Intraarticular placement of Kurosaka interference screws.

Interference screws are commonly used for graft fixation in arthroscopically assisted anterior cruciate ligament reconstruction. A potential complication with the use of interference screws is intraarticular violation of the screw tip. Postoperative radiographs often lend confusion to the exact screw location. We devised an anatomic-radiologic study to evaluate factors (including local anatomy, radiographic projection, and radiographic technique) in order to clarify these misunderstandings. We found that radiographic projection, obliquity, and the local bony anatomy contribute to erroneous radiographic interpretation. The most significant finding was that the lateral radiograph and cephalad oblique lateral radiographs frequently suggest screw tip violation of the joint when in fact it has not occurred. Caudal oblique lateral radiographs fail to raise concern even when joint violation by the screw tip has occurred. Good quality true anteroposterior radiographs appear most consistent in determining true screw tip location.

Adult

Simple bunionectomy with medial capsulorrhaphy.

Bunionectomy with medial capsulorrhaphy was performed on 54 feet (37 patients) and follow-up was obtained on 49 feet (33 patients) at a mean of 4.8 years postoperatively. Results were excellent in 17 feet, good in 16, fair in 7, and poor in 9. Mean motion of the first metatarsophalangeal joint was 81 degrees. The primary reason for failure was recurrent deformity. Patients were dissatisfied with the results in 41% of the feet. Mean intermetatarsal 1-2 angle increased 1.7 degrees and mean metatarsophalangeal-1 angle increased 4.8 degrees. The operation was significantly more successful in patients who underwent it for painful medial eminence. Bunionectomy with medial capsulorrhaphy is recommended for elderly patients with symptoms primarily due to prominent medial eminence, and its application for the young patient with significant hallux valgus deformity is limited.

Genetic analysis of non-recessive factors of resistance to diazinon in the SKA strain of the housefly (Musca domestica L.).

The recent allocation of many visible mutants to specific linkage-groups has made possible their use as genetic markers in the study of the inheritance of insecticide resistance. In most organophosphorus-resistant strains, resistance is controlled by a single gene on the V linkage-group responsible for resistance and low ali-esterase activity, but in some strains more than one factor is present. The present work was done to analyse the genetics of resistance to diazinon in a strain (the SKA strain) bred from two diazinon-resistant strains. Crosses between the SKA flies and four susceptible recessive marker strains, each marked on one linkage-group, followed by test-crosses with recessive markers and by bioassays of each cross, have shown that non-recessive factors for resistance to diazinon are present on the IV and V linkage-groups. The SKA strain genetically resembles its parents but differs by being considerably more resistant.
