The luminal aspect of intrarenal arteries and veins in the rat as revealed by scanning electron microscopy.

The luminal aspect of intrarenal arteries and veins in the rat has been investigated by scanning electron microscopy (SEM). The endothelium of the intrarenal arteries consists of spindle-shaped cells and forms longitudinally running ridges which correlate with the folding pattern of the underlying internal elastic lamina. Intraarterial "cushions" were found at the origins of afferent arterioles from arcuate arteries and along the entire course of interlobular arteries. The intrarenal veins are made up of a thin, extensively fenestrated epithelium equal to that of peritubular capillaries. The outer aspect of the endothelium contacts adjacent tubules as closely as the capillaries proper. Thereby, the luminal aspect of the veins exhibits a striking "tubule relief" created by the underlying tubules. This wall structure of the intrarenal veins suggest that diameter and shape of the veins are probably highly dependent on the surrounding interstitial pressure.

Use of i.m. ranitidine for the prophylaxis of aspiration pneumonitis in obstetrics.

Twenty patients who underwent elective Caesarean section received ranitidine 150 mg by mouth 8-14 h, and 50 mg i.m. 90 min, before surgery. Intraoperative gastric aspiration resulted in contents with a pH greater than 2.5 and volume less than 25 ml in all patients (mean pH 6.5 (SD 0.8); mean volume 9.0 (SD 7.2) ml). Sixty patients in labour, who received ranitidine 50 mg i.m. 6-hourly, underwent emergency surgery. Half of this group received, in addition, a single preinduction dose of either 15 or 30 ml of sodium citrate 0.3 mol litre-1. A further 30 patients who remained unmedicated during labour and required emergency surgery received a preinduction dose of 15 or 30 ml of sodium citrate 0.3 mol litre-1 alone. Ranitidine medication resulted in a mean aspirated gastric volume of 31.4 (26.6) ml and pH of 5.3 (2.1); five of 30 patients had a pH less than 2.5. The addition of sodium citrate 0.3 mol litre-1 resulted in gastric pH greater than 2.5 in all patients and a mean gastric volume of 43.2 (38.3) ml. The group who received only sodium citrate 0.3 mol litre-1 had a mean pH of 5.3 (1.1) and a mean volume 122.7 (98.2) ml.

Survey of the practice of epidural analgesia in a regional sample of obstetric units.

An investigation into practices of the epidural services in a regional sample of obstetric units was undertaken following recent reported disasters associated with epidural analgesia for labour pain. A questionnaire was completed by all 22 obstetric units in the region, which included six teaching, 14 district and two independent centres. In three units the epidural service was shared with the obstetricians. A continuous anaesthetic presence was provided in 16 of 22 units. There was considerable variation in the attendance upon epidural patients by the anaesthetists. Instructions to midwives for top-ups and subsequent care of patients followed no uniform pattern. Midwife in-service training in the initial management of serious epidural complications and in cardiopulmonary resuscitation was inconsistent. In some units, the avoidance of aortocaval compression was not emphasised in the management of serious complications such as severe maternal hypotension, total spinal blockade or cardiac arrest of the parturient. The results obtained in this survey suggest that there is a need to review the requirements in the provision of obstetric epidural services and consideration should be given to the establishment of a generally accepted standard of practice.

Platelet aggregation, rheological parameters and blood glucose profiles in diabetic children (type I) treated with human and porcine insulin.

In a three-year bicentric cross-sectional investigation on type I diabetic children between six and eighteen years of age, blood sugar profiles and spontaneous thrombocyte aggregation were assessed besides anamnestic and clinical data. In the children treated with human insulin raised spontaneous thrombocyte aggregation was significantly more frequent than in those treated with porcine insulin. At the same time blood sugar fluctuation from day to day measured between seven and nine a.m. tended to be raised in the children treated with human insulin; the fluctuation in the diurnal profile measured for fourteen days was indeed very much greater. Since the two groups were comparable as to sex distribution, age, duration of disease, quality of compensation, application and dose of insulin, the greater fluctuation of blood sugar in the children treated with human insulin appears to be the cause for the raised spontaneous thrombocyte aggregation.

[Rotor syndrome: relevance of the determination of coproporphyrin isomers in the urine in comparison with intrahepatic (alcohol-induced) cholestasis].

Porphyrin isomer examinations have been performed in two patients with Rotor syndrome (RS), one patient with Gilbert-Meulengracht syndrome and 12 patients with alcohol toxic cholestasis. Under both conditions, cholestasis and RS, total urinary coproporphyrin excretion as well as coproporphyrin isomer I was relatively and absolutely increased. Despite the different degree of the increase of coproporphyrin isomer I excretion between RS (69 vs. 72%) and cholestasis (47% on average), there are single cases with a coproporphyrin isomer I portion around 60%. In such cases, the differential diagnosis is quite difficult, so that the diagnosis "Rotor syndrome" should never be gained by one distinct examination; it is a diagnosis performed by exclusion of other diseases.

[Spontaneous thrombocyte aggregation and blood sugar profile in children with diabetes mellitus type I treated with human and porcine insulin].

Nalbuphine for obstetric analgesia. A comparison of nalbuphine with pethidine for pain relief in labour when administered by patient-controlled analgesia (PCA).

A double-blind, randomised study of 60 patients who received intravenous increments of nalbuphine 3 mg or pethidine 15 mg by patient-controlled analgesia during the first stage of labour, was carried out. Pain intensity, sedation, uterine contractions, maternal cardioventilatory variables and fetal heart rate were recorded as well as any side effects. Apgar scores, time to sustained respiration and resuscitative measures required for the neonate were noted at delivery. Modified neonatal neurobehavioural studies and a retrospective assessment of maternal analgesia, satisfaction and tolerance were also carried out. Group mean values of pain scores of nalbuphine-medicated primiparous women were statistically significantly lower than those of pethidine-medicated patients (p less than 0.01). Other assessments did not demonstrate a statistical significance between the two groups.

Use of long-acting oxytetracycline in the immunisation of cattle against Babesia bovis and B bigemina.

Forty Friesian one-year-old calves were vaccinated simultaneously with live Babesia bovis and B bigemina vaccines. Three groups of 10 calves each were treated with two, three or four doses of 20 mg kg-1 long-acting oxytetracycline (OTC/LA) at six- to seven-day intervals starting from day 6 after vaccination. Ten animals remained untreated. The treated calves showed considerably fewer days of patency and higher packed cell volumes than the vaccinated untreated calves. All calves developed serum antibodies to both parasites following vaccination. Five months later the 40 vaccinated and 30 new calves were challenged with syringe-transferred virulent parasites of both species. The vaccinated calves showed no parasites or clinical manifestations while calves of the new group exhibited severe clinical babesiosis. These results show that when OTC/LA is administered following anti-babesial vaccination, parasitaemia and red blood cell destruction are significantly reduced without, however, inhibiting the development of immunity.
