Office varicose vein surgery under local anesthesia.

Varicose veins of any size (except telangiectasia) and in any site of the lower limb (except the saphenofemoral junction) can be avulsed through multiple 0.5-mm (or larger) incisions under local infiltration anesthesia (Müller's technique). When necessary, high ligation, division, and stripping of the proximal long saphenous vein can also be performed under local anesthesia. It is thus possible to treat even the most complex varicose cases with outpatient office-based surgery.

[Endoscopic assessment of hemorrhage from the gastrointestinal tract].

The causes of gastrointestinal bleedings was assessed by fiber gastroscopy, rectoromanoscopy and fiber colonoscopy. The most frequent causes of bleedings from the upper gastrointestinal tract are gastric and duodenal ulcers, erosive hemorrhagic gastritis, gastric cancer, liver cirrhosis with bleeding from varicose veins, polyps, diverticuli, Mallory-Weiss syndrome, etc. The most frequent causes of bleedings from the lower gastrointestinal tract are hemorrhoids, anal fissures, colonic polyps, chronic ulcerohemorrhagic colitis, rectal carcinoma, etc. The diagnostic importance of urgent endoscopic examinations is pointed out.

Postsclerotherapy hyperpigmentations: a one-year follow-up.

In a prospective study of 100 sequential varicose patients treated with sclerotherapy, 15 had some light brown pigmentations at the end of the treatment. One year later, 1 patient still had some linear pigmentations, while 4 other patients had a single, macular, barely visible pigmentation of no cosmetic significance.

[The incidence of duodenogastric reflux and its relation to stomach and duodenal diseases].

The data available in the literature on the incidence of duodenogastric reflux and its relation with diseases of the stomach and duodenum are controversial. A study was performed based on the results of 4256 primary endoscopies of the stomach and duodenum: 3673 in nonoperated patients and 583 in patients having undergone vagotomy and resection. For duodenogastric reflux one judged from the yellow coloration of the gastric juice and regurgitation of bile at the moment of examination. Reflux had 428 patients (10.05 per cent)--330 in the nonoperated group (8.98 per cent) and 98 in the operated group (16.81 per cent). Inference is drawn that duodenogastric reflux does not coincide in incidence with reflux gastritis and that causal relation might be searched between gastritis, reflux esophagitis and duodenitis, on the one hand, and duodenogastric reflux, on the other.

[Comparative fiber colonoscopic and x-ray studies in diseases of the large intestine].

For a two year period 79 patients were examined by fiber colonoscopy and X-ray examination of the colon. The purpose of the study is to find out the sensitivity, specificity and accuracy of the methods for the diagnosis of the most frequent diseases of the colon. The results allow the conclusion that fiber colonoscopy is the method of choice in the diagnosis of the neoplasms and the inflammatory diseases of the colon. When all segments of the colon cannot be examined by fiber colonoscopy a double-contrast barium enema is advised. Fiber colonoscopy should be performed to all patients with clinical data suspicious for a colonic disease and a negative X-ray examination of the colon.

[Peritonitis in patients treated by continuous ambulatory peritoneal dialysis].

With the present study the authors set themselves the task to compare the number of peritonitis episodes in patients treated with two types of systems: "Sorin-Biomedica" and "Travenol-(UV-XD)", in which disinfection of the connecting devices is achieved accordingly with chemical agents and with ultraviolet irradiation. Eleven patients have been observed from August 1984 through February 1989. The total duration of treatment was 156 months. Twenty one peritonitis episodes were observed--15 with "Sorin-Biomedica" system and 6 with "Travenol-(UV-XD)" system--an average of one episode in 4 1/2 months with the former system and one episode in 14.7 months with the latter. It is pointed out in conclusion that the "Travenol-(UV-XD)" system with ultraviolet disinfection has significantly reduced the incidence of peritonitis at the dialysis center where the authors work.

[Effect of autonomic pathology in uremia on the blood pressure during hemodialysis].

The state of the vegetative nervous system was studied by the deep respiration test, Valsalva test and the orthostatic test in 12 patients with terminal chronic renal failure and hypotension in the course of hemodialysis, 12 patients with stable hemodynamics in the course of hemodialysis and 12 healthy controls. By the deep respiration test the mean change of the cardiac rate in the patients with hypotension is significantly lower than that in the healthy controls (p less than 0.001) and that of the normotonic patients on hemodialysis (p less than 0.01). In the dialyzed normotonic patients the change of the cardiac rate is significantly lower than that in the healthy controls (p less than 0.02). Similar results are achieved with the Valsalva test. The impairment of the vegetative nervous system is considerably better expressed in the hypotensive patients. The proposed tests are noninvasive, easy to perform and give reliable information for the affection of the vegetative nervous system.

Postpartum pneumopericardium.

Postpartum pneumopericardium has been previously reported in only 4 patients, but it may occur more frequently than is currently recognized. Two patients are presented with pneumopericardium and pneumomediastinum with symptoms appearing on the first and second postpartum days. The second stage of labor was prolonged in both patients and associated with strenuous maternal effort. Both patients were primiparous and developed dyspnea, pleuritic chest pain, and had Hamman sign. Chest x-rays established the diagnosis in both patients. Echocardiography was nondiagnostic in one patient and mimicked pericardial effusion in the other. Although fatal tension pneumopericardium has been reported in association with other causes, the course in these 2 patients and in the other 4 reported patients has been benign. Symptoms resolved spontaneously in these 2 patients by 72 hours after their appearance. A postulated mechanism for postpartum pneumopericardium is presented.

[Experimental investigation of the preservation of kidneys damaged during warm ischemia].

The author presented his observations of a small group of rabbits to which he administered small doses of dopamine under conditions of warm ischemia (the time between the clamping of the renal artery till the beginning of hypothermic extracorporeal circulation). Another group of rabbits did not receive dopamine. Those rabbits that were given dopamine lived longer and were more resistant to uremia and toxemia. The effect of dopamine lived longer and were more resistant to uremia and toxemia. The effect of dopamine is thought to be due to its vasodilator action on the kidney during periods of hyoxia ad hypotension. The author explained that the kidneys withstood three hours of warm ischemia when treated with dopamine. p.s.
