[Detection of glycosylated protein of renal tissues using NBT reaction in STZ-induced diabetic rats].

We are carried out to determine the detection of glycosylated protein of renal tissues using nitroblue tetrazolium (NBT) reaction in streptozotocin (STZ)-induced diabetic rats. The levels of glomerular non-enzymatic glycosylation were measured by thiobarbituric acid (TBA) method. There was no significant difference in the intensity of NBT in renal tissues between the 4-week diabetic rats and the same age control rats. The intensity of NBT in the glomerular mesangial areas and capillary walls was marked in 12-week diabetic rats. The staining of NBT was also observed in the tubular epithelial cells in 4- and 12-week diabetic rats. The levels of glomerular non-enzymatic glycosylated protein in 4- or 12-week diabetic rats were significantly greater than those in the same age control rats. It appears that the non-enzymatic glycosylation of renal tissues increased in the early stage of diabetic nephropathy, i.e. 4- or 12-week diabetic rats. It is concluded that the non-enzymatic glycosylation may play an important role in the initiation of renal injuries in diabetic nephropathy.

Isolated bilateral abducens nerve palsies caused by the rupture of a vertebral artery aneurysm.

We report two cases with isolated bilateral abducens nerve palsies due to the rupture of a vertebral aneurysm. Surgery revealed that the aneurysm did not directly compress the abducens nerve. Within a year after the subarachnoid hemorrhage, the patients gained full recovery from the bilateral abducens nerve palsies. In view of the clinical and operative findings, it may be regarded as a compression and/or stretching of the bilateral abducens nerves by a thick clot in the prepontine cistern, and not as a manifestation of the raised intracranial pressure. The mechanisms of the isolated abducens nerve palsy are discussed.

Progress on chemical knowledge of ciguatoxins.

The chemical data setted up on the ciguatoxins responsible for ciguatera fish poisonings are summarized and discussed. The multiplicity of the toxic compounds isolated from fish and algal material is described. A tentative screening of the principal toxins still on process in the two laboratories has shown that (1) CTX is dominant in carnivorous fish, (2) less polar toxins are dominant in herbivorous fish, (3) CTX precursors are produced by G. toxicus in natura and in culture conditions. The increasing polarity of the toxins in step with the food chain levels supports the hypothesis of an oxidative modification of the precursors during the bio-accumulation in fish.

[Comparative study of brain tumors treated at China Medical University, China and Kyushu University, Japan].

We have re-examined histopathological specimens of brain tumors extirpated at China Medical University from May, 1990 to June, 1992. During the last 2 years, about 400 cases of brain tumors were operated and 349 cases were histopathologically diagnosed and classified by one of the authors (T. I). The most common tumor was meningioma, 97 cases, followed by gliomas, 87 case and neurinoma, 71 cases. The sex and age distributions of these three tumor types were compared to those of Kyushu University Hospital, Japan. The most striking difference was age of meningioma and neurinoma patients, and Chinese patients were 10 years younger than Japanese patients. The incidence of 30th age group of meningioma was 25% in China and 10% in Japan. Thirty percent of neurinoma patients were operated at 30th and 40th age in China and Japan, respectively. The sex and ages of astrocytoma patients were almost the same frequencies in China and Japan. Among 22 cases of congenital tumors, 11 cases of epidermoid tumor in cerebello-pontine cistern was included. Cerebral tuberculoma, although rarely encountered in Japan, was 2 cases in China.

Microsurgical and angiographic analysis of anterior communicating artery aneurysms with associated anomalies.

A direct operation was performed on 110 patients with an aneurysm of the anterior communicating artery (ACoA). During the surgery, in 52% of the cases, the neck of the aneurysm was found at the junction of the A-1 segment and ACoA and they presented with a hypoplastic contralateral A-1 segment. Associated ACoA anomalies were seen in 13 (12%) of the 110 cases. The most common anomaly was duplication of the ACoA, found in 11 cases (10%). In 2 other cases triplicate ACoA was found. Preoperative bilateral carotid angiograms were reviewed and duplication of the ACoA associated with the aneurysms able to be pointed out on angiograms in 4 cases, retrospectively. The anatomical and radiological aspects of aneurysms with associated anomalies are also discussed.

[Usefulness of transcutaneous gas monitoring during hemorrhagic shock].

This study was undertaken to confirm whether transcutaneous (tc) gas analysis during hemorrhagic shock could be used as an alternative to mixed venous gas analysis. Tc gases were measured and correlated with arterial and mixed venous gases in 10 anesthetized dogs during hemorrhagic shock and volume resuscitation. Throughout this experiment PtcO2 correlated well with PvO2 (r = 0.78, P less than 0.01), while PaO2 remained mostly constant, and PtcCO2 correlated well with PvCO2 (r = 0.82, P less than 0.01) rather than with PaCO2 (r = 0.63, P less than 0.01). A more detailed observation showed that during progressively decreased cardiac output, PtcO2 became lower than PvO2 and PtcCO2 became higher than PvCO2. We inferred from these observations that the changes of tc and mixed venous gases reflected those gases in tissues and that during severe shock maldistribution of peripheral blood flow prevented mixed venous gases from coming into equilibrium with tissue and tc gases. We conclude that the measurement of tc gases during shock is a more convenient and more reliable monitor of tissue gases than mixed venous analysis.

[Microsurgical anatomy of the region near the porus acusticus internus; arteries around the facial and acoustic nerves bundle].

The microsurgical anatomy of the cerebellopontine cistern around the porus acusticus was studied under the surgical microscope, using the heads of 20 cadavers. The relationships among the porus acusticus, facial and acoustic nerves, and neighboring arteries were noted carefully. The arteries were the meatal loop of the cerebellar artery, the internal auditory artery (I.A.A.), the subarcuate artery (S.A.), and the perforating artery (P. A.). The cerebellar arteries made the meatal loop and the arterial-nerve complex while they passed in front of the porus acusticus. Most of the cerebellar arteries were the main or rostral trunk of the AICA. The I.A.A. supplying the facial and acoustic nerves usually originated from the meatal segment of the cerebellar artery and ran into the anterior part of the porus acusticus to enter the internal auditory canal. The S. A. penetrating the subarcuate fossa made a common trunk with I. A. A. or branched from the cerebellosubarcuate artery. One recurrent P. A., a special type of the P. A., was usually present on each side. Since the artery ran between the nerve bundles of the facial and acoustic nerves, it could not be seen through the lateral suboccipital approach.

Ipsilateral pallidal control on the sternocleidomastoid muscle in cats: relationship to the side of thalamotomy for torticollis.

To elucidate the role of the basal ganglia on neck movement control and thus clarify the side of targets for stereotactic surgery of spasmodic torticollis, effects of electrical stimulation to the globus pallidus-entopeduncular nucleus complex (GP-EP) upon the activities of the sternocleidomastoid (SCM) muscle motoneurons were studied in cats. Repetitive stimulation of the ipsilateral GP-EP increased discharge rate in the majority of the SCM motoneurons, whereas contralateral GP-EP stimulation caused a decrease in discharge rate. These effects were gradually enhanced in the course of repetitive stimulation and lasted even after its cessation. Bilateral ablation of the motor and premotor cortices did not influence the effects of GP-EP stimulation. Latency studies suggested that the pallidal control of the SCM motoneuron is mediated by polysynaptic pathways on both sides; however, these same studies produced evidence of an ipsilateral oligosynaptic connection, as well. Repetitive antidromic stimulation of the accessory nerve caused a gradual increase in discharge rate of the SCM motoneurons, similar to the augmentation induced by ipsilateral GP-EP stimulation. Features of the responses indicate that the pallidal control on the neck muscles involves reverberating circuits in the brain stem and spinal cord. These experimental findings suggest that stereotactic thalamotomy of the ventrolateral nucleus for spasmodic torticollis should be performed on the side ipsilateral to the contracting SCM muscle.

[Therapy and clinical symptoms in patients with obstructive sleep apnea in Japan].

To investigate the long-term outcome of therapy and clinical symptoms of patients with OSAS (obstructive sleep apnea syndrome) in Japan, we studied 34 patients with OSAS who had been diagnosed by standard polysomnography more than one years (36.0 +/- 19.7 months: mean +/- SD) previously. They were 53.8 +/- 10.5 years old, body mass index was 27.8 +/- 5.2 kg/m2, and AHI (apnea & hypopnea index) was 50.0 +/- 24.2/h. The subjects had achieved weight loss (p less than 0.01), but had regained their initial weight at our follow-up. Eleven patients with OSAS were initially candidates for NCPAP (nasal continuous positive airway pressure), but only 5 patients used it for a prolonged time. The other 6 patients with OSAS could not use NCPAP because they did not wish to purchase a NCPAP instrument. One of 5 patients who used NCPAP for a long time died from lung cancer; thus, 4 patients used NCPAP continuously. Ten patients were commenced on ACZ (acetazolamide); however, only 5 patients took it continuously. ACZ resulted in some improvements in the sleep parameters (AHI index, desaturation time below SaO2 90%), but apnea & hypopnea duration and the difference in transcutaneous PCO2 between wake and sleep were not significantly improved by ACZ administration. ACZ was not as effective as NCPAP. Almost 60% of patients with OSAS had excessive daytime sleepiness. Hypertension was detected in about 60% of patients. Nine of 25 patients who had an automobile license had had more than one automobile accident. Nine patients who had had more than one automobile accident showed AHI greater than or equal to 30 in our study.(ABSTRACT TRUNCATED AT 250 WORDS)

Lectin histochemistry of human fetal notochord, ecchordosis physaliphora, and chordomas.

Lectin histochemical studies of human fetal notochord, ecchordosis physaliphora, and eight chordomas were performed. Ecchordosis physaliphora and eight chordomas were stained with Ricinus communis type I, Canavalia ensiformis, Triticum vulgaris, and Limax flavus. Ricinus communis type I, T vulgaris, and L flavus reacted with both tumor cells and the extracellular mucinous matrix, while C ensiformis mainly reacted with the cytoplasm of tumor cells. Thus, tumor cells were most recognizable with the C ensiformis stain. After neuraminidase treatment, ecchordosis physaliphora and chordomas invariably showed positivities for Arachis hypogaea. The lectin-binding patterns of chordomas closely reflected those of the human fetal notochord. Chordomas were completely sialylated regardless of either the clinical course or histopathological findings. Among the eight lectins, C ensiformis heavily labeled chordoma cells but not extracellular space. This was probably because asparagine-linked N-glycosylation was active in the chordomas, while the high-mannose-type oligosaccharides were apt to remain in the cytoplasm.

Interactions of an antimicrobial peptide, tachyplesin I, with lipid membranes.

Tachyplesin I, isolated from the acid extracts of hemocytes of Tachypleus tridentatus, is a cyclic broad-spectrum antimicrobial peptide forming a rigid, antiparallel beta-sheet because of two intramolecular S-S linkages. The strong binding of the peptide to lipopolysaccharides cannot explain the susceptibilities of gram positive bacteria and fungi to the peptide. We found that tachyplesin I caused a rapid K+ efflux from Escherichia coli cells, concomitant with a reduced cell viability. This result suggests that the peptide-induced permeability enhancement of the bacterial membranes may be a plausible action mechanism. Thus, we studied the interactions of tachyplesin I with various large unilamellar vesicles (LUVs) to reveal the molecular machinery of the antimicrobial activity. Tachyplesin I induced the leakage of calcein, a trapped fluorescent marker, from LUVs of acidic phospholipids, especially phosphatidylglycerol (PG), but not from phosphatidylcholine LUVs. A detailed analysis found that the affinity of the peptide to the PG membranes is very strong and that the binding of one peptide molecule to approx. 200 lipid molecules leads to a significant leakage. The location of tachyplesin I in membranes was estimated by use of the Trp-2 fluorescence of the peptide. The presence of PG LUVs caused a blue shift of the maximum wavelength, an increase in the quantum yield, and a complete protection from fluorescence quenching by an aqueous quencher, acrylamide. Moreover, the degree of fluorescence quenching of the Trp residue by n-doxylstearates was in the order n = 5 greater than 7 greater than 12 approximately equal to 16. These results show that the Trp residue of tachyplesin I seems to locate in a hydrophobic environment near the surface of the PG bilayers.

Purification and characterization of three forms of differently glycosylated recombinant human granulocyte-macrophage colony-stimulating factor.

We have purified recombinant human granulocyte-macrophage colony-stimulating factor (hGM-CSF) produced in human lymphoblastoid Namalwa cells. From the results of tunicamycin treatment and N-glycosidase F digestion, it was demonstrated that Namalwa-derived hGM-CSF was highly glycosylated at two potential N-glycosylation sites and several O-glycosylation sites as previously shown for naturally occurring hGM-CSF. We classified the hGM-CSF molecules into three groups according to the molecular weight corresponding to the degree of N-glycosylation: the molecules with two N-glycosylation sites occupied (designated 2N), the molecules with either site glycosylated (1N), and the molecules lacking N-glycosylation (0N). Despite such varied degrees of N-glycosylation, almost all molecules were O-glycosylated. To investigate the role of carbohydrate moieties of hGM-CSF, we isolated each form of hGM-CSF and examined its biological properties. The 2N-type showed 200-fold less in vitro specific activity compared with unglycosylated Escherichia coli-derived hGM-CSF, although the activity of the 0N-type was equivalent to that of the E. coli-derived material. The 1N-type showed an intermediate level of activity. However, in terms of clearance from blood circulation in the rat, the 2N-type showed a half-life five times longer than that of the 0N-type and E. coli-derived hGM-CSF. From these findings, we concluded that N-linked carbohydrate moieties of hGM-CSF play conflicting physiological roles in the efficacy of the protein in vivo but that O-linked carbohydrate moieties do not have such effects.

[MRI of brain abscesses].

Four cases of brain abscess were examined on 1.5T MR and their findings were evaluated retrospectively. Preoperative cases had relatively characteristic findings, such as 1) peripheral edema producing hypointensity on T1-weighted images (T1WI) and hyperintensity on T2-weighted images (T2WI); 2) central necrosis with abscess fluid hypointense relative to white matter on T1WI and hyperintense on T2WI; 3) abscess rim iso- to hypointense on T2WI. Gd-DTPA enhanced T1WI carried further information; comparison of iso- to hypointense rim on T2WI (abscess rim) and enhanced area by Gd-DTPA on T1WI suggested that the former represented zone of macrophage infiltration.

Pineal and third ventricle tumours in the CT and MR eras.

We have experienced 29 cases of tumour in the pineal region and posterior third ventricle in the CT and MR eras (from 1976 to 1989). An aggressive surgical removal was performed except for germinomas and suspected germinomas. The aggressive surgical cases were 5 teratomas including one inclusion foetus, one yolk sac tumour, 3 astrocytomas, one neurocytoma, one cavernous haemangioma and one telangiectasia. The surgery was performed by either the infratentorial supracerebellar approach or the occipital transtentorial approach. There was no surgical death and the quality of postoperative survival was good in patients with benign tumours. The cases of a typical teratoma, teratoma with an arterio-venous shunt, inclusion foetus, haemorrhagic nodule of telangiectasia etc. are briefly presented. A remarkable progress in the diagnosis and surgical treatment of tumours in the pineal region in the CT and MR eras is noted in our results.

Glioma-derived PDGF-related protein presents as 17 kd intracellularly and assembled form induces actin reorganization.

We have electrophoretically obtained platelet-derived growth factor (PDGF)-related protein from human glioma (glioma derived PDGF-related protein: GD-PDGF) and produced rabbit antiserum against the monomer of GD-PDGF. By methods of immunoaffinity chromatography and Western blotting, we analyzed GD-PDGF in cultured human glioma cells and conditioned medium. The intracellular GD-PDGF was only detected at 17 kd molecular weight by the purified rabbit antibody. When the intracellular 17 kd monomer was purified by the IgG-coupled immunoaffinity chromatography, the eluted protein was not detected at 17 kd but at 52 kd. The 52 kd GD-PDGF was spontaneously and immediately converted to 56 kd, which was partly degraded to 32 and 35 kd within 24 hours. On the other hand, in the conditioned media of glioma cell lines GD-PDGF presents mainly as 56 kd. The assembled forms of GD-PDGF exhibited a powerful activity to induce membrane ruffle formation and reorganization of actin filaments in cultured glial cells and glioma cells. These results indicated that GD-PDGF is intracellularly stored as 17 kd monomer and exists extracellularly as assembled forms, which may act as an autocrine and paracrine effect on the surrounding cells.

Comprehensive lectin histochemistry of normal and neoplastic human choroid plexus cells: alternation of lectin-binding patterns through neoplastic transformation.

Lectin histochemistry of the normal and neoplastic human choroid plexus cells [six choroid plexus papillomas (CPPs) and three choroid plexus carcinomas (CPCs)] was performed using eight representative lectins to study the development of sugar chain structures and also to determine whether lectins were useful for a histopathological diagnosis of choroid plexus neoplasms (CPNs). The normal choroid plexus cells reacted with Ricinus communis (RCA-I). Canavalia ensiformis (Con A), Limax flavus (LFA) and Triticum vulgaris (WGA), while Arachis hypoaea (PNA) stained them only after the removal of sialic acid. Human fetal choroid plexus cells at 8 weeks gestation already showed the same lectin-binding patterns as adult ones. All CPNs were stained by RCA-I and Con A in a similar manner as the normal choroid plexus cells. Although seven CPNs were positive for LFA, two CPCs were not stained by LFA, which bound to sialic acid. Two LFA-positive CPPs were stained by PNA before the removal of sialic acid. Moreover, unlike the normal choroid plexus cells, Ulex europaeus-, Glycine maximus- and Dolichos biflorus-binding sites often appeared, and WGA-binding sites of three CPNs remained even after sialic acid removal. In conclusion, the glycosialylation in normal choroid plexus cells was completed during the early embryonic stage. The lectin-binding patterns of CPNs were heterogenous in each case. The alternation of the glycosialylation and/or acquisition of binding sites for some lectins was sometimes observed through a neoplastic transformation.

Lateral ventricular arteriovenous malformations: natural history and surgical indications.

The authors report 21 cases of lateral ventricular arteriovenous malformation (AVM) focusing on their natural history and surgical indications. Eighteen of 21 patients (86%) had bleedings prior to admission. We performed definitive surgery in 9 patients and conservative treatment in 12 patients. In 7 of 9 patients (78%) in the operative group and in 5 of 12 patients (42%) in the non-operative group, the nidus of the AVM was less than 4 cm in diameter. The other 2 AVMs in the operative group, more than 4 cm in diameter, were located in the temporal lobe and widely extended to the temporal horn of the lateral ventricle. The nidi of the AVMs were totally removed in all 9 cases in the operative group. All patients have been doing well except one who had a mental breakdown 6 years after surgery. In 10 patients in the non-operative group, AVMs were located in the left cerebral hemispheres. Three patients in the non-operative group had rebleeding of the AVM. Two of the 12 patients in the non-operative group (17%) died of the recurrent haemorrhages. The rate of bleeding of the lateral ventricular AVM seems to be higher than that of the cerebral cortical AVM, but the mortality due to recurrent bleeding might be similar between the two. The operative indications we made depended on the sizes and locations of the AVMs. For AVMs in the temporal horn, even though they were large, we performed total removals of the nidi and had good results. We did not perform any definitive surgery for AVMs more than 4 cm in diameter except for those in the temporal horn. The mortality and morbidity of the 21 cases were 10% and 14%, respectively. Patients with AVMs in the temporal horn and patients with small AVMs in the frontal horn were good candidates for definitive surgery. We undertook conservative treatment for patients with large AVMs, and the results were acceptable.
