[Long-term survivors of non-small cell lung cancer treated with non-surgical therapeutic modalities].

Of 225 patients with a non-small cell lung cancer treated with non-surgical therapeutic modalities, 21 patients (9.3%) survived for two years and more. The performance status and weight loss were important prognostic factors. There was no relation between response to treatment and long-term survival in patients with an adenocarcinoma. All two-year survivors with a squamous cell carcinoma achieved a complete response or a partial response with non-surgical treatment. It is considered that improving response rate with a combined non-surgical modality treatment, including a combination of chemotherapy and radiotherapy is important to obtain a long-term survival for those with a non-small cell lung cancer, excepting those determined to have an adenocarcinoma.

[Phase II study of ifosfamide and vindesine combination of non-small cell lung cancer in elderly patients and patients with reduced renal function].

A pilot phase II study of a ifosfamide and vindesine combination of non-small cell lung cancer was conducted in the elderly patients and patients with reduced renal function. The combination consisted of ifosfamide, 2.3 g/m2, on day 1 through 3, and vindesine, 3 mg/m2, on day 1 and 8. Cycles were repeated at 4-week intervals. The dose of ifosfamide and vindesine was reduced in the elderly. Of 24 evaluable patients, one had a complete response and 8 had partial response, with an overall response rate of 37.5%. The projected median response duration was 6.8 months and the median survival for all patients was 15.1 months. The toxicity was tolerable with moderate myelosuppression. These results indicate that this combination chemotherapy is safe and effective for non-small cell lung cancer in the elderly patients and patients with reduced renal function and warrants further clinical trials.

[Anastomosis of the superficial temporal artery to the middle cerebral artery for occlusive disease of the bilateral internal carotid arteries with dementia].

Two cases are reported of occlusive disease of the bilateral internal carotid arteries with dementia in which the anastomosis of the superficial temporal artery to the middle cerebral artery (STA-MCA bypass) resulted in marked improvement in clinical aspects. One patient was a 29-year-old male who complained of transient weakness of the extremities and memory impairment. Computerized tomography (CT) scans showed multiple small infarctions, while cerebral angiography demonstrated findings of Moya-like disease. The cognitive function tests were subnormal and the study of cerebral blood flow (CBF study) showed diffuse low flow in both hemispheres. Based on his clinical symptoms and CBF study, the STA-MCA bypass was performed on both sides in two stages without complications. Postoperatively, his clinical symptoms and cognitive function improved gradually, in accordance with increased CBF in both hemispheres. Six months after the operation, cognitive function tests were within normal limits. Another patient was a 61-year-old hypertensive male who complained of motor weakness, impairment of memory and urinary incontinence. CT scans showed multiple small infarctions, while cerebral angiography revealed occlusion of both internal carotid arteries at the cervical portion. The cognitive function was at the pre-dementia level, and CBF study revealed diffuse low flow in both hemispheres. Based on the clinical symptoms and CBF study, the STA-MCA bypass was performed on both sides in two stages. Postoperatively, clinical symptoms and cognitive function markedly improved. From our results, the diagnosis of vascular dementia, and indications for the use of STA-MCA bypass in this category of patients are discussed.

Effect of pulsed ultrasound versus placebo on muscle soreness perception and muscular performance.

The purpose of this study was to compare the analgesic effect of pulsating ultrasound treatment and placebo on delayed onset of muscle soreness produced by an eccentric exercise bout. In addition, the effect of pulsed ultrasound on muscular performance following an eccentric exercise bout was studied. Eighteen untrained subjects were randomly assigned to: 1) ultrasound (A) [N = 6] over the areas of concentrated muscle soreness, i.e. proximal vastus lateralis and distal vastus medialis; 2) placebo ultrasound (B) [N = 6]; and 3) no therapeutic intervention (C) [N = 6]. Baseline data were recorded for maximum isometric knee extension contraction (MVC), maximum knee extension torque (MT), knee extension work (W), and soreness perception (SP). All values were subsequently reassessed 24 and 48 hours after intense muscular activity. Immediately following the 24 hour reassessment the A group received ultrasound treatment, the B group received placebo ultrasound, while the C group received no treatment. Percent deviation from baseline of SP, MVC, MT and W were significantly less for A than B and C (p less than 0.05) at 48 hours post muscle soreness bout. These data indicate that pulsed ultrasound accelerates restoration of normal muscle performance, and thus is effective in decreasing delayed onset of muscle soreness. The mechanism for decreasing soreness perception in the muscle is unknown, but may be related to decreasing intramuscular pressure and/or decreasing the inflammatory response.

In vivo T1 measurements of canine brain for deep tissue temperature measurements with MRI.

The possibility of deep tissue temperature imaging with a magnetic resonance (MR) imager was studied in 30 mongrel dogs. The coronary view of the brain of a dog anesthetized with muscle relaxant and pentobarbital and ventilated automatically with air was taken with a special head coil by a 0.1 Tesla MR imager, Asahi Mark-J, driven by a spin warp pulse sequence. T1s in the imaging of the right and left white matter, grey matter, thalamus and caudate nucleus were measured with rectal temperature in group I (10 dogs) for a control study every 30 min until 120 min without intervention, in group II (10 dogs) for hyperthermia with extracorporeal bypass method, before heating, at 41 and 42 degrees C, in group III (10 dogs) for hypothermia, before cooling, at 35 and 30 degrees C and after rewarming, respectively. In groups I, II and III, the T1s at all sites except the white matter at 42 degrees C did not change significantly at any rectal temperature. T1 prolongation in the white matter at 42 degrees C may be due to edema, probably because of biological reaction to hyperthermia. The present study suggests that deep tissue temperature imaging in vivo by T1s measured with the MR imager may be impossible in spite of obvious evidence of T1 dependance on temperature in theoretical, in vitro and ex vivo studies already achieved.

[Curability of multidisciplinary treatment including combined hyperthermia, radiotherapy and chemotherapy].

Reported are the results in 311 patients who received a multidisciplinary treatment for their cancers, said treatment combining hyperthermia, radiotherapy, and chemtherapy. Of his number, 266 patients were in the advance stage of their disease while another 42 still had localized cancers. According to the criteria of Koyama and Saitoh for evaluating solid tumors that had received chemotherapy, of 298 evaluable patients, 64 showed a complete response (CR), 94 showed a partial response (PR), 89 showed no change (NC), and 57 showed a progressive disease (PD), for an overall response of 53.0% (158 cases). A multidisciplinary treatment combining radiotherapy and hyperthermochemotherapy was used for cases involving localized tumors of the vulva, the skin, the thyroid gland, the prostate gland, the uterus, the rectum, the soft tissue bone, and the ovary, though our treatment regimen could not be continued in cases of tumors involving the stomach, the esophagus, the lung, the mediastinum, the liver, and the pancreas. Our data has shown that a high overall rate of curability was achieved in cases involving the stomach, the bladder, the blood, the skin, the head, and neck, and the esophagus.

[Experience in the use of Kyosera Injection Port as a kind of implantable reservoir (the second report)].

A total of 30 pieces of Kyosera Injection Port, as a kind of implantable reservoir, was used in 29 cases of liver tumor for an intermittent repeated chemotherapy. Of these 27 cases were actually used between Aug. 1st, 1986 and Aug. 31st, 1989. In this paper we studied the safety of the reservoir. The observation period averaged 259.4 days, with the highest of 760 days and the lowest of 29 days. The injection times averaged 12.7 times, with the maximum of 40 times and the minimum of one. The reservoir was placed under the laparotomy, and the cases which underwent cholecystectomy were 93.1%. 16 cases were treated with intraarterial infusion, and 14 with intraportal infusion. We investigated the complication of Kyosera Injection Port in 27 cases. As a result, there were no complication such as catheter occlusion and infection via catheter. But of 27 cases, 8 cases were recognized a complication, and the rate was 29.6%. Lumbago was noted during infusion chemotherapy in three cases, and on the other hand, there were steatolysis and necrosis of the tissue surrounding the implantation site, a movement of port, a catheter out of vessel, a leakage of the drug, and intraabdominal bleeding after taking off the catheter. But of 8 complication cases, only two cases were not able to inject drug. As a result, the rate of complication was 70.4% in over all. On the other hand, of 27 cases, there were 5 cases which recognized the complication caused by reservoir in itself. The safety rate was 81.5%. In such a circumstance this reservoir was very safe and seemed suitable to the implantable reservoir.

[Two cases of megaureter in children].

We reported 2 cases of megaureter in children. The courses of renal function and ureteral dilatation in these patients were observed by urography and renoscintigraphy to determine the timing of surgery. These two patients with unilateral megaureter were treated by reconstructive operation involving excision of the stenotic portion of the ureter, tapering of the dilated ureter and re-implantation. The courses after reconstruction were excellent after 5 years. Recently, active surgery has been recommended for megaureter together with identification of the cause. Urograms and diuretic renoscintigrams were useful for determining the timing of surgery in our cases.

[Experimental study of arterial damage induced by anti-cancer drug infusion].

In order to clarify the cause of arterial changes after intra-arterial infusion of anti-cancer drugs (AI-AD) experiments were made on the arteries of rabbits. Histopathologic sections, 7 days after AI-AD revealed endothelial damage characterized by pyknosis, hyalinization with edematous change of the intimal layer. Proliferation of the endothelial cells was also observed. The cause of narrowing or occlusion of the arteries was considered to occur with thrombus formation surrounded by the proliferated endothelial cells and this suggested that thrombolytic agents would be effective to prevent these arterial changes.

Reinvestigation of deoxyribonucleoside phosphorothioites: synthesis and properties of deoxyribonucleoside-3' dimethyl phosphites.

Further investigation of the synthesis of deoxyribonucleoside-3' (t-butyl) O-(2-cyanoethyl) phosphorothioites as monomer building units for the phosphorothioite approach has led us to conclude that internucleotidic bond formation proceeded via bis(deoxyribonucleoside-3') (2-cyanoethyl) phosphite intermediates, which proved to be activated by iodine, rather than the mechanism previously reported. In connection with this study, deoxyribonucleoside-3' dimethyl phosphites were synthesized and detailed properties of them are also described.

[Experimental study of embolo-hyperthermia for treatment of liver tumor--induction heating to ferromagnetic particles injected into tumor tissue].

Hepatic arterial embolization was performed on VX2 liver tumor of rabbit with subsequent induction heating to the tumor. Prior to the heating, iron particle suspension was injected into tumor tissue as a target of induction heating (500 KHZ, 9 KW). The temperatures of the tumor and liver parenchyma were measured with fluoroptic thermometer. Elevation of the tumor temperature during initial heating for 6 minutes were well correlated to the dose of iron particles injected; 1.4 degrees C with 1 g, 3.0 degrees C with 2 g, and 4.9 degrees C with 3 g. The temperature of liver parenchyma were below 39 degrees C even with 3 g injection.

[Effects of neuromedin B and neuromedin C on insulin release from isolated perfused rat pancreas].

Neuromedin B and neuromedin C are novel decapeptides that have recently been isolated from porcine spinal cord and canine intestinal mucosa. We have studied the effects of neuromedin B and neuromedin C on insulin release from the isolated perfused rat pancreas. The effect of neuromedin B was detectable at a concentration of 10mM, and that of neuromedin C was detectable at a concentration of 1nM. Further increase in the concentration of neuromedin B and neuromedin C resulted in dose-dependent increases in insulin secretion. The effectiveness of neuromedin B and neuromedin C as insulinotropic agents depended on the glucose concentration; both were more effective at a higher concentration of glucose. However, insulinotropic effects of these peptides in the presence of 8.3mM glucose were limited to the first 3 min of a 20-min perfusion. These results, coupled with a recent study demonstrating bombesin-like immunoreactivity in nerves in the pancreas, suggest that neuromedin B and neuromedin C exert a direct local action on insulin secretion in the pancreas.

Thrombus formation on the aorta injured by angioplasty and its prevention with dilazep in atherosclerotic rabbits.

To estimate the effect of dilazep in preventing restenosis after a transluminal angioplasty, we have attempted an animal experiment in which the efficacy of drug was tested on an angioplastic injury superimposed on to atherosclerotic lesions. Using a modification of Block's method, atherosclerotic lesions first were made in the rabbit aorta by an initial ablation of the endothelium, followed by successive cholesterol feedings. Then, for our second step, an angioplastic injury was inflicted on the atherosclerotic lesions. The thirty two rabbits used were divided into 3 groups: those given dilazep (100 micrograms/kg i.v.), those given dipyridamole (100 micrograms/kg i.v.) and a control group that was given the same volume of 0.9% saline. The angioplastic area in which thrombi developed was semiquantitatively measured and compared among all three groups. The mean value of the thrombus area in the dilazep group was 60% smaller and the distribution pattern of the thrombus by size was found to be composed of smaller thrombi than those of the control group. Dipyridamole showed the same trend as did dilazep, but less effectively. Our animal model and semiquantitative evaluation method that we employed were found useful from the view point of an easy applicable and inexpensive methodology. Our results point towards the possible clinical use of dilazep in the future, so as to prevent thrombus formation, which seems to cause the restenosis phenomenon that often occurs in patients who have undergone a coronary angioplasty.

Single-crystal EPR study of novel azide complex of cyanogen bromide-modified myoglobin. Influence of specific modification of the heme distal histidyl residue on the ligand-binding structure.

Stable azide complex of cyanogen bromide-modified met-myoglobin (metMb) was prepared and crystallized. The principal values and eigen vectors of g-tensor were determined by single-crystal EPR spectroscopy at 77 K: gxx = 1.50, gyy = 2.32, and gzz = 2.91. These g values were similar to those of tetrazole derivative rather than azide derivative of native metMbs, suggesting that tetrazole derivative might be formed from N-cyanoimidazole of distal histidyl residue via nucleophilic attack of azide ion by 1,3-dipolar cycloaddition reaction. The orientation of the maximal g value (gzz) of the novel product was found to deviate about 13 degrees from the heme normal of native aquometMb. Thus, the orientation of the heme plane might be altered in passing from native metMb to cyanogen bromide-mediated metmyoglobin. The present EPR results demonstrated that the modification of the histidyl residue at the heme distal side causes the changes in the stereochemical and electronic natures of the ligand binding to the heme.

Effect of a new cholinergic agonist, aclatonium napadisilate, on exocrine and endocrine rat pancreas.

The effect of aclatonium napadisilate, a choline sulfonate derivative, on exocrine and endocrine pancreatic functions was compared with that of carbamylcholine in both isolated pancreatic acini and the isolated perfused pancreas of rats. In the isolated acini, aclatonium napadisilate and carbamylcholine stimulated amylase release. While the relative efficacy of aclatonium napadisilate was the same as that of carbamylcholine, aclatonium napadisilate was about 20-fold less potent. In the isolated perfused pancreas, 0.1 microM or higher concentrations of aclatonium napadisilate elicited a significant insulin release in the presence of 8.3 mM glucose, whereas an appreciable increase in pancreatic exocrine secretion was obtained with a 10 times higher concentration (1.0 microM). In contrast, carbamylcholine did not stimulate insulin release at a dose (0.1 microM) that stimulated pancreatic exocrine secretion. The insulin-releasing effect of aclatonium napadisilate depended on the glucose concentration. These stimulatory effects of aclatonium napadisilate on endocrine and exocrine pancreatic secretion were inhibited by the muscarinic receptor antagonist pirenzepine but were not affected by the cholecystokinin receptor antagonist proglumide. These results indicate that aclatonium napadisilate stimulates both endocrine and exocrine pancreatic secretion via muscarinic receptors and that its action on B cells is more potent than on the exocrine pancreas.

Loxiglumide. A new proglumide analog with potent cholecystokinin antagonistic activity in the rat pancreas.

D,L-4-(3,4-dichlorobenzoylamino)-5-(N-3-methoxypropyl-pentylami no)-5- oxopentanoic acid (CR 1505; loxiglumide) is a newly developed analog of proglumide. We examined the inhibitory effects of loxiglumide on pancreatic exocrine function in the isolated pancreatic acini and the isolated perfused pancreata of rats. Loxiglumide inhibited cholecystokinin octapeptide (CCK-8)-stimulated amylase release and, similarly, binding of [125I]CCK-8 to isolated rat pancreatic acini. Loxiglumide was about 3000 times more potent than the reference substance proglumide, but was about 1000 times less potent than L-364,718, another new CCK antagonist having benzodiazepine ring, in inhibiting CCK-8-stimulated amylase release. The inhibitory effect of loxiglumide displayed competitive kinetics and was specific for CCK in that the effects of other receptor secretagogues or agents bypassing receptors were not altered. The inhibitory effect of loxiglumide was fully reversible in isolated acini. However, the pancreata perfused with 10 microM loxiglumide for 20 min did not respond to CCK-8 for more than 20 min even after the removal of loxiglumide infusion. In contrast, an immediate increase in pancreatic exocrine secretion was observed after proglumide removal. Loxiglumide appeared to be bound to the receptors on acinar cells in a slowly dissociating state. These results indicate that loxiglumide acts as a potent, competitive, and specific CCK antagonist on the exocrine pancreas and, because of its prolonged inhibitory action, may be useful as a therapeutic agent in pancreatic disease.
