[Study on digestibility and energy availability of daily food intake (Part 4. Shiitake mushroom)].

The digestibilities of nutrients and energy in Shiitke mushrooms (Lentinus edodes) was measured in healthy adult males. A test diet was given for 5 days after 5 days on a basal diet which supplied 40g/day of protein and 45kcal/kg/day of energy. The test diet was identical with the basal diet except that shiitake was incorporated. Digestibility was calculated by the difference of fecal output between the test period and the basal period. The results obtained were follows: 1. Shiitake 60g/day intake level Digestibility of protein was 69.3 +/- 11.2%. Digestibility of fat was 66.6 +/- 24.9%. Digestibility of carbohydrate was 55.7 +/- 10.5%. The ratio of the total available energy to intake energy (Net Energy Availability) was 55.4 +/- 15.4%. 2. Shiitake 40g/day intake level Digestibility of protein was 85.5 +/- 23.8%. Digestibility of fat was 70.0 +/- 90.1%. Digestibility of carbohydrate was 65.8 +/- 13.1%. The ratio of the total available energy to intake energy (Net Energy Availability) was 67.4 +/- 25.9%.

Bombesin and bradykinin increase inositol phosphates and cytosolic free Ca2+, and stimulate DNA synthesis in human endometrial stromal cells.

The present studies were carried out to investigate the effect of several growth factors on human endometrial stromal cells. In human endometrial stromal cells, bombesin and bradykinin provoked an increase in intracellular free Ca2+ and in labelled inositol phosphates when pre-incubated with [3H]myoinositol. Some or possibly all of the initial increase in intracellular free Ca2+ represented a mobilization of Ca2+ from intracellular stores and the second phase of the response depended on Ca2+ influx from the extracellular medium. [3H]Thymidine was added to human cultured endometrial stromal cells with bombesin, bradykinin, epidermal growth factor (EGF), prostaglandin F2 alpha, vasopressin and platelet-derived growth factor. Bombesin, bradykinin and EGF stimulated the incorporation of [3H]thymidine into DNA in quiescent cells. In conclusion, bombesin and bradykinin are growth factors which activate phospholipase C in human endometrial stromal cells, while EGF stimulates DNA synthesis without the activation of phospholipase C.

Phase II study of ifosfamide, cisplatin, and vindesine combination in advanced non-small cell lung cancer.

Twenty-seven previously untreated patients with unresectable non-small cell lung cancer were treated with a 3-drug combination of ifosfamide, cisplatin, and vindesine as a phase II study. Patients received ifosfamide, 1.3g/m2, on days 1 to 5; cisplatin, 20mg/m2, on days 1 to 5; and vindesine, 3mg/m2, on days 1 and 8; with a sufficient parenteral hydration. Courses were repeated every 4 weeks. Twenty males and seven females with a median age of 61 years were treated and fully evaluated. Five patients had stage IIIA, seven had stage IIIB, and 15 had stage IV disease. One patient with adenocarcinoma achieved a complete response and 16 achieved a partial response, for an overall response rate of 63% (95% confidence limit: 45% to 81%). The median duration of response was 34 weeks (range: 9 to 52 weeks). The median survival time was 58 weeks for patients with IIIA/B disease, and 33 weeks for those with IV disease. The major toxicity was myelosuppression, however, it was generally well-tolerated. These results indicate that the 3-drug combination is active against non-small cell lung cancer and warrants further clinical trials.

Brain metastasis of testicular tumor with massive hemorrhage--report of two cases.

The authors report two cases of brain metastasis from testicular tumor with massive, sudden intratumoral hemorrhage. In both cases, the hemorrhage occurred during the 1st admission day and carried a high risk of fatality. Early, aggressive surgical removal is advisable before general deterioration. Postoperative chemotherapy with an agent different from the one applied to primary lesion is also recommended because of drug tolerance.

[Double cancer in small cell lung cancer patients treated with intensive chemotherapy with or without radiation therapy].

Development of double cancer was evaluated in 311 small cell lung cancer patients who had received intensive chemotherapy with or without radiotherapy. Of those, 10 patients (3.2%) developed a second malignancy:stomach cancer in four, non-small cell lung cancer in three, acute myelogenous leukemia in two, and liver cancer in one. The cumulative risk for the development of double cancer was 1.0% at 1-year, 17.0% at 3-years, and 100% at 8.1 years. The relative risk for the development of double cancer calculated by person-year method utilizing age and sex adjusted cancer incidence in Japan was 2.96-fold (p less than 0.01). The risk of non-small cell lung cancer (6.65-fold) and acute myelogenous leukemia (54.9-fold) was particularly high. Of 21 patients who survived disease-free for more than 2 years, 8 patients died; four patients (50%) died of second malignancy, two died of infectious disease, and only two patients died from recurrent small cell lung cancer. These results indicate that a cautious follow-up program for the detection of double cancer is indicated in patients surviving small cell lung cancer.

[Neoadjuvant chemotherapy of head and neck cancer].

Chemotherapy for head and neck cancer has made great progress after CDDP was introduced at the clinical level. Chemotherapy prior to regional treatment (neoadjuvant chemotherapy) has become a popular approach for incorporating chemotherapy into multimodality treatment. Numerous studies support the general observation that response rates, especially the rates for complete response, are higher in patients who have not had prior regional treatment. The goals of this treatment are 1) to increase the effectiveness of local treatment (surgery and radiation therapy) while obtaining superior disease-free survival, and 2) to prevent metastases by eradicating micrometastatic lesions. Despite the excellent response rates after reported for regimens with CDDP, randomized studies have failed to prove statistically significant superior disease-free survival. The reason for these negative results is considered to be the patient's compliance and difficult of obtaining sufficient numbers of patients in each kind of primary sites, because this type of cancer is relatively infrequent. More study of neoadjuvant chemotherapy with high quality control is needed. Some trials with neoadjuvant chemotherapy to preserve the function of larynx or hypopharynx are promising new strategies. Concurrent chemo-radiotherapy using CDDP was proved to potentiate radiotherapy and showed a good response rate. Adjuvant chemotherapy after surgery or radiotherapy is also expected to prolong disease-free survival when it is incorporated into multimodality treatment.

[A case of colon cancer with severe anal bleeding caused in a preoperative chemotherapy].

UFT was orally administered at a dose of 600 mg/day as a preoperative chemotherapy for a 44-year old male with colon cancer. On the seventh day of the administration, severe anal bleeding occurred, and an emergent colectomy was performed. The macroscopic findings of the resected tumor were different from the first endoscopic picture, and the histopathological findings showed the significant effects of preoperative chemotherapy.

Pigmented paravenous retinochoroidal atrophy in a 68-year-old man.

A 68-year-old man with patchy paravenous zones of retinal degeneration with bone spicule pigmentation OU was diagnosed as having pigmented paravenous retinochoroidal atrophy. We reviewed the literature and found 71 previously reported cases of this condition. Most of the affected individuals were young adults. By comparison, our patient was afflicted at a considerably older age.

HTLV-1 Tax has distinct but overlapping domains for transcriptional activation and for enhancer specificity.

Tax1 of human T-cell leukemia virus type 1 (HTLV-1) activates viral transcription dependent upon three 21-bp enhancer elements in the long terminal repeat. Difficulties in detecting any association of Tax1 with the viral enhancer have hampered elucidation of the molecular mechanisms of Tax1-mediated transcriptional activation. By constructing a fusion protein with the heterologous DNA-binding domain of yeast GAL4, Tax1 was shown to be a potent transcriptional activator dependent on the presence of GAL4-binding sites. Deletions of the Tax1 portion of the fusion protein revealed that almost the entire region of Tax1 (amino acids 2-337) is required for activation, and the activity correlated well with that of the viral enhancer. The GAL/Tax1 mutant lacking 41 residues of the C-terminus of Tax1, GAL/Tax1(2-312), was inactive for the viral enhancer, but activity was recovered by adding the heterologous activation domain of herpes simplex virus VP16. These results indicate that Tax1 has two distinct but overlapping functional domains for transcriptional activation and for enhancer specificity. Thus, Tax1 is thought to be a transcription factor acting in the enhancer complex rather than as a catalytic or allosteric modifier of pre-existing cellular transcription factors.

[The effects of lowering of blood pressure on pain sensitivity in spontaneously hypertensive rats].

It has been reported that elevation of blood pressure produces a reduction in pain sensitivity. This study was designed to clarify the correlation in spontaneously hypertensive rats (SHR) between fall in blood pressure and pain sensitivity. In seven-weeks-old male SHR, the angiotensin converting inhibitor delapril (10 mg/kg/day) or calcium antagonist nifedipine (3 mg/kg/day) was administered orally every day for 8 weeks. Systolic blood pressure (SBP) pretreatment was significantly higher in the SHR than in normotensive Wistar-Kyoto (WKY) rats and pain sensitivity measured with the hot plate method was significantly lower in the SHR than in the WKY rats. Administration of both drugs produced a significant suppression of elevation of SBP, and produced a significant elevation of pain sensitivity. Furthermore, at 8 weeks after drug administration, urinary norepinephrine (UNE) significantly decreased and plasma beta-endorphin (beta-end) significantly increased. A significant correlation was noted between pain sensitivity and SBP and also between pain sensitivity and UNE. Of these, pain sensitivity was the more closely correlated to degree of change in UNE than to degree of change in SBP. It appears that elevation of pain sensitivity is due to suppression of the sympathetic nervous system by antihypertensive drugs, but not to elevation of beta-end levels. These data suggest that a fall in blood pressure through administration of delapril or nifedipine reverses decrease in pain sensitivity in SHR and that decrease in sympathetic tone plays an important role in the restoration of levels of sensitivity to pain.

HTLV-1 Tax induces expression of various immediate early serum responsive genes.

Human T-cell leukemia virus type 1 (HTLV-1) is an etiological agent of adult T-cell leukemia (ATL). We showed here by mobility-shift assay that T-cell lines transformed with the virus contained high levels of AP-1 activities. Consistent with this result, these cell lines expressed increased levels of mRNAs encoding the AP-1 proteins, c-Fos, Fra-1, c-Jun, JunB, and JunD. Previously, transcription of the c-fos gene has been reported to be transactivated by the viral transcription factor, Tax1. By using the human T-cell line (JPX-9), in which expression of the Tax1 is inducible, we showed that expression of mRNAs for Fra-1, c-Jun, and JunD was also transactivated by Tax1. Moreover, Tax1 activated expression of two other transcription factors having zinc finger motifs, Egr-1 and Egr-2, in the same cells. The Tax1-inducible transcription factors identified here are encoded by the members of immediate early genes under the control of growth signals. Thus, Tax1 was suggested to replace growth signals, at least in part, by this mechanism.

[Effects of mannitol on the function of red blood cells].

The effects of mannitol on the function of red blood cell were studied in 12 neurosurgical patients for 4 hours after intravenous administration of 2 g.kg-1 mannitol taking 30 min. After the administration of mannitol, osmotic pressure, Na and K in serum and of red blood cell were all altered. P50, 2, 3-DPG rose gradually and red blood cell deformity improved significantly. These results indicate that mannitol can shift oxyhemoglobin dissociation curve rightward, improve red blood cell deformity, and increase tissue oxygenation significantly. Mannitol administration not only decreases intracranial pressure but also improves peripheral circulation and oxygen transport during brain resuscitation.

[Study of pre-operative chemotherapeutic methods for colonic cancer--the concentration in blood and tissues after pre-operative administration of UFT].

As one of the fundamental studies of the pre-operative chemotherapeutic methods in 22 cases with colonic cancer, we studied a progress of the concentration in blood and concentration in tissues including those in the portal vein after administration of UFT. The concentration of 5-FU in blood reached to the peak at 2 hours after administration of UFT and then decreased rapidly, while it was maintained for a long term in comparing with that of 5-FU (oral remedies; tablet and dry syrup). This fact suggested that UFT can be expected for metastatic inhibition and prevention of the recurrence because it showed an effect in free-cancerous cells before and during operation. The anti-tumorous effect was also expected because the concentration of 5-FU in cancerous tissues was significantly higher than that in non-cancerous region. In our study in this time, down staging was considered by pre-operative chemotherapeutic method of colonic cancer in the future because concentration of 5-FU was also highly obtained in metastatic lymph nodes. From the above-mentioned results, pre-operative chemotherapeutic method using UFT was expected as a part of the intensive therapies.

[CABG in 2 patients with severe left ventricular dysfunction].

Coronary artery bypass grafting was performed in two patients with left ventricular dysfunction (EF 16%). In both cases they had chest pain at rest and ischemic myocardium were detected by the SPECT. We considered them candidate for the operation regardless of LV dysfunction. They were eventful in the postoperative course. One case was discharged and is free from chest pain now, but another unfortunately died for multiple organ failure.

[A study of MRI diagnosis for rheumatoid arthritis in the knee joint--imaging and pathologic correlation].

We performed MR (magnetic resonance) imaging of 25 knee joints in 20 patients with rheumatoid arthritis (RA) before synovectomy and the findings were compared with the pathological specimens. When based on the findings on T2-weighted images of the examined joints, the degree of joint involvement by RA could be divided into four stages: stage 1 in which only joint effusion was present; stage 2 in which synovial thickening, lower in signal intensity than joint effusion, was present; stage 3 in which cartilage destruction was present; stage 4 in which apparent bone destruction was present. There were eleven joints which appeared unremarkable on conventional X-ray films but three of them were at stage 2 and six at stage 3. In conclusion, MR imaging is of help in diagnosis of early involvement of the knee joints by RA.

[Application of an angiographic catheter with side holes to intra-arterial drug infusion--improvement of drug distribution by occlusion of the catheter tip].

To improve distribution of drug by intraarterial infusion a new catheter system with sideholes, tip of which was able to be occluded by an embolus, was developed. Inner diameter of the catheter was 0.038 inch. Tip of the catheter was so smoothly tapered to 0.025 inch in inner diameter that it could be occluded by an embolus of 0.038 inch in diameter. After occlusion of the tip, drug flowed out rectangly to catheter axis from only sideholes, resulting that drug flowed out was mixtured by blood stream to make a uniform distribution of it.

Properties and repeated use of a reversibly soluble-insoluble yeast lytic enzyme.

A yeast lytic enzyme was covalently immobilized on an enteric coating polymer, Eudragit S, that is reversibly soluble and insoluble (S-IS) depending on the pH of the reaction medium. The yeast lytic enzyme immobilized on Eudragit S (Y-E) showed a sharp response of solubility to slight changes in pH without decrease in enzymatic activity. The specific activity per amount of enzyme protein of Y-E for dry yeast cells was about two-thirds that of the native enzyme. In both lysis reactions of dry and pressed baker's yeast cells, changing the pH of the reaction medium from 7.0 to 4.8 at an appropriate interval allows the insoluble Y-E and the reaction products (soluble protein for dry yeast cells and invertase and soluble protein for pressed baker's yeast cells) to be repeatedly separated. The reaction method using a reversible S-IS enzyme is a promising procedure for repeated use of the enzyme in a heterogeneous reaction system containing yeast cells as a substrate.
