Immunocytochemical localization of progesterone receptors in endocrine cells of the human pancreas.

Progesterone receptors (PgR) have been immunocytochemically localized in the nuclei of several (40% to 75%) endocrine cells of the human pancreas and in a more variable number of neoplastic cells of 7 of 18 endocrine pancreatic tumors. Conversely the exocrine epithelial cells of the pancreas did not exhibit any PgR immunoreactivity in normal as well as in different pathologic conditions, including pancreatic adenocarcinomas. Estrogen receptors were not detected in any of the pancreatic samples investigated. Double immunocytochemical experiments have documented that PgR immunoreactivity in normal Langerhans islets is a consistent feature of most (75%) glucagon-producing A cells, of approximately 5% to 20% of insulin-producing B cells, and of a variable percentage of pancreatic polypeptide (PP)-producing cells, ranging from 5% to 70%. These figures were not affected by the sex, age, or underlying disease of the patients. The reported findings corroborate previous clinical and experimental evidence indicating that sex steroid hormones may have some regulatory effects on the functional activity of the endocrine pancreas.

Coexpression of cytokeratins and vimentin in normal and diseased thyroid glands. Lack of diagnostic utility of vimentin immunostaining.

We documented the coexpression of cytokeratins and vimentin in epithelial cells of the thyroid gland in 100 samples examined from 65 patients. These included normal, inflammatory, and neoplastic tissues that were routinely fixed in formalin and embedded in paraffin. The number of epithelial cells coexpressing the two intermediate filament proteins as well as the subcellular compartmentalization of vimentin immunoreactivity did not correlate with the various conditions of the thyroid gland. Therefore, we conclude that the immunolocalization of vimentin does not represent a useful adjunct tool for the histopathological diagnosis of thyroid diseases.

In situ demonstration of tissue proliferative activity using anti-bromo-deoxyuridine monoclonal antibody.

Immunohistochemical staining with anti-bromo-deoxyuridine (BrdU) monoclonal antibody was performed on a variety of human tissues following in vitro incubation with BrdU. The effect of different fixatives and DNA denaturation techniques on the reactivity with anti-BrdU was investigated. Optimal preservation of the antigenicity of BrdU incorporated into the DNA of proliferating cells was seen in tissues fixed in Bouin's fluid, while samples which had been fixed with cross-linking reagents, such as formalin, were usually unreactive. Positivity for BrdU was restored in formalin fixed tissues after digestion with pepsin, but this was usually associated with loss of morphological details. Acid and thermal DNA denaturation techniques gave similar results. It is concluded that Bouin fixation followed by acid or thermal denaturation of DNA is the method of choice for the in situ detection of cells in S-phase using anti-BrdU monoclonal antibody.

Chronic encapsulated intracerebral haematoma: pathogenetic and diagnostic considerations.

Two cases of chronic encapsulated intracerebral haematoma are reported. The patients presented with progressive neurological deficits. Computed tomography scan showed a roundish, intracerebral lesion, that revealed ring blush after contrast infusion, with mass effect. At operation a thick, fibrous, brownish capsule, containing clots in different states of formation, was removed. The hypothesis of capsule formation due to an exuberant proliferation of arachnoidal fibroblasts, is discussed. Differentiation between encapsulated intracerebral haematoma and other chronic intracerebral blood collections is considered mandatory for appropriate treatment.

[Testicular biopsy in cryptorchism: prognostic parameters].

470 testicular biopsies performed in prepuberal cryptorchides has been reviewed in order to assess the relationship, if any, between the morphological parameters and the prognosis of future fertility. The main parameter is the presence and the number of germinal cells, which are progressively decreasing in the following groups of patients: descended testis in monocryptorchid patients, monolateral undescended testis, bilateral undescended testis. The results observed are similar to those reported by various Authors. The age of the patients biopsied was irrelevant with respect to the results of the predictability of fertility. Other parameters than germinal cells are of no practical interest.

Coexpression of cytokeratins and vimentin in common epithelial tumours of the ovary: an immunocytochemical study of eighty-three cases.

An immunocytochemical investigation has been performed on 83 common epithelial tumours of the ovary, to ascertain their capability of expressing vimentin in addition to cytokeratins. Our results demonstrate that vimentin coexpression is related to the tumour histotype and -to a lesser extent- to the degree of differentiation of malignant variants. Indeed, most serous tumours (80%), some endometrioid adenocarcinomas, and all the clear cell carcinomas investigated exhibited a variable number of neoplastic cells co-synthesizing the two distinct intermediate filament (IF) proteins, whereas only one of 29 mucinous tumours and none of the Brenner tumours displayed vimentin-immunoreactive cells. Moreover, in serous and endometrioid carcinomas, the expression of vimentin was related to the degree of tumour differentiation, being consistently identifiable in the better differentiated cases. The immunocytochemical findings of a parallel investigation on IF expression in the ovarian coelomic epithelium and in the müllerian-derived epithelia of the female genital tract allowed us to ascertain that ovarian epithelial tumours (with the possible exception of poorly differentiated carcinomas) maintain the pattern of IF expression typical of the normal epithelia. This investigation emphasizes the usefulness of IF typing as a tool for the more precise characterization of the origin and differentiation of human neoplasms.

A clinicopathological study of seven cases of primary high-grade malignant non-Hodgkin's lymphoma of the central nervous system.

Seven cases of primary non-Hodgkin's lymphomas of the central nervous system are described. Six cases were diagnosed after pathologic examination of surgical material; in one case, a patient with acquired immunodeficiency syndrome, the diagnosis was made at autopsy. The mean age of the patients was 52 years. The lesions were supratentorial in all cases, and unifocal in 6: the autopsy case had multicentric lesions. The cytologic examination of the cerebrospinal fluid was performed in 3 cases and was negative. The most common histologic type was immunoblastic lymphoma. The mean postoperative survival time was 12 months; in 2 cases, surgery combined with radiotherapy prolonged the survival for more than 2 years. Leptomeningeal involvement was considered to indicate a poor prognosis.

[Lipid cell carcinoma of the breast].

A review of cases treated in 1978-82 revealed 1 case of lipid cell carcinoma of the breast out of 310 mammary carcinomas. Given the rarity of this histological type and the generally poorer than average prognosis, it seems likely that it should be considered separately from the other cases.

Bronchogenic carcinoma: survival after surgical treatment according to stage, histologic type and immunomorphologic changes in regional lymph nodes.

The survival rates of 90 patients who underwent operation for bronchogenic carcinoma were analyzed statistically according to sex, age, tumor stage and histologic type, types of surgical procedures (lobectomy or pneumonectomy), and to immunomorphologic parameters of immunologic activity in peribronchial and hilar lymph nodes. The Stage I group survived significantly longer than did the Stage II and III groups, the lobectomized patients survived significantly longer than the pneumonectomized patients. The absence or presence of lymph node metastases was one of the major determinants of survival. The mean values (percent of total node cut surface) of lymph node sinus histiocytosis and of paracortical area could be correlated directly to survival in each of the histologic tumor-type groups, while the development of follicular cortex and germinal centers correlated inversely with survival. Increased survival might be associated with changes concomitant with immune reactivity in lymph node T-cell areas and with the sinus histiocytosis pattern, the latter representing probably a tumor--host reaction of the delayed hypersensitivity type. By contrast, increased activity of lymph node B-cell areas tended to correlate with poor prognosis of lung cancer patients. Moreover, patterns showing germinal center hyperplasia were statistically associated with lymph node neoplastic invasion, while the pattern with sinus histiocytosis and expanded paracortical areas was statistically associated with tumor-free lymph nodes.

Ultrastructure of the tubular wall of the prepuberal cryptorchid human testis.

The tubular wall in the human prepuberal testis of cryptorchid patients has been examined by ultrastructural and histochemical analysis. When compared with the normolocated testes, a disorganization of the tubular wall due to cryptorchidism is apparent. Alterations regarding single elements of the tubular wall are described and a positive correlation seems to exist between the age and the amount of the damage. A possible relation between morphological changes and hormone therapy is discussed.

[Primary carcinoid of the testis. Clinical case].

A very rare case of carcinoid of the testicle combined with teratoma is illustrated. In fact the literature of the last twenty years reports only eleven cases of carcinoid of the testicle, of which only four combined with teratoma. It is stressed that it is possible that these neoplasias arise on already existing teratomas which may be very small and hence not always evidenced.

[Glomus tumors in unusual sites].

Two cases of glomus tumour in unusual sites are described. Their indistinct symptomatology, site and histological polymorphism are made the basis of a general discussion of the histogenesis and possible ongoing pictures of glomus tumours, the classification of which is also felt to be unsatisfactory.
