[Papanicolaou test in pregnancy].

OBJECTIVE: To investigate the necessity, safety and acceptability of routine Papanicolaou (Pap) smear in pregnant women and cytological characteristics of the smears during pregnancy. METHODS: Pap smear were performed in 954 pregnant women, 1,053 normal controls and 1,908 gynecological out-patients. The cytological preparations were analyzed by using PAPNET method and following Bethesda (TBS-NCI) diagnostic criteria. Quality control was carried out by following TBS-NCI standard. RESULTS: Abnormal epithelial change was found in 89 women (89/954, 9.3%) of pregnant group, which was quite similar to normal controls group and gynecological group. Occurrence rate of cervical squamous intraepithelial lesion (SIL) was the lowest in pregnant group (0.94%, 9 in 954 cases), comparing with control group (2.75%) and gynecological group (6.23%). However, pregnant group had the highest infection rate (89.9%), as compared to gynecological group (64.5%), and normal controls group (69.1%) (P < 0.01). CONCLUSIONS: Cervical Pap smear test is necessary, safety and acceptable for routine prenatal tests. Any pregnant women with infections must be treated as nonpregnant one.

[Clinical analysis of 12 cases with venous thromboembolism during pregnancy and puerperium].

OBJECTIVE: To study the incidence, cause and clinical manifestation of venous thromboembolism during pregnancy and puerperium, and its diagnosis and treatment. METHODS: 12 cases of venous thromboembolism admitted in our hospital from 1984-1997 were analysed retrospectively. The plasma protein C, protein S and antithrombin III activities were measured in 4 of the cases and activated protein C resistance (APC-R) were assayed by activated partial thrombinplastin time (APIT) in the presence and absence of APC (APC-APIT) and FV Leiden gene mutation were analysed by PCR restriction fragment length polymorphisms (RFLP) methods as well. RESULTS: Four cases occurred before delivery and 8 postpartum. Two cases complicated by pulmonary thromboembolism, and of them 1 died. APC-R(+), reduced activity of antithrombin-III and protein S each were found in 3 separate cases. No FV Leiden gene mutation was found in the 4 cases. CONCLUSIONS: The formation of venous thromboembolism during pregnancy and puerperium is highly associated with the deficiency of anticoagulant proteins. Anticoagulation is recommended in the high risk women of thromboembolism.

Prevention of maternal-fetal blood group incompatibility with traditional Chinese herbal medicine.

OBJECTIVE: To evaluate the effect of traditional Chinese herbal medicine on the prevention of hemolytic disease caused by maternal-fetal blood group incompatibility. METHODS: A total of 126 Chinese patients with maternal-fetal blood group incompatibility were studied, including 105 cases of ABO type and 21 cases of Rh type incompatibility. The traditional Chinese herbal medicine was used for preventative treatment in 79 cases, and 47 cases served as controls. The prescription consisted of Herba leonuri 500 g, white peony root 180 g, Banksia rose 12 g, root of Chinese angelica 150 g and Rhizomal ligustica 150 g. RESULTS: Preventative treatment with traditional Chinese herbal medicine significantly reduced the perinatal mortality rate in cases of Rh type incompatibility, from 50.0% in the control group to 7.7% in the treatment group (P < 0.05). There was no death in the cases of ABO incompatibility. The traditional Chinese herbal medicine also decreased the degree of hemolysis. The percentage of severe cases in the control group was 29.8%, while it was 15.2% in the treatment group (P = 0.05). The duration of treatment was closely related to the outcomes. In patients who used traditional Chinese herbal medicine for more than 10 weeks, the incidence of severe hemolysis was 8.2%, compared with 38.9% in those treated for less than 10 weeks (P < 0.05). The change of serum immune antibody (A and/or B) titers during the treatment was a very important predictive factor for the outcome of the pregnancy. CONCLUSION: Traditional Chinese herbal medicine is effective not only for ABO type but also for Rh type maternal-fetal blood group incompatibility, with no side effects.

[Clinical observation of labor of induction with nutritional food].

386 pregnant women in 3rd trimester having indications of induction labor were induced with nutritional food. The success rate was 77.92% (301/386) and correlated with the cervical Bishop score. Furthermore the success rate increased significantly in term with premature rupture of the membrane (90.54%). This method, which was found to be safe and effective can significantly shorten the first and total stage of labor.

Imaging of regional spread of breast cancer by internal mammary lymphoscintigraphy, CT, and MRI.

Forty women with breast cancer underwent imaging by internal mammary lymphoscintigraphy (IMLS), which was correlated with the results of CT and MRI of the chest. IMLS was performed and interpreted using the previously described methods of Ege. It identified 22 instances of ipsilateral internal mammary nodal involvement, none of which corresponded to cases of abnormally enlarged (diameter greater than 1.0 cm) internal mammary nodes on CT and/or MRI. Positive IMLS was associated with axillary nodal metastases in 15 out of 22 instances. The authors conclude that IMLS provides information on regional nodal spread of breast cancer that is not available with either CT/MRI imaging or axillary biopsy.
