Cardiology.

Emergency cardiac problems are a frequent and significant occurrence in the daily life of the emergency physician. The first part of this article discusses some of the pearls and pitfalls of caring for the cardiac patient ranging from treating wide-complex tachycardia and troubleshooting pacemaker malfunction to diagnosing acute myocardial infarction in the setting of bundle branch blocks. The second part of this article updates the reader on several of the newer technologies and treatments, such as transesophageal echocardiography and intravenous amiodarone, now in use in the emergency department setting.

Hemophilia.

Hemophilia and von Willebrand's disease are the most common hereditary bleeding disorders. A variety of treatment modalities of these disorders are discussed. Common organ system bleeding and its treatment and disposition are outlined. Finally, potential complications of both the disease and treatment are reviewed.

Dual AV nodal paths leading to AV nodal reentrant tachycardia.

A 27-year-old white female with a history of paroxysmal supraventricular tachycardia presented to the emergency department complaining of intermittent palpitations. Although no tachydysrhythmia was present, she was noted to have two distinct PR intervals during normal sinus rhythm while in the emergency department. The patient was referred for electrophysiologic study. This study demonstrated dual AV nodal paths, and AV nodal reentrant tachycardia was induced and terminated. She was placed on flecainide for outpatient management of her dysrhythmia. Dual AV nodal pathways leading to AV nodal reentrant tachycardia is discussed.
