy skin over 2 weeks. Within the scope of the study 3 groups with different atopy scores were compared. As washing solutions, 0.1 m SLS and a slightly acid soap-free washing emulsion were used in comparison to tap water. Prior to the 1st washing procedure, on days 3, 5, 8, 10, and 12, the transepidermal water loss, the horny layer moisture, and the skin blood flow were measured as parameters of barrier function, as well as the inflammatory reaction. The results prove that the atopy score has only limited validity as a predictive method for the acceptance of washing-active substances. The repetitive washing test, however, seems to be more adequate for evaluating the individual barrier function as well as the eczema risk. Irritation by a washing procedure may be greatly influenced by choice of the washing solution.

Opposing effects of glycerol on the protective function of the horny layer against irritants and on the penetration of hexyl nicotinate.

BACKGROUND: It is known that glycerol in an oil-in-water emulsion has a protective effect against irritating substances. OBJECTIVE: To answer the question: is the protection effect of glycerol based on a regenerative process? METHODS: Upon irritation by either tape stripping or acetone treatment, we applied glycerol to the skin surface under an occlusive dressing to create transepidermal water movement. As a control we used water under the occlusive dressing on the contralateral forearm. After 5 h we compared the barrier function using biological tests. RESULTS: A significant improvement of the protective barrier function was observed in the glycerol-treated areas, as shown by the alkali resistance and by the irritant effect of dimethyl sulfoxide (DMSO) as well as sodium lauryl sulfate. Surprisingly, at the same time penetration of hexyl nicotinate improved on the glycerol-treated areas. A direct physicochemical protection effect on the surface of the skin was ruled out in additional studies using NaOH and DMSO. CONCLUSIONS: Under the given conditions glycerol leads to a more rapid reconstitution of the protective skin barrier and initiates a regenerative skin protection. In contrast to that, it is acting as a penetration enhancer.

Antibacterial and sebosuppressive efficacy of a combination of chloramphenicol and pale sulfonated shale oil. Multicentre, randomized, vehicle-controlled, double-blind study on 91 acne patients with acne papulopustulosa (Plewig and Kligman's grade II-III).

In a 3-armed, multicentre, randomized, double-blind, vehicle-controlled study involving 91 patients with acne papulopustulosa, Plewig's grade II-III, evidence could be provided of a significant reduction of the propionibacteria as well as a subosuppressive effect (squalene reduction) under a combination of 1% chloramphenicol (CAS 56-75-7) and 0.5% pale sulfonated shale oil versus the alcoholic vehicle (1-2 ml twice daily). Likewise, monotherapy with chloramphenicol resulted in a significant reduction in bacteria compared to the vehicle. The combination therapy was superior to the monotherapy with regard to the sebosuppressive effects. Based on a kinetics test carried out for a total of 2 h, a clinically relevant percutaneous absorption of chloramphenicol was ruled out. The chloramphenicol serum level was between < 5.0 microgram/l to 180 microgram/l (average 25 micogram/l). This is important because with systemic application (peroral, i.v.), the therapeutic chloramphenicol level is > 25 mg/l (25,000 microgram 1). None of the blood count and serum parameters were pathologically changed in a clinically relevant way before and after the therapy. An induction of resistance against chloramphenicol in the propionibacteria could be excluded. No adverse events and side effects occurred. The topical therapy of acne papulopustulosa with chloramphenicol as a monosubstance or in combination with pale sulfonated shale oil represents an effective and safe local antibiotic treatment possibility.

Influence of vitamin E acetate on stratum corneum hydration.

The influence of vitamin E (tocopherol, CAS 10191-41-0) on stratum corneum hydration was tested in O/W and W/O emulsions. Additionally, the O/W emulsion was used in an in vivo/in vitro method to gravimetrically obtain evidence concerning the water-binding capacity of the stratum corneum. In the W/O emulsion, 2.5%, 5%, and 7.5% vitamin E were compared. With both types of emulsions, vitamin E increased the stratum corneum hydration statistically significantly (p = 0.0002). In addition, we could provide evidence of an enhanced water-binding capacity after treatment with vitamin E (p = 0.05). For the hydrating effect of vitamin E. its concentration is of importance. The optimum concentration turned out to be 5%.

[Poppyseed anaphylaxis].

We describe 3 patients with a severe allergic immediate-type reaction to poppy-seeds, diagnosed from clinical findings, skin tests and specific IgE antibodies (CAP). All show a serologic positivity to sesame seed, possibly due to a cross reactivity to similar allergens. From 1978 until 1987 402 food allergy patients seen at the Allergy Unit, Department of Dermatology, University Hospital Zurich, Switzerland, included no case of allergy to poppy-seeds. Because of today's trend to vegetarian food such immediate type reactions can be expected to occur more frequently.

Modeling the effects of anterior chamber fluid viscosities on intraocular pressure following glaucoma filtering surgery.

BACKGROUND: Hypotonia, flat anterior chamber, and choroidal effusion are not infrequent and undesirable consequences of glaucoma filtering surgery. Methods developed so far to prevent or combat these complications have only been moderately successful. Viscous fluids admixed to the aqueous humor are known to influence fluid dynamics and hence intraocular pressure during variable time intervals following surgery. It was the intention of the authors to develop a mathematical model which predicts the influence of sodium hyaluronate determining the intraocular pressure following injection into the anterior chamber. METHODS: The model is based on first-order approximations and derives from the theory of rheology and fluid dynamics in narrow tubes, and is essentially based on Hagen-Poiseuille's law. RESULTS: The predictions of the model (to be published in a paper to follow) were verified in experiments on owl monkeys and showed a high degree of predictive power. CONCLUSIONS: One may conclude that sodium hyaluronate, injected once or repeatedly with fine canulas into the anterior chamber, qualifies as an important tool to maintain normal on near-normal intraocular pressure in the immediate and late postoperative phase following surgery.

Damage to the skin by repetitive washing.

Damage to the skin following the repeated use of 2 different wash solutions was investigated. Stratum corneum capacitative resistance, stratum corneum lipids, transepidermal water loss, skin surface pH, laser Doppler flow and skin reddening were determined. All skin function parameters already showed a marked change after a single wash (e.g., the median of TEWL values increased by more than 0.5 g/m2 h). Repetitive washing for 1 week led to a further deterioration of TEWL and corneometry values (e.g., TEWL increased for about 2.9 g/m2 h after repetitive washing with sodium lauryl sulfate). The rate of skin function regeneration after repetitive washing was unaltered compared to regeneration after a single wash. The surfactants used showed quantitatively differing effects on corneometry, TEWL and laser Doppler flow (e.g., after repetitive washing with Geliderm, the median of TEWL values increased only up to 1.55 g/m2 h). However, damage arising from repetitive washing could not be completely prevented by the selection of a mild surfactant. Skin function regeneration showed no difference with the 2 surfactants used.

[Prepubertal hypertrichosis].

Hypertrichosis must be assessed in the light of the type of hair involved, the pattern of abnormal hair growth, associated abnormalities, age of onset, and family history including ethnic background. Associated diseases and the associated medication must be considered, as drug-induced hair growth is not uncommon. Although disorders of the hair usually have little medical significance and involve no interference with the general physical condition, they may be present in congenital syndromes or be associated with other abnormalities and metabolic disorders. The individual psychological impact of excessive hair growth also has a bearing on the problem with regard to its management. We describe in a 7-year-old girl and an 8-year-old boy a state of prepubertal hypertrichosis, characterized in otherwise normal children by a form of generalized excessive terminal hair growth which does not conform to the pattern of (androgen-mediated) hirsutism. The nosology of this form of excessive hair growth is discussed. The question whether it represents an atavistic trait is controverted.

Recurrent Pseudomonas folliculitis.

Pseudomonas aeruginosa has attracted the attention of the dermatologic literature through repeatedly reported outbreaks of folliculitis occurring in small epidemics related to contaminated whirlpools, hot tubs, and swimming pools. In contrast, sporadic cases without these recreational exposures may present a diagnostic challenge. Two sisters, age 8 and 4 years, had sporadic Pseudomonas folliculitis. It is important to recognize the organism as the cause of this recurrent, papulopustular, follicular eruption on the trunk and gluteal regions, also called nonrecreational Pseudomonas folliculitis, to avoid misdiagnosis and mismanagment. Inadvertent therapy with steroids may result in rapid spread of the lesions. Unless a meticulous investigation of contaminant sources in the environment leads to the detection of the vehicle of infection, the dermatitis tends to recur and become chronic.

Enhancement of the penetration of dithranol and increase of effect of dithranol on the skin by liposomes.

An assessment was made in healthy skin of dithranol erythema caused by a 10-min occlusive application of 0.5% dithranol (CAS 1143-38-0) in liposomal gel (Natipide II) with and without the addition of 3% salicylic acid, and of 0.5% dithranol in vaseline and in a removable standard ointment base, both with 3% salicylic acid. Both liposomal preparations led to a significant potentiation of the dithranol erythema. The results indicate a strong promotion of the penetration of dithranol by the liposomal gel.
