Intermittent catheterisation with hydrophilic-coated catheters (SpeediCath) reduces the risk of clinical urinary tract infection in spinal cord injured patients: a prospective randomised parallel comparative trial.

OBJECTIVES: To compare the performance of SpeediCath hydrophilic-coated catheters versus uncoated polyvinyl chloride (PVC) catheters, in traumatic spinal cord injured patients presenting with functional neurogenic bladder-sphincter disorders. METHODS: A 1-year, prospective, open, parallel, comparative, randomised, multi centre study included 123 male patients, > or =16 y and injured within the last 6 months. Primary endpoints were occurrence of symptomatic urinary tract infection (UTI) and hematuria. Secondary endpoints were development of urethral strictures and convenience of use. The main hypothesis was that coated catheters cause fewer complications in terms of symptomatic UTIs and hematuria. RESULTS: 57 out of 123 patients completed the 12-month study. Fewer patients using the SpeediCath hydrophilic-coated catheter (64%) experienced 1 or more UTIs compared to the uncoated PVC catheter group (82%) (p = 0.02). Thus, twice as many patients in the SpeediCath group were free of UTI. There was no significant difference in the number of patients experiencing bleeding episodes (38/55 SpeediCath; 32/59 PVC) and no overall difference in the occurrence of hematuria, leukocyturia and bacteriuria. CONCLUSIONS: The results indicate that there is a beneficial effect regarding UTI when using hydrophilic-coated catheters.

Gravimetric and chemical features of airborne PM 10 AND PM 2.5 in mainland Portugal.

This paper describes concentration amounts of arsenic (As), particulate mercury (Hg), nickel (Ni) and lead (Pb) in PM(10) and PM(2.5), collected since 1993 by the Technological and Nuclear Institute (ITN) at different locations in mainland Portugal, featuring urban, industrial and rural environments, and a control as well. Most results were obtained in the vicinity of coal- and oil-fired power plants. Airborne mass concentrations were determined by gravimetry. As and Hg concentrations were obtained through instrumental neutron activation analysis (INAA), and Ni and Pb concentrations through proton-induced X-ray emission (PIXE). Comparison with the EU (European Union) and the US EPA (United States Environmental Protection Agency) directives for Ambient Air has been carried out, even though the sampling protocols herein--set within the framework of ITN's R&D projects and/or monitoring contracts--were not consistent with the former regulations. Taking this into account, 1) the EU daily limit for PM(10) was exceeded a few times in all sites except the control, even if the number of times was still inferior to the allowed one; 2) the EU annual mean for PM(10) was exceeded at one site; 3) the EPA daily limit for PM(2.5) was exceeded one time at three sites; 4) the EPA annual mean for PM(2.5) was exceeded at most sites; 5) the inner-Lisbon site approached or exceeded the legislated PMs; 6) Pb levels stayed far below the EU limit value; and 7) concentrations of As, Ni and Hg were also far less than the reference values adopted by EU. In every location, Ni appeared more concentrated in PM(2.5) than in coarser particles, and its levels were not that different from site to site, excluding the control. The highest As and Hg concentrations were found in the neighbourhood of the coal-fired, utility power plants. The results may be viewed as a "worst-case scenario" of atmospheric pollution, since they have been obtained in busy urban-industrial areas and/or near major power-generation and waste-incineration facilities.

Alien hand syndrome without a corpus callosum lesion.

A 58 year old right handed woman developed a left alien hand syndrome after a right capsulothalamic haemorrhage with mesencephalic extension. No lesion was detected in the corpus callosum. Positron emission tomography suggested that right cortical dysfunction might account for the absence of recognition of the left hand whereas involuntary motor activity was attributed to the lesion itself. This case shows that the alien hand syndrome is not always the expression of a disconnection syndrome and may occur after a large, strategically located, posterior lesion.

Defining death. Developments in recent law.

The authors trace the progress among courts, legislatures, and professional and advisory bodies toward the acceptance of legal standards that define death by reference to neurologic criteria. Competing legal formulations and medical proposals are examined, and those standards ultimately adopted in various jurisdictions are set forth.
