[Distribution of HBs-(Australia) antigen in progressive systemic scleroderma and other connective tissue disorders].

Of 173 patients, mostly suffering from collagen diseases and rheumatoid disease, the HBs antigen (HBsAg) was present in 1.73%. This figure is no higher than that observed among blood donors from the same area (Naples and surroundings). One HBsAg positive patient suffered from chronic active hepatitis and rheumatoid arthritis; another from chronic active hepatitis and a secondary type of reticuloendotheliosis; the third was one of seven patients with psoriatic arthritis. HBsAg was not found in any of 48 patients with other collagen diseases, including 22 patients with Progressive Systemic Sclerosis. These data do not support the hypothesis based on observations of polyarteritis nodosa that HBsAg plays an ethiopathogenetic role in collagen diseases with serious vasculitis.

[Our experience in the transanal excision of tubulovillous adenomas of the rectum].

BACKGROUND: Personal experience on transanal excision of rectal adenomas without affecting sphincteric function is reported. METHODS: From 1985 to 1997, 27 patients suffering from rectal adenomatous polyps underwent surgery; the sites of lesions were within 3 to 10 cm from anal orifice in the whole series; the age of patients ranged from 30 to 81 years. Two different procedures were employed: the Parks' technique and the electroresection by traction flap technique according to Faivre. RESULTS: Any postsurgical complication such as hemorrhage, stenosis or incontinentia occurred; surgical mortality was absent. Histological examination disclosed severe dysplasia as well as in situ carcinoma in 6 patients (22.2%) and malignant polyps in 9 patients (33.3%). Only in a case a palliative excision was performed since the poor general conditions of this patient did not permit a more extended treatment; a local relapse of the tumour associated with liver metastases led the patient to death 22 months after surgery. Three patients were lost to follow-up and 2 patients died because of other causes, 6 and 8 years after surgical excision, respectively. CONCLUSIONS: The conclusions are is drawn that either Park's and Faivre's procedures are useful and safe for the surgical treatment of rectal villous polyps extended up to 8-12 cm from anal orifice, in spite of the presence of malignant foci within their mass. These surgical procedures are simple and relatively poor traumatic; for this reason they are more suitable than other transabdominal or abdomino-perineal approaches for older patients and other at risk-patients. It is underlined that the treated patients require a long-term follow-up aimed at the early diagnosis of possible relapses of adenomatosis.

[Our experience in internal medioposterior sphincterotomy with anoplasty].

BACKGROUND: The technique of the posterior medial internal sphincterotomy according to Arnous is described and advice from personal experience reported. METHODS: From 1981 to 1995 posterior medial internal sphincterotomy with anoplasty was performed in 270 patients (132 males and 138 females) affected with chronic anal fissure, alone (112 cases) or associated with hemorrhoids (80 cases); moreover, the Arnous's operation was performed as well for hemorrhoids with posterior subsidiary packet and internal sphincter hypertonia (52 cases), relapsed hemorrhoids with postoperative anal stenosis (24 cases) and postoperative anal stenosis with painful fissure and residual internal sphincter hypertonia (2 cases). RESULTS: The results have been excellent; the average stay in hospital has been 7 days and the complete recovery occurred after 4-6 weeks without early or late complications. CONCLUSIONS: The importance of anorectal manometry for the preoperative valuation of the sphincteric hypertonia is emphasized: in this manner it is possible to modulate the sphincterotomy avoiding too economic sphincteric sections with next residue hypertonia or, in the contrary, too plentiful sphincteric section with problems of continence. Finally, the internal lateral-left sphincterotomy is mentioned, which is efficacious in the treatment of acute anal fissure. However, the proctological surgeon, on the basis of his experience, will propose the most convenient technique.

[Colonoscopic screening in first-degree relatives of patients with sporadic colorectal cancer].

UNLABELLED: Colonoscopic screening has been recommended in asymptomatic first-degree relatives of patients with colorectal cancer. In fact this population is believed to have an increased risk in developing colorectal neoplasia. The purpose of this study is to report the impact of colonoscopy in a series of completely asymptomatic first-degree relatives of patients operated on for colorectal cancer at our institution. A total of 480 individuals was requested to participate in a screening program based on faecal occult blood testing (FOBT) and colonoscopy in those with positive FOBT. Colonoscopy was also suggested to persons with negative FOBT. After the first 195 examined relatives, FOBT was abandoned because of continuously increasing acceptance of colonoscopy. RESULTS: Two hundred fifty four subjects (52.4%) accepted to participate at the screening program. After the first 195 examined relatives, FOBT was abandoned because of continuously increasing acceptance of colonoscopy. A total of 142 colonoscopies was performed. Colonoscopy was completed in 112 relatives (78.9%). Thirty-three subjects (23.2%) had a positive colonoscopy: one had invasive adenocarcinoma, one had large villous adenoma and 31 had 54 polyps. Twenty-three lesions (40.3%) were located proximal to the splenic flexure. Of the 54 polyps, 45 (83.3%) were less than 1 cm in size. These findings confirm the utility of colonoscopic screening in asymptomatic first-degree relatives of patients with colorectal cancer. However, its feasibility with the current endoscopic facilities remains an unsolved question.

[Neuromuscular and vascular hamartoma of the small intestine. Report of a clinical case and review of the literature].

We present the case of a 50-year old patient, operated on for three adjoining annular strictures in the jejunum with proximal dilatation. He presented with a one-year history of recurrent cramping in the upper abdomen and vomiting, with two episodes of intestinal obstruction. Microscopic examination of the stricture revealed, in the submucosa, disorganised fascicles of smooth muscle derived from the muscularis mucosae, bundles of non-myelinated nerve fibres with scattered abnormal ganglion cells and haemangiomatous vessels. The pathological findings observed were similar to those described as neuromuscular and vascular hamartoma, a rare stricturing condition of the small intestine. Many authors have questioned the hamartomatous nature of this disorder, since identical features may be seen in Crohn's disease, in ischaemic enteritis, in radiation enteritis and in non-steroidal antiinflammatory drug-induced small intestinal strictures. On the basis of a review of the 5 previously described cases and of our own experience, we believe that neuro muscular and vascular hamartoma of the small bowel should be considered as a distinct entity if histological hallmarks of Crohn's disease are absent, in patients with no history of gastrointestinal disease, or of chronic ingestion of non-steroidal antiinflammatory drugs.

[Herpes zoster of the trigeminal nerve: a case report and review of the literature].

Herpes zoster (shingles) is caused when the varicella zoster virus that has remained latent since an earlier varicella infection (chicken-pox) is reactivated. Herpes Zoster is a less common and endemic disease than varicella: factors causing reactivation are still not well known, but it occurs in older and/or immunocompromised individuals. Following reactivation, centrifugal migration of herpes zoster virus (HZV) occurs along sensory nerves to produce a characteristic painful cutaneous or mucocutaneous vesicular eruption that is generally limited to the single affected dermatome. Herpes zoster may affect any sensory ganglia and its cutaneous nerve: the most common sites affected are thoracic dermatomes (56%), followed by cranial nerves (13%) and lumbar (13%), cervical (11%) and sacral nerves (4%). Among cranial nerves, the trigeminal and facial nerves are the most affected due to reactivation of HZV latent in gasserian and geniculated ganglia. The 1st division of the trigeminal nerve is commonly affected, whereas the 2nd and the 3rd are rarely involved. During the prodromal stage, the only presenting symptom may be odontalgia, which may prove to be a diagnostic challenge for the dentist, since many diseases can cause orofacial pain, and the diagnosis must be established before final treatment. A literature review of herpes zoster of the trigeminal nerve is presented and the clinical presentation, differential diagnosis and treatment modalities are underlined. A case report is presented.

[Psychosocial adaptation in adolescents with chronic kidney disease].

BACKGROUND: Chronic disease represents a very critical experience for personal identity. This is far more true in adolescence when corporeal, emotional, relational and cognitive changes are taking place. The aim of the present study is to draw attention to the social and emotional balance and the psychosocial adaptation in a group of nephropathic adolescents and to formulate a suitable assistance program. METHODS: We studied two adolescent groups, one group of nephropathics and one group of healthy individuals, by administering the MMPI (Minnesota Multiphasic Personality Inventory) personality test. RESULTS: We found significant differences between the mean score of the two groups in the clinical scale for depression, hypochondria and hysteria (p < 0.5); in addition, we observed a statistically significant difference in the emotional balance indicators. CONCLUSIONS: Our experience suggests that nephropathic adolescents require global awareness of their psychological problems through the use of innovative assistance methods. A new strategy involving a more modern approach (i.e., educational camps) and specific psychosocial rehabilitation programs should be introduced to improve the quality of life.

[Clinical evaluation of the Paired Filtration Dialysis in children].

PURPOSE: An important challenge for a pediatric nephrologist is to achieve in children the innovations of current adult dialytic technology. METHODS: Five children, mean age 14.1 +/- 1.5 yrs, mean weight 31.5 +/- 5.8 kg, mean dialytic age 32 +/- 20 months, were evaluated after a 6-month treatment with paired filtration dialysis (PFD). We used two capillary membranes: a 0.4 m 2 polysulfone hemofilter and a 0.8 m 2 cuprophan dialyzer. There was a reinfusion system (1540 +/- 150 mL/h) between these two filters. QD 500 mL/min and QB 230 +/- 10 mL/min. Ultrafiltration (UF) 2400 mL/h was planned for the hemofilter. At base-line and after 6 months of treatment, in addition, we evaluated routine clinical parameters, and other parameters such as beta2-microglobulin, dialytic adequacy (Kt/V) and nutritional status (dietetic diary). The data were analyzed using the Student's t-test for paired values. RESULTS: Medium values of small molecules did not demonstrate meaningful variations after 6 months of treatment. The extraction percentage after each session was between 63% phosphorous and 76% for urea, but only 25% for convection. Instantaneous urea clearance after 60 min was 178 +/- 10 mL/min. Dialytic efficiency and nutritional intake were appropriated: Kt/V 1.66 +/- 0.2; PCRn 1.65 +/- 0.2; protein intake 2.2 +/- 0.5 g/kg/die; caloric balance 71.2 +/- 15 Kcal/kg/die; protein balance 0.53 +/- 0.4 g/kg/die; and azotic balance 85.8 +/- 74 mg/kg/die. We confirmed the good depuration capacity from the middle molecules with convective treatments: beta2-microglobulin extraction was 53%. Moreover, in children we found good clinical tolerance to PFD with modest interdialytic symptomatology: headache (8%), hypotension (6%), and cramps and vomiting (<2%). Increased priming of the extracorporeal circuit was not a significant technical problem. The cuprophan membrane has been subsequently replaced with the polysulfone membrane. CONCLUSIONS: We can assert that this technique can also be performed in the pediatric age with similar results as in the adult age: good depuration of the small and averages molecules, good clinical tolerance, and shortening the dialytic sessions.

[ProMACE-MOPP vs. ProMACE-CytaBOM polychemotherapies in the treatment of large-cell and unclassifiable non-Hodgkin's lymphomas].

The results of two distinct and successive clinical pilot studies investigating feasibility of aggressive chemotherapy in large cell and unclassified non-Hodgkin's lymphomas are reported. In the first study (1986-87) the ProMACE-MOPP chemotherapy (P-M), a 2nd generation regimen, was administered to 10 patients, whereas in the second study (1987-88) the ProMACE-CytaBOM schedule (P-C), a 3rd generation regimen, was administered to 13 patients. The clinical and prognostic features of the two groups of patients were quite comparable, the only difference being the different follow-up times (median: 43 vs 26 months). The number of complete remissions, freedom from relapse and overall survival were slightly better in the P-M group, with-out a statistically significant difference, despite the longer follow-up time. Hematological toxicity was higher in the P-M study, while gastrointestinal (mucositis, hepatic transitory damage) and neurological toxicity (peripheral neuritis) were somewhat lower. In conclusion, the P-M regimen is worth considering for the treatment of high grade malignant lymphomas, although it was not recently designed and is not widely used at present.

[The use of misoprostol in prevention of gastric disease from nonsteroidal anti-inflammatory drugs in oral-maxillofacial surgery].

The results of a clinical experiment with a new synthetic prostaglandin for the prevention of gastric lesions induced by NSAID: the misoprostol have reported. Twenty patients were treated with this drug and the results were compared to those obtained with the use of ranitidine in the same number of patients. The clinical experiment showed an excellent misoprostol-induced gastric protection which compared to ranitidine presents the advantage of a multiple and pathogenetically oriented action mechanism.

Myelodysplastic syndromes: a multiparametric study of prognostic factors and a proposed scoring system.

In 72 consecutive patients with previously untreated myelodysplastic syndromes (MDS) having a median follow-up of 44.9 (range: 6-198) months, a multiple regression analysis was conducted of the prognosis significance of 26 clinical and laboratory parameters, including bone marrow (BM) biopsy characteristics. Parameters which had independent prognostic meaning served to construct a scoring system for survival prediction. Only 7 parameters were significant: hemoglobin, BM cellularity, BM blast percentage, abnormal location of immature precursors (ALIP), fibrosis, dysmegakaryopoiesis and the erythro/myeloid ratio. They enabled us to predict 42% of all MDS patient survival (p less than 0.03) and 84% (p less than 0.02) of survival of patients who lived over 20 months. Based on the value of these parameters in individual cases, the patient population had a score ranging from 0 to 13, with a median value of 5. Median survival of patients with a score less than or equal to 5 was 117, while that of patients with a score greater than 6 was 33 mos. This scoring system, which has been draw from a wide panel of clinical and laboratory parameters, will be verified on prospective studies.

Acute pancreatitis as the initial manifestation of systemic lupus erythematosus.

A 20-year-old female with three laboratory ARA criteria pro SLE (LE cells, FANA, and positive Coombs test with hemolytic anemia), not under steroid therapy, developed polyarthritis, erythematous rash and acute pancreatitis simultaneously. The latter regressed with high doses of 6-methylprednisolone. Twenty-five months after remission of pancreatitis, no new clinical manifestations (of SLE) have appeared. In another 74 cases of SLE with an average follow-up of 3 years and 8 months there were no other cases of pancreatitis.
