[Pure red cell aplasia induced by B19 parvovirus during allogeneic bone marrow transplantation].

We report a patient who had abrupt onset of pure red cell aplasia (PRCA) induced by B19 parvovirus during allogeneic bone marrow transplantation (BMT). A 14-year-old girl with APL in complete remission was admitted in February 1988, for the purpose of BMT. She was received marrow from HLA identical sister on March 17, 1988 (day 0). She received 120 mg/kg cyclophosphamide and 12 Gy total body irradiation for conditioning of BMT. For graft-versus-host disease (GVHD) prophylaxis she was given cyclosporine and short term methotrexate. She did not develop acute GVHD after BMT, but on the day 28 a bone-marrow aspirate revealed findings of PRCA. During this course the number of white blood cell and platelet favorably recovered. B 19 parvovirus DNA was detected in the serum of the day 30 and day 42. Antihuman B 19 parvovirus (HPV) antibody titers were increased: the values of anti-HPV IgM were suddenly elevated and those of anti-HPV IgG were elevated. Serum on the day 42 inhibited erythroid progenitors (CFU-E, BFU-E) but not inhibited myeloid progenitors (CFU-C). A reticulocyte count recovered on the day 50. As the patient was HPV-IgG negative prior to BMT and the donor was HPV-IgG seronegative, the source of infection may be platelet transfusion (day 7 through 14).

[Transient fever with abnormal liver function in patients underwent bone marrow transplantation--clinical symptom of cytomegalovirus infection].

In seventeen (38.6%) out of forty-four children underwent bone marrow transplantation, transient fever with abnormal liver function tests occurred between 30 and 70 days following bone marrow transplantation. More than half of cases suffered from nausea, anorexia and general malaise. C reactive protein was negative in all but one case, and thrombocytopenia occurred in five cases. Transient fever with abnormal liver function tests occurred more frequently in patients with cytomegalovirus (CMV) infection (63.4%) than in patients without CMV infection (16.0%) (P less than 0.01). Therefore, CMV infection was considered to be the cause of transient fever with abnormal liver function tests. The incidence of transient fever with abnormal liver function tests was not significantly different between the patients with pretransplant positive anti-CMV titer and the patients with pretransplant negative anti-CMV titer. Clinical symptoms improved in 16 patients (93.4%), but fatal CMV interstitial pneumonia followed in one case.

Large cell carcinoma of the lung secreting human chorionic gonadotropin which responded to combination chemotherapy: case report.

A 68-year-old man was admitted to the National Cancer Center Hospital on November 10, 1988 to undergo chemotherapy for recurrent lung cancer, which had been resected on June 22, 1988. The tumor was diagnosed histologically as large cell carcinoma with trophoblastic differentiation, and human chorionic gonadotropin (HCG) was immunohistochemically found in tumor cells. Chemotherapy was initiated on November 14, 1988. The patient received three courses of methotrexate, actinomycin D and cyclophosphamide (MAC); and two courses of cisplatin and etoposide (PVP). Following this therapy, partial response was achieved, however, the tumor soon progressed and the serum HCG level increased to 7,571 mIU/ml. The patient was consequently given four courses of cisplatin, adriamycin, cyclophosphamide and etoposide (PACE) up to September 13, 1989, and received 52 Gy irradiation to the chest from July 20 to August 24. The tumor regressed markedly and the serum HCG decreased to within normal limits. The changes in serum HCG levels in serial samples correlated well with the clinical tumor burden. The patient is now in partial remission having survived for more than 15 months after the initiation of chemotherapy.

[Clinical utility of long-term administration of trapidil (MDR-1865) in the treatment of diabetic retinopathy].

In order to investigate the usefulness of prolonged administration of trapidil in the treatment of diabetic retinopathy, a follow-up study was carried out after the end of a placebo-controlled, double-blind study of the drug in case of the trapidil-treated group who continued to be treated with 300 mg/day of trapidil (Rocornal) (TT group) and those of the placebo group who were not given any treatment after the end of the study (PP group). The TT group was significantly superior or tended to be superior to the PP group in improvement of hard exudates, retinal hemorrhages and soft exudates, and also in improvement of capillary microaneurysms on fluorescein fundus angiography. In the course of diabetic retinopathy by the global evaluation of findings in individual cases, the TT group was significantly superior or tended to be superior to the PP group. Photocoagulation was performed on 20 (28%) of 72 eyes of the PP group and 13 (19%) of 68 eyes of the TT group, with the mean interval to photocoagulation being 7.2 months for the PP group and 14.0 months for the TT group. There was a significant difference between the 2 groups in terms of the interval to photocoagulation. Randomized fundus photographic evaluation of the findings by the central evaluation committee showed that the TT group was significantly superior or tended to be superior in improving retinal hemorrhages, soft exudates and leakage of fluorescein dye, and was also significantly superior or tended to be superior in relation to the course of the retinopathy, which was consistent with the evaluation by the attending ophthalmologists.(ABSTRACT TRUNCATED AT 250 WORDS)

[Experimental study on the difference of ischemic threshold between cerebral cortex and thalamus with middle latency auditory evoked potentials].

Middle latency auditory evoked potentials (ML-AEPs) have not yet been established as a device of assessing neurological events because their origins have not been definitely identified. In this study, we assessed the neurological usefulness of MLA-EPs through experimental approach using canine models of acute ischemia localized within the cerebral cortex or thalamus. Two types of localized cerebral ischemia were produced in mongrel dogs by clipping of major cerebral arteries and inducing of hypotension; they were, unilateral cortical ischemia involving the right primary auditory area (group A) and unilateral thalamic ischemia involving the right medial geniculate body (group B). Using these two ischemia models, auditory evoked potentials (AEPs) were intracranially and extracranially recorded in addition to the measurement of local cerebral blood flow (1-CBF). Prior to the induction of ischemia, it was confirmed that positive waves with a latency of 20 ms (P20) were evoked in the bilateral primary auditory areas by sound stimulation (90 dB 5 Hz click) given to the ear contralateral to the planned ischemic side in both groups. The right P20 disappeared when the 1-CBF in the right primary auditory area decreased below the ischemic flow threshold of synaptic transmission failure, approximately 18 ml/100 g/min, in group A, and when the 1-CBF in the right medial geniculate body decreased below the threshold, 10 ml/100 g/min, in group B.(ABSTRACT TRUNCATED AT 250 WORDS)

[A sensitization effect of hematoporphyrin oligomer (HpO) and caffeine for X-ray radiation of skin cancer].

Human malignant melanoma cells (G-361) were irradiated with X-ray for different irradiation times. The amount of DNA damage was much increased by the treatment of the cells with HpO before X-ray irradiation. Furthermore, it was found that the caffeine inhibited effectively the repair of the damaged DNA. Furthermore, HpO was administered before X-ray irradiation of transplantable skin tumor in C3H mice and/or caffeine+ was to be given the mouse after X-ray irradiation. The growth of the tumor irradiated with HpO and/or caffeine was more inhibited and the necrotic area was more expanded than that of the tumor irradiated X-ray only. Labeling Index utilizing bromodeoxyuridine (BrdU) in the viable area of the treated tumor decreased when HpO and/or caffeine were administered. The amount of DNA damage more increased in the tumor cells irradiated with HpO than that irradiated only. And, the repair of the damaged DNA was more delayed in the tumor cells irradiated with HpO and caffeine than that irradiated with the other treatments. In conclusion, it was considered that HpO might be a radioactive sensitizer and caffeine enhanced the inhibition of the repair of DNA damaged. We hope that the treatment of X-ray radiation with HpO and caffeine is useful for clinical treatment of human malignant tumor.

Analysis of GFA-protein mRNA expression in developing bovine brain by in situ hybridization and northern blot hybridization.

Using bovine brains of adult and developmental stages, the time and place of the appearance of mRNA for glial fibrillary acidic protein (GFA-protein) were studied by in situ- and Northern blot-hybridization. Double-stranded cDNA labeled with [3H]dCTP or [32P]dCTP was used as the probe for this mRNA. To compare the location of GFA-protein mRNA and GFA-protein itself on serial sections. GFA-protein immunohistochemistry was used. By in situ hybridization with adult bovine brain. GFA-protein mRNA was detected in astroglia, most of which were in the white matter. The distribution of these astroglia by in situ hybridization was consistent with the findings by GFA-protein immunohistochemistry and Northern blot hybridization, indicating that each techniques were specific. Concerning fetal stages, GFA-protein mRNA could be detected in the brain of a fetal calf with a body length of 28 cm by in situ hybridization using the 32P-labeled probe, and the mRNA was localized in the subpial area and the fornix. These results indicated that glial maturation first became recognizable at least in the subpial area and the fornix in the brain of a fetal calf measuring 28 cm. In this fetal brain, GFA-protein mRNA was almost undetectable by Northern blot hybridization. This suggested that in situ hybridization was more sensitive and useful for the analysis of gene expression than Northern blot hybridization, when the target mRNA is present in only a limited area, such as in the brain.

[Aspergillosis of the maxillary sinus in a patient with Ph1 positive acute lymphoblastic leukemia: a case report].

A 46-year-old woman was admitted to our hospital because of leukocytosis. A diagnosis of acute lymphoblastic leukemia (FAB: L2 type) was made by reviewing peripheral blood smear and bone marrow aspirate. Chromosome analysis showed the presence of Philadelphia chromosome. A combination chemotherapy with L-asparaginase, doxorubicin, vincristine, and prednisolone was started, but complete remission was not achieved. During a neutropenic period after combination chemotherapy with doxorubicin, vincristine, vinblastine, and VP-16, high fever and tender swelling of the right cheek were noticed. A diagnosis of maxillary sinusitis was made with tomography and CT scan of the maxillary sinus. Since culture of the aspirate from the maxillary sinus grew aspergillus, a diagnosis of aspergillosis of the maxillary sinus was made. Immediately after the intravenous administration of amphotericin B and the lavage of the sinus with amphotericin B was started, high fever subsided and clinical improvement was observed. Several regimens of chemotherapy failed to obtain hematological remission, she died of sepsis of Enterobactor cloacae without evidence or relapse of dissemination of aspergillosis after initial successful treatment. While a few cases with aspergillus maxillary sinusitis were reported in leukemic patients, the possible occurrence of this complication must be kept in mind in a severe neutropenic period after intensive chemotherapy. The combination of intravenous administration and local lavage of amphotericin B appeared to be an effective treatment in the Aspergillus maxillary sinusitis.

[Autologous bone marrow transplantation in children with acute leukemia].

Eight children with acute leukemia were treated with chemoradiotherapy or high-dose chemotherapy followed by infusion of autologous marrow unpurged or purged mainly with monoclonal antibodies. Four were treated in their first remission, three in their second remission, and one in the fourth remission. Patients were isolated in laminar air flow rooms with sterile conditioning. Infused mononuclear cells ranged from 1.0 to 3.8 x 10(7)/kg. Hematologic engraftment was obtained in five patients. The other three patients, who were treated with monoclonal antibody or drug, received unpurged back up marrow because of delayed bone marrow reconstitution. Two recovered, but one died before engraftment. Hematologic recovery was delayed after autologous bone marrow transplantation. The median time to achieve 1,000 leukocytes/microliters, 500 neutrophils/microliters, and last platelet transfusion was 36 days (12-90), 36 days (12-90), and 70 days (39-214), respectively. Moreover, further increases beyond these levels remained unusually slow, especially in platelets. Two of eight patients relapsed at 5 and 15 months and the other patient died of interstitial pneumonitis. Actuarial disease free survival was 62.5%. There was a trend toward improved disease free survival for patients transplanted in the first remission (100%).

Role of actin filaments and microtubules in the spreading of rabbit corneal epithelial cells on the fibronectin matrix.

The migration and spreading of the corneal epithelial cells adjacent to a wound is the first step in successful epithelial resurfacing. To understand the role of actin filaments and microtubules of the cytoskeletal system in the spreading of corneal epithelial cells, we plated rabbit corneal epithelial cells on a fibronectin matrix and studied the effects of cytochalasin B, which inhibits actin filaments assembly, and colchicine, which inhibits microtubules assembly, on the ability of individual cells to spread. Changes in the morphology of actin filaments and microtubules were also studied using immunofluorescent microscopy. The area of spread epithelial cells depended on the concentration of fibronectin used to coat the surface. In spread cells, stress fibers of actin filaments were evenly distributed throughout the cytoplasm, whereas microtubules were observed only at the perinuclear region. The presence of cytochalasin B during the cell attachment and spreading decreased the area of the spread cells more than did colchicine. However, once the epithelial cells were spread on a fibronectin matrix, cytochalasin B and colchicine each decreased the cell area only slightly, and to the same extent. These results indicate that formation of actin filaments is more important than formation of microtubules to the spreading of corneal epithelial cells.

[A case of tuberculous pericarditis].

A case of tuberculous pericarditis successfully managed with medical treatment alone was reported. A 78-year-old male was admitted because of cough, dyspnea and fever. Chest X-P and echocardiogram revealed massive pericardial effusion. His clinical symptoms and signs suggested cardiac tamponade. Mycobacterium tuberculosis was detected from pericardial fluid. ADA activity in pericardial fluid was high. Thoracic CT scan showed tracheobronchial, pretracheal, paratracheal and superior mediastinal lymph-node swelling. The diagnosis of tuberculous pericarditis was confirmed. Anti-tuberculous therapy consisting of INH, RFP, EB in combination with prednisolone was started. One month later pericardial effusion was controlled and six months later he was in good clinical condition without surgical treatment.

[Three adult intussusception cases with colorectal cancer; CT diagnosis and its characteristic findings].

We have experienced three adult intussusception cases with colorectal cancer. In all three cases CT studies were performed prior to administering a barium enema, where we could make the exact diagnosis of the lesions in both the plain and the contrast studies. In the CT studies, the intussusception lesions showed the characteristic eccentric layered pattern. It was difficult to distinguish the tumors from the edematous intestinal walls at the proceeding portion in two cases. While in one case, the proceeding portion (the tumor itself) was pointed out as the enhanced soft tissue density mass of layered pattern with the irregular margins.

Relationship of corneal endothelial morphology to diabetic retinopathy, duration of diabetes and glycemic control.

Using specular microscopy and computer-assisted morphometry, the morphologic features of the corneal endothelium were evaluated in three groups of patients with type II diabetes mellitus: 20 patients without diabetic retinopathy, 24 with background retinopathy, and 26 with proliferative retinopathy. When compared to age-matched nondiabetic controls (30 patients), all diabetic groups had similar endothelial cell densities but demonstrated significant increases in cell size and shape variability (pleomorphism). However, there was no significant difference in the degree of these endothelial changes among the three diabetic groups. Moreover, none of the endothelial morphologic parameters was found to correlate with the duration of diabetes or glycemic control, as estimated from glycosylated hemoglobin (HbA1) concentrations.

[A primary malignant melanoma of the small intestine].

In October 14, 1982, a 30-year-old male was admitted to the hospital because of a left flank pain and melena. On undergoing testing, an upper gastrointestinal series revealed barium retention in the small bowel, in which an abdomen X-p showed an abnormal gas shadow. Endoscopic examinations revealed depressive lesions, and the histological diagnosis was malignant neoplasm with a duodenal wall infiltration. A superior mesentric arteriogram showed the dilatation of the media colic artery and neovascular lesions. Thus a leiomyosarcoma of the small intestine was suspected, and the patient underwent surgery on November 15. The tumor was sack-like and looked black; histologically it was diagnosed as a small intestine, because the primary origin was only detected in the small intestine. Four months after the operation, the tumor recurred and the patient died in June, 1983.

[Pharmacokinetics and clinical studies of aztreonam in neonates and premature infants].

Pharmacokinetics and clinical evaluation of aztreonam (AZT) in the neonates and premature infants were studied with the following results: 1. Serum concentrations of AZT in 60 minutes intravenous drip infusion of AZT 20 mg/kg to 6 cases of neonates with 2 to 22 days of age were 45.5 +/- 0.87 micrograms/ml immediately after the completion of intravenous drip infusion, 37.8 +/- 1.62 micrograms/ml 1 hour after, 31.2 +/- 1.92 micrograms/ml 2 hours after and 19.7 +/- 2.36 micrograms/ml 4 hours after, respectively. Serum half-life was 3.61 +/- 0.53 hours on the average. 2. Urinary excretion rate 6 hours after intravenous drip infusion was 26.4 +/- 6.86% on the average. 3. Clinical evaluation was given to 1 sepsis case of 7-days of age and it was effective. There was no abnormal clinical or laboratory finding considered to be associated with AZT.

[A comparison of the functional vulnerability of the cerebral cortex and thalamus to acute ischemia].

Using cerebral cortical and thalamic ischemia models produced in mongrel dogs, the reversibility of short-latency somatosensory-evoked potentials (SSEPs) and the effects of ischemic brain edema on reversibility were compared. The mean systemic blood pressure (MSBP) of animals was reduced by exsanguination until cortical SSEPs disappeared, and was held constant at that level. The MSBP was recovered by autogenous blood transfusion at 30 minutes (subgroup A), 60 minutes (subgroup B) and 90 minutes (subgroup C) after SSEP disappearance in the cortical ischemia group; and at 15 minutes (subgroup D) and 30 minutes (subgroup E) after SSEP disappearance in the thalamic ischemia group. Local cerebral blood flows (lCBF) were measured and SSEPs were monitored serially up to 2 hours after blood transfusion. At the end of measurement, the leakage of Evans blue was evaluated and brain tissue water content was measured. Cortical SSEPs disappeared when lCBF in the right cerebral cortex, measured by hydrogen clearance method decreased to 18.4 +/- 5.4 ml/100 g/min (mean +/- SD) and neuronal transmission failure in thalamus occurred when thalamic blood flow decreased to 10.0 +/- 3.3 ml/100 g/min. After blood transfusion. SSEP reappeared in all 12 animals in subgroup A, but did not appear in 2 of 9 animals in subgroup B and in all of 7 animals in subgroup C.(ABSTRACT TRUNCATED AT 250 WORDS)

Decrease in polylactosaminoglycans associated with lysosomal membrane glycoproteins during differentiation of CaCo-2 human colonic adenocarcinoma cells.

The proportion of labeled polylactosaminoglycans found in glycoproteins decreases during spontaneous differentiation of CaCo-2 human colonic adenocarcinoma cells to enterocytes in culture (A. Youakim and A. Herscovics, Biochem. J., 247: 299-306, 1987). To identify polylactosaminoglycan-containing glycoproteins, CaCo-2 cells were incubated with [3H]glucosamine or [3H]fucose, for 24 h, and membrane glycoproteins solubilized with 0.5% Nonidet P-40 were fractionated by affinity chromatography on Datura stramonium (DSA)-agarose. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis and fluorography showed that a restricted set of glycoproteins with a molecular weight of about 100,000 bound to DSA-agarose. These labeled glycoproteins were shown to contain polylactosaminoglycans by DSA-agarose chromatography and endo-beta-galactosidase digestion of Pronase-derived glycopeptides. Immunoprecipitation of the [3H]glucosamine-labeled Nonidet P-40 extract with polyclonal antibodies to the lysosomal membrane proteins h-lamp-1 and h-lamp-2 followed by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and fluorography also revealed a band with a molecular weight of about 100,000. The immunoprecipitates were digested with Pronase, and the resulting glycopeptides were first fractionated on Bio-Gel P-6 into excluded (Fraction I) and included (Fraction II) glycopeptides, and then by DSA-agarose affinity chromatography. A much greater proportion of labeled glycopeptides in undifferentiated cells (3 to 5 days in culture) than in differentiated cells (19 to 27 days in culture) was recovered in Fraction I; these glycopeptides were bound to DSA-agarose and were sensitive to endo-beta-galactosidase. This decrease in polylactosaminoglycans was associated primarily with h-lamp-1. These results indicate that h-lamp-1 of CaCo-2 cells contains polylactosaminoglycans and that it undergoes a change in glycosylation with differentiation.
