Keziot--a new endemic site of cutaneous leishmaniasis in Israel. An epidemiological and clinical study of a non-immune population entering an endemic area.

In this study we describe a new endemic focus of cutaneous leishmaniasis at Keziot in southern Israel, and the pattern of the disease among a group of soldiers newly based in the area. Interviews and physical examinations were conducted on 296 soldiers, and 247 were subjected to a leishmanin skin test. Positive tests were recorded in 18% of the soldiers; in 16% of them Keziot was identified as the area where the infection was acquired. Positive skin tests were found among 93% and 41% of soldiers with typical cutaneous leishmanial lesions and atypical lesions, respectively. The relatively high proportion of atypical cases with positive leishmanin tests emphasizes the difficulty of distinguishing clinically, in an endemic area, between cutaneous leishmaniasis and other scars and secondarily infected wounds. In a concurrent study Leishmania major was isolated from rodents of the genus Meriones and from Phlebotomus papatasii sandflies in the Keziot area. Keziot should be considered an endemic area for cutaneous leishmaniasis, due to the high morbidity rate and demonstration of all the components of the disease cycle, namely the parasite, the vector and the reservoir.

Estradiol binding in ovariectomy-responsive and -nonresponsive rat mammary carcinoma.

MTW9, a transplantable mammary tumor in Wistar-Furth rats, is dependent upon high levels of serum prolactin for growth. The mammary tumor can be maintained by coimplantation with a pituitary mammosomatotropic tumor (MTW9-MtT) or by chronic perphenazine administration (MTW9-P). MTW9-MtT does not regress after ovariectomy (OVEX) alone, while, MTW9-P regresses rapidly whether or not rats are maintained on perphenazine. Cytosol estradiol binding was determined in MTW9-MtT, 47.6 +/- 3.6 (N = 9), Kd 9.9 +/- 2.0 X 10(-11) M; MTW9-P, 195.5 +/- 9.1 (n = 16), Kd 27.6 +/- 4.7 X 10(-11) M; MTW9-P after perphenazine was withdrawn, 86.0 +/- 6.0 (n = 6), Kd 9.9 +/- 1.5 X 10(-11) M. Eight S and 4 S peaks were found in cytosols from both tumors by sucrose gradient centrifugation; the peaks differ qualitatively and quantitatively. The possibility that differences in binding levels result from sites filled with endogenous estradiol was critically examined and rejected. Withdrawal of perphenazine results in reduction of estradiol binding but retention of response to OVEX. Hence, response of MTW9-P to OVEX cannot be explained by the 3- to 4-fold increase in estradiol binding over MTW9-MtT. The relationship between estradiol binding and response to OVEX is under study using both mammary tumor models.

Differentiation of erythroleukemic cells in vitro: irreversible induction by dimethyl sulfoxide (DMSO).

The inclusion of DMSO in the media of suspension cultures of Friend erythroleukemia cells results in the erythroid differentiation of these cells. The studies reported here were directed towards answering two questions. (1) How long an exposure to DMSO is necessary to induce the differentiation of these cells; and (2) What is the fate of the differentiating cells when DMSO is removed from the medium. Exposure to DMSO for less than 24 hours failed to produce any detectable evidence of erythroid differentiation. On the other hand, culture in the presence of DMSO for 24 hours followed by culture in DMSO-free medium for four additional days produced a small but detectable increment in the proportion of benzidine positive cells in the culture. Once the differentiation of an individual cell was initiated, the process continued after removal of DMSO from the medium. The cell became progressively more differentiated as evidenced by increases in the intensity of benzidine staining as well as the rate of heme synthesis and heme content. However, when cells which had been induced to differentiate by DMSO were cultured in DMSO-free medium for more than 3--4 days, they became vacuolated and apparently died. This latter phenomenon, as well as the more rapid proliferation of the undifferentiated cells in the culture, accounts for the observation that when new cultures are established from cultures which have been grown in the presence of DMSO for several days, the culture which results ultimately contains only differentiated cells.

Anaerobic bacteremia: a retrospective four-year analysis in general medicine and cancer patients.

Anaerobic bacteremia was studied in 32 medical patients (mean age 72 years) in a four-year retrospective analysis. Malignancy was the most common underlying disease and probable portal of entry. The gastrointestinal tract was affected most often, followed by the respiratory and urinary tracts. Bacteremia occurred either following invasive (non surgical) procedures or spontaneously. The clinical course ranged from asymptomatic bacteremia, to mild febrile illness, to sepsis and septic shock (two, 12, 16 and two patients, respectively). The case fatality rate was 25%. The causative organisms were Clostridium and Bacteroides species. All organisms isolated were susceptible to chloramphenicol. Early diagnosis and prompt treatment may reduce mortality in cases of anaerobic sepsis.

Hemophilus influenzae biotype III cellulitis in an adult.

The case of a patient with systemic lupus erythematosus presenting with severe leg cellulitis caused by Hemophilus influenzae non-B biotype III is reported. Skin infections caused by H. influenzae in general, and of the extremities in particular, seem to be rare in adults. This is the first reported case of cellulitis caused by H. influenzae biotype III. The infection was treated successfully with antibiotics. This case highlights the importance of blood cultures and prompt antimicrobial treatment in febrile adults with cellulitis, especially immunocompromised patients.

Postural epigastric pain: a unique symptom of primary cytomegalovirus gastritis?

Cytomegalovirus (CMV) gastritis has been reported in transplant patients. Symptoms are considered nonspecific, and gastroscopy with biopsy is usually performed to establish the diagnosis. Three patients are described here 1 to 3 months after solid organ transplantation, with primary CMV gastritis, confirmed by gastroscopy, histopathologic examination and cultures. The clinical presentation in all three cases was sharp epigastric pain that decreased in a supine position, increased while sitting and further increased when standing or walking. The epigastric pain completely resolved after treatment with ganciclovir. To the best of our knowledge, such postural epigastric pain has not been described as a specific symptom in any other clinical entity and may be a unique sign of primary CMV gastritis.

Pseudomonas aeruginosa bacteremia: an analysis of 123 episodes, with particular emphasis on the effect of antibiotic therapy.

OBJECTIVES: To review current experience with Pseudomonas aeruginosa bacteremia and compare outcome of patients treated with single-drug, versus combination therapy. METHODS: The charts of all patients with P. aeruginosa bacteremia between 1990 and 1992 were reviewed, and pertinent demographic, clinical, and bacteriologic data were retrieved. In addition, similar data were collected from a series of patients with P. aeruginosa bacteremia from the literature of the past 20 years. RESULTS: One hundred and twenty-three episodes of P. aeruginosa bacteremia in 121 patients were identified. Most patients were older than 70 years, had at least one underlying condition, and had acquired the infection in the hospital. Attributable mortality was 34%. After exclusion for early mortality and inappropriate therapy, 57 patients remained eligible for comparison of outcome according to therapy protocol. Mortality from infection was equal between the group of 42 patients who received monotherapy and the 15 patients who received combination therapy (14% and 13%, respectively). The literature review revealed eight articles describing 21 to 410 episodes of Pseudomonas bacteremia. The clinical characteristics of these series did not differ significantly from those of the present series. CONCLUSIONS: Incidence, epidemiology, clinical characteristics, and outcome of pseudomonas sepsis did not change significantly over the past 2 decades. Appropriate monotherapy was as effective as combination drug therapy for individuals with pseudomonas bacteremia surviving the first 2 days of infection.

Helminths of birds and mammals of Israel. VII. - Pneumospirura rodentium n. sp. (Pneumospiruridae - Thelazioidea).

Pneumospirura rodentium a lung parasite of Gerbillus dasyurus and Meriones crassus is described. P. hainanensis Wu and Hu, 1938 and Metathelazia servalis Chabaud and Biocca, 1950, have cephalic structures resembling those of P. rodentium. But the three species can be differentiated by the size of spicules, their localization, hosts and geographical distribution.
