We investigated the pharmacokinetics of OK-432, au immunomodulator of streptococcus preparation which, is used in cancer patients for active nonspecific immunotherapy. First, OK-432 was labeled with 99mTechnetium in vitro. Four patients with malignancy were studied. By the method of scintigraphy using gamma camera, OK-432 administered intravenously was found to be distributed in the liver, lung and spleen, by the decreasing grade. When OK-432 was administered subcutaneously or intramuscularly in the buttocks, most of the radioactivity of 99mTechnetium remained locally at the injected site. These results suggested that OK-432 given intravenously was effectively phagocytized by cells of reticuloendothelial system (RES). Compared with other routes of administration, the intravenous route of OK-432 administration is thus considered more effective in order to stimulate RES, which is responsible for the first step of immune reaction.
The concentration of norethindrone in plasma samples from subjects receiving norethindrone (norethisterone) and lynestrenol orally was measured by high pressure liquid chromatography (HPLC). Norethindrone is a synthetic gestagen widely used in contraceptive formulations, and lynestrenol is also a synthetic gestagen which is metabolized to norethindrone in humans. But the evaluation of plasma norethindrone levels in subjects receiving lynestrenol has not yet been reported. After the administration of norethindrone, the peak level of norethindrone in the plasma was obtained within 2h, and the peak concentration in the plasma was about 3.5 ng/ml/mg norethindrone. During a period of 2-6hs after the administration of norethindrone, the half life of norethindrone in the plasma was approximately 1.8h, and during the period of 6-24hs, half life was variable. On the other hand, after the administration of lynestrenol, the peak level of norethindrone in the plasma was obtained within 4h, and the peak concentration of norethindrone was about 1.9 ng/ml/mg lynestrenol. During a period of 4-12hs, the half life of norethindrone was about 2.5h. The peak of the norethindrone level after the administration of lynestrenol was lower and appeared later than that after the administration of the same dose of norethindrone. Norethindrone in plasma in subjects receiving lynestrenol could be measured for a longer period than in those receiving the same dose of norethindrone. These results suggest that lynestrenol is stored in fat tissue and is slowly metabolized to norethindrone.
Alpha-fetoprotein (AFP) is present in the serum of normal healthy adults; an increase in level has been found as a rule in patients with diseases that can affect the liver. In the course of routine examination, we encountered a moderate increase in serum AFP in some patients with acute bleeding anemia, not accompanied by elevation of serum transaminase levels. We examined the relationship between anemia and serum AFP levels, using CIS kits for the assay. The least detectable concentration of below 0.25 ng/ml, which was two standard deviations outside the value obtained for the zero-dose response, was measurable by a two-step incubation: pre-incubation of serum and antiserum followed by addition of [125I]AFP and further incubation. The mean AFP level in 75 normal control sera was 2.4 1.6 ng/ml. There was a significant increase in the mean value of serum AFP levels in acutely anemic patients with no evidence of liver involvement. The increase was moderate, many of the patient samples being within the normal range. The reticulocyte counts did not correlate with the elevated AFP levels, indicating low significance of extra-hepatic AFP synthesis. A moderate increase in AFP production might occur in acutely anemic patients with no detectable hepatic cell damage.

Ten plasmids were isolated as covalently closed circular deoxyribonucleic acid from antibiotic-resistant thermophilic bacteria. Of the 10 plasmids tested, 2 could transform Bacillus subtilis, yielding resistance to specific antibiotics. Plasmid pTB20 (2.8 X 10(6) daltons, approximately 24 copies per chromosome) specifies resistance to tetracycline (Tcr), whereas pTB19 (17.2 X 10(6) daltons, approximately 1 copy per chromosome) renders the host resistant to both kanamycin and tetracycline (KMrTcr). Three plasmids were not self-transmissible. The restriction endonuclease cleavage maps of the two plasmids, pTB19 and pTB20, were constructed. pTB19 and pTB20, both of which were originally isolated from thermophilic bacilli, were tested for stability in B. subtilis. Digestion of pTB19 followed by ligation yielded deletion plasmids pTB512 (Kmr), pTB52 (Tcr), and pTB53 (KmrTcr). Determinants of Kmr, Tcr, and DNA replication were associated with EcoRI fragments R1b (4.2 X 10(6) daltons), R3 (2.8 X 10(6) daltons), and R1a (4.2 X 10(6) daltons), respectively. Restriction endonuclease cleavage maps of pTB51, pTB52, and pTB53 were constructed. Tetracycline resistance of pTB20 was confirmed to be in the EcoRI fragment (1.85 X 10(6) daltons).
Computed tomography (CT) was performed on phantoms to evaluate beam hardening effect in the CT measurement of bone mineral content of the vertebrae. It was found that the CT number of bone-simulating solutions was in linear relationship with the logarithm of exp(-zeta 2l) X 10(7). This exponential is a good estimate of the beam hardening effect in a vertebral study.
The purpose of the present study was to examine noninvasively the myocardial performance before and after hemodialysis in 40 patients of chronic renal failure. Using systolic time intervals, the following results were obtained. 1) Left ventricular ejection time was shortened significantly. The reduction of circulating blood volume estimated by the body weight change seemed to be responsible for this finding. 2) Pre-ejection period seemed to be a sensitive indicator of the left ventricular performance in hemodialysis. 3) The less the circulating blood volume, the less the suppression of cardiac performance. 4) Left ventricular function in patients without cardiomegaly was improved. 5) Left ventricular function tended to improve in the young age group. The present study demonstrated that the left ventricular performance tended to be suppressed in older age patients, in patients with cardiomegaly and in the cases with excessive removal of circulating blood volume during dialysis. The significance of noninvasively obtained left ventricular hemodynamic parameters in patients with hemodialysis was discussed.

Effects of 1,4-bis-[3-(3,4,5-trimethoxybenzoyloxy)-propyl]-perhydro-1,4-diazepine (dilazep), a powerful coronary dilator via adenosine potentiation, on regional blood flow during acute selective coronary hypotension were studied in the canine heart under anesthesia and thoracotomy. The proximal left circumflex coronary artery (LCX) was partially obstructed and mean LCX pressure distal to obstruction was reduced to 40-45 mmHg and two different doses of dilazep were given i.v. The left ventricular free wall and interventricular septum were divided into normally perfused, borderline ischemic, moderately ischemic and severely ischemic area according to the level of regional myocardial blood flow measured by tracer microspheres. 50 microgram dilazep/kg b.w. increased myocardial flow in the normally perfused and borderline ischemic areas without a significant alteration of blood flow to the moderately and severely ischemic areas. The fall of aortic pressure was more striking in dogs receiving 100 microgram dilazep/kg b.w. than in animals with 50 microgram dilazep. 100 microgram dilazep increased flow in the normally perfused area, but increased the number of segments in the moderately and severely ischemic area. These results indicate that an appropriate dose of dilazep causing a slight reduction of aortic pressure increased flow to the borderline ischemic as well as normally perfused myocardium in selective coronary hypotension.

We describe a method for determination of guanidino compounds in plasma and erythrocytes. The compounds are separated by liquid chromatography, the concentrations in the effluent being estimated fluorometrically (Trans. Am. Soc. Artif. Intern. Organs 24: 61, 1978). Guanidinoacetic acid, guanidinosuccinic acid, guanidinobutyric acid, guanidine, methylguanidine, taurocyamine, and arginine can be detected and quantitated in both plasma and erythrocytes from healthy individuals. The method was also applied to patients in chronic renal failure. Guanidinosuccinic acid, creatinine, guanidine, and methylguanidine are substantially increased in the plasma and erythrocytes of the uremic patient. Guanidino compounds can be effectively extracted from plasma with 0.5 volume of 300 g/L trichloroacetic acid, from erythrocytes with five volumes of 120 g/L trichloroacetic acid.

A composite plasmid pLS253 was constructed from pLS103 [carrying the Bacillus subtilis leucine genes on B. subtilis (natto) plasmid pLS28] and pHV14 [a recombinant plasmid composed of pBR322 and the staphylococcal R-plasmid pC194] employing BamHI endonuclease, T4 DNA ligase, and B. subtilis transformation. All the Leu+ Cmr transformants tested harbored not only pLS253 but also two smaller plasmids designated as pLS251 and pLS252. pLS253 DNA, when purified on an agarose gel, retained both Leu+ and Cmr transforming activities; however, in all the Leu+ Cmr transformants, the two smaller plasmids reappeared. pLS251 and pLS252 exhibited Leu+- or Cm4-transforming activity, respectively, and must have been derived from the pLS253 parent by an intramolecular recombination event, since the sum of the pLS251 and pLS252 DNAs represent the entire pLS253 genome. The recombination occurred between specific sites on the B. subtilis (natto) and Staphylococcus aureus plasmids. When the composite plasmid, pLS254, was constructed by BamHI cleavage of pLS251 and pLS252 followed by ligation, Leu+ Cmr transformants segregated two smaller plasmids which were indistinguishable from the original plasmids pLS103 and pHV14, respectively. They must have been derived from pLS254 through a reversal of the original recombination event. No intermolecular recombination between pLS251 and pLS252 DNA was detected. The recombination process was independent of recE function of the host cells, and its mechanism is discussed.
Relationships between Normotest, prothrombin time, albumin, gamma-globulin, GPT, and the hepatic fractional clearance of 198Au-colloids as a measure of effective hepatic blood flow were studied by correlation analysis, in 50 patients with liver diseases, including chronic hepatitis or cirrhosis. Simple correlation coefficients were highly significant between Normotest, prothrombin time, albumin, and the hepatic fractional clearance; however, not between GPT or gamma-globulin and the other parameters. Further correlation analysis revealed a highly significant partial correlation coefficient between Normotest and albumin and the hepatic clearance; however, not between Normotest and albumin. As effective hepatic blood flow is considered to be proportional to the liver parenchymal volume, the above results indicate that vitamin-K-dependent clotting factors, as well as albumin, reflect the liver cell mass.
A total of 30 patients with portal hypertension examined by computed tomography (CT) was reviewed for identification of portosystemic collaterals and other changes in the portal venous system. CT demonstrated esophageal varices in 18 of 25 patients shown to have varices by other methods. Mediastinal venous masses secondary to a dilated azygos system and paraesophageal veins were also recognized by CT. Other collaterals, such as gastrorenal, paraumbilical pathways, and caput medusa, could be seen and instances of splenic vein dilatation and portal vein aneurysm were encountered. CT in portal hypertension can be useful in establishing that mass lesions may in fact be dilated veins.
