[Investigated reports of tooth fractures cases from November, 1986 to September, 1988].

Researching the actual condition of tooth fractures, clinical records and X-ray photographs of 2717 patients, visited P.D.I. Dental Clinic from November, 1986 to September, 1988 were investigated statistically. 174 cases were tooth fractures in 2717 patients. The frequency rate of tooth fractures at an early age was low, increased from the latter half of the 30s and was highest at the latter half of the 50s accounted for over 10%, and decreased from the 60s. In the upper anterior teeth, the crown fractures caused by traumatic factors, at an early age showed a high proportion, and in the lower anterior the tooth fractures were rare. In the posterior the root fractures of non-vital teeth attributed to dental treatment at the middle and advanced age indicated a high proportion. In the case of tooth restored with metalcore and bridge, the proportion of the root fractures were high, the metalcore, was used in 63%, and in the bridge in 27% of the root fractures.

[Influence of surface properties on the bending strength of OPTEC porcelain].

Influence of properties on bending strength of OPTEC porcelain, an experiment on four-point bending test was studied using four kinds of specimens (5 X 3 X 20) made of OPTEC porcelain (INCISAL) each of which was surface-treated; (1) glaze firing (no treatment) (2) polishing by emery paper #150 (3) polishing by alumina powder (0.3 microns) (4) glaze firing after polishing by emery paper #150 The test specimens were immersed in distilled water maintained at 37 +/- 1 degrees C broken into four-point bending at 1.0 mm/min, and their bending jig had on inner and outer span of 5 mm and 15 mm. The specimens graze fired after polishing had the highest bending strength, of 914 kg/km2, those polished with alumina powder, 831 kg/cm2, those graze 763 kg/cm2, and those polished with emery paper 705 kg/cm2. In conclusion, the bending strength of the porcelain was decreased by the surface roughness.

Between January, 1983 and January 1988, the diagnosis of hyperparathyroidism was made on 14 patients (8 primary and 6 secondary), and was identified operatively in all. The procedures for image diagnosis were computed tomography (CT), ultrasonography and scintigraphy. Scintigraphy was performed using Tc-99m and Tl-201 by a subtraction technique. For primary hyperparathyroidism, the accuracy of localization was 100% by CT, 87.5% by ultrasonography, 100% by scintigraphy, and 100% by the combination of these three methods. For secondary hyperparathyroidism, it was 68% by CT, 68% by ultrasonography, 36% by scintigraphy and 84% by the combination of these three methods. Ultrasonography and CT were quite useful for imaging of small abnormal parathyroid glands. The accuracy of scintigraphy in secondary hyperparathyroidism was low. Because the swellings of glands do not occur equally in most secondary hyperparathyroidism cases, we consider that large glands absorb the majority of injected radionuclides. Some discussion on merits and demerits of each image procedure was done.

[Treatment of urinary tract infection with cefixime].

Cefixime (CFIX) was administered orally in the treatment of 59 cases of urinary tract infection (UTI). According to the response criteria defined by the Japanese UTI committee, the rate of clinical efficacy for 26 cases of uncomplicated cystitis was 96.2%. The rate of clinical effectiveness of five patients with complicated UTI and five with uncomplicated pyelonephritis was 80.0%. In the other 23 cases which did not meet the response criteria, the efficacy rate was also high. The results indicated that CFIX was an effective drug for the treatment of UTI.

[A papillary-cystic neoplasm of the pancreas, a calcifying-inverted variant].

A calcifying-inverted variant of a papillary-cystic neoplasm (PCN) is reported, presenting presented the same histological and cellular features, and similar immunohistochemical characteristics and the ultrastructure of a papillary-cystic neoplasm of the pancreas (PCN) that usually afflicts younger women. It took the form of a partiality of a capsule, a common trait for a PCN, and proliferated on infiltrating the pancreatic parenchyma. The case involves a 51-year-old woman, whose PCN was detected by an abdominal X-ray that showed a calcified shadow in a health examination, though there was no palpable abdominal tumor. Usually, a PCN is detected by palpation tumor. Thus, the author demonstrates the existence of PCN variants, characterized by intrapancreatic infiltrating proliferation and an incomplete cyst formation or intratumoral calcified foci, and reports on variants of the usual PCN.

The effect of temperature on recombination activity in testes of rodents.

An extractable enzyme system capable of catalyzing recombination in vitro was described in murine spermatocytes [Hotta et al. (1985) Chromosoma 93, 140-151]. The system is specific to meiosis, its activity increasing 400-fold between the premeiotic S-phase and mid-pachytene. The present study examines the effect of temperature on this system since the elevation of testicular temperature is one of the major factors causing impairment of testicular function. A strong depression of in vitro recombination activity occurred immediately after raising the testicular temperature in vivo by translocating the testes into the abdominal cavity (cryptorchid). The in vitro study also showed that the extract from spermatocytes preferred lower temperatures (30-32 degrees C) than somatic cells (37 degrees C) for maximal activity of recombination. These results suggest that the strong depression of recombination activity may be an important factor which causes degeneration of testes by heat.

Deoxyribonucleic acid polymerase activity in the testes of infertile men with varicocele.

Activities of deoxyribonucleic acid (DNA) polymerase alpha, beta, and gamma were measured in extracts of testicular biopsy specimen obtained from 37 cases of male infertility with left varicocele and compared with those of 6 normal controls. It was observed that levels of DNA polymerase alpha, beta, and gamma were significantly lower in infertile men than normal controls on both sides of testes. Among three DNA polymerases, the level of DNA polymerase beta activity well correlated with the histological findings (Johnsen's score), i.e., the extent of differentiation of germinal cells. DNA polymerase beta activity appeared to be the lowest in the patients whose sperm density was less than 5 X 10(6)/ml. On the other hand, no correlation was apparent between levels of DNA polymerases and other clinical parameters, e.g., testicular volume, sperm motility, grade of varicocele, and serum hormone levels. These results suggest that the combined decrease in the DNA polymerase activities may be one of the factors that have deleterious effects on spermatogenesis in varicocele patients.

Radioimmunotherapy of transplanted small cell lung cancer with 131I-labelled monoclonal antibody.

Monoclonal antibody TFS-4 has previously been shown to react selectively with human small cell lung cancer (SCLC). We evaluated the use of 131I-labelled TFS-4 for the treatment of established human SCLC transplanted in nude mice. The specific accumulation of the antibody in the transplanted tumour was recorded by both scintigraphic and biodistribution studies. Administration of 200 microCi 131I-labelled TFS-4 inhibited tumour growth when compared with the same radiation dose of the control monoclonal antibody. The therapeutic effect was dose-dependent and complete disappearance of the tumour was observed transiently in one out of the three animals following the administration of 500 microCi 131I-labelled TFS-4.

Changes of enzymes involved in DNA synthesis in the testes of cryptorchid rats.

The activities of DNA polymerase alpha (EC 2.7.7.7) and topoisomerase I did not fluctuate up to 7 days after surgery to induce cryptorchidism and showed no significant difference from those in control testes (sham-operated). In contrast, the activity of DNA polymerase beta decreased by 43% at 5 days (P less than 0.01) and by 47% at 7 days (P less than 0.001). The activity of DNA polymerase gamma also decreased by 46% at 3 days (P less than 0.02) and by 78% at 7 days (P less than 0.01) after surgery. The amount of mRNA for DNA polymerase beta decreased in parallel with enzyme activity. Since the sensitivity to heat inactivation of testicular DNA polymerase beta was exactly the same as that from liver, the decrease in DNA polymerase beta activity may be, at least in part, due to reduced biosynthesis of enzyme protein. The morphological changes in cryptorchid testes suggested that the decrease in DNA polymerase beta and gamma activities might be related to the deleterious effects of elevated temperature on spermatogenesis.

Decrease of topoisomerase I activity in the testes of infertile men with varicocele.

Topoisomerase I, which is involved in DNA metabolism, was measured in the testes of 37 infertile men with varicocele, using a minute amount of testicular biopsy specimen. In 38% of infertile men topoisomerase I activity was lower than that of normal men. Forty-six percent of patients with high grade varicocele (grade III) showed reduced topoisomerase I activity, while only 26% of patients with low grade varicocele (grades I and II) showed reduced levels of topoisomerase I. Johnsen's score of the patients with low topoisomerase I activity was lower than that of patients with normal levels of topoisomerase I. The decrease of topoisomerase I activity may be correlated with at least some fraction of hypospermatogenesis associated with varicocele.

[Application of monoclonal antibodies to diagnosis and treatment of small cell lung cancers].

Recently we developed monoclonal antibodies against small cell lung cancer (SCLC) and applied them for diagnosis using histology and imaging, and treatment using SCLC transplanted into nude mice. One monoclonal antibody designated TFS-2 is cytotoxic to SCLC cells with complement. This antibody seems to be useful for purging tumor cells from the bone marrow in vitro when autologous bone marrow transplantation is applied. The other monoclonal antibody, TFS-4, has a high specificity to SCLC. This antibody was useful for diagnosis of SCLC using histology, and for the detection of metastatic tumor cells in the bone marrow. It was possible using imaging by radioisotope-labelled TFS-4 to reveal the localization of SCLC transplanted into nude mice. As for treatment with TFS-4, we injected 500 mu Ci of 131I-labeled TFS-4 into nude mice bearing SCLC, and recognized a reduction in tumor size of about 60%.

[A case of a complete paraplegic fathering a girl following artificial insemination after an intrathecal neostigmine injection].

A case of a 32-year-old paraplegic male who fathered a girl following artificial insemination after an intrathecal neostigmine injection is described. The patient became a complete paraplegic after injury to the spinal cord caused by a traffic accident in 1971, resulting in ejaculatory disturbance. In June, 1984 the first intrathecal neostigmine injection was tried and 2.7 ml of semen was collected and offered to artificial insemination. However, the first to ninth attempt did not result in fertilization. The tenth attempt was made in March 7, 1986, and 3 ml of semen presenting 240 X 10(6) spermatozoa per ml with 5% of motile sperms was collected. This resulted in pregnancy, and the patient's wife delivered a healthy girl weighing 2552 g by cesarean section on November 19, 1986. This is the fourth case of a paraplegic who fathered a child following artificial insemination after an intrathecal neostigmine injection in the Japanese literature.

[Male infertility with chromosomal abnormalities. III. 46, XYq-].

A deletion of the long arm of the Y chromosome (46,XYq-) was found in two cases with normal male habitus but with azoospermia. The first case was of a 28-year-old married man, an office worker, complaining of infertility. His height was 153 cm, weight 57 kg and distance of extended hand 152 cm. Both testes were 13 ml in size. The second case was a 32-year-old married man, a fireman, complaining of infertility. His height was 160 cm, weight 64 kg and distance of extended hand 161 cm. Both testes were 10 ml in size. In each case, the external genitalia showed a normal male type, but azoospermia was identified in semen analysis. The testicular biopsy specimens revealed spermatogenic arrest with thickening of basement membrane. Chromosomal analysis showed 46,XYq- karyotype. Basal serum levels of luteinizing hormone and follicle-stimulating hormone were slightly elevated and the serum testosterone level was within normal limits. Eight cases of 46,XYq- karyotype in the Japanese literature including our cases were reviewed.

[Experience of Elcitonin treatment in metastatic bony pain of urological tumors].

Fourteen cases of urogenital tumors (9 prostatic carcinomas, 4 renal cell carcinomas and 1 bladder carcinoma) which had bone metastases were treated with eel-calcitonin, Elcitonin injections for relief of bony pain. Forty mgs. of Elcitonin was injected intramuscularly, 2 to 3 times a week, to out-patients. Forty to 80 mgs. of Elcitonin was injected intramuscularly, daily to hospitalized patients. Relief of the pain was obtained in 71.4% of all patients (71.4% of out-patients and 71.4% of hospitalized patients) and especially in 88.9% of prostatic carcinoma patients. Hypercalcemia was seen in only one patient of renal cell carcinoma. It is considered that Elcitonin treatment is useful for relief of bony pain in the patients with bone metastases, with or without hypercalcemia.
