[Candidiasis prevention in newborn infants of diabetic mothers].

Two groups of 70 newborns each, all from diabetic mothers with and without nystatin prophylaxis, were compared, during the first three weeks of age, for incidence of mucocutaneous Candida albicans contamination (17 or 30 per cent) and for moniliasis (17 per cent in the group without prophylaxis and 71 per cent with Candida albicans contamination). Reference is made to modes of birth for comparison between pathogenesis and incidence of moniliasis and the need for prophylaxis of newborns delivered by diabetic mothers or by women with intact metabolism.

[Computerized tomography in adrenal diseases with excessive production of hormones].

In 41 patients with adrenal diseases and hormonal excess (pheochromocytoma n = 9, Cushing's syndrome n = 6, primary aldosteronism n = 26) computer tomography was performed for localization and classification of the adrenal lesion(s). In all patients diagnosis was confirmed by surgery. In all patients with pheochromocytoma a tumor was detected. 8 patients had unilateral adrenal pheochromocytoma, whereas in one female patient the pheochromocytoma was multilocular. 4 of the 6 patients with hypothalamic-pituitary Cushing's syndrome had enlargement of the adrenals. In the 26 patients with primary aldosteronism, computed tomography showed normal adrenal glands in 2 of the 17 patients with unilateral adenoma (with a diameter of 8 mm in both of the 2 patients) and in 6 of the 9 patients with bilateral hyperplasia. In the remaining patients adenoma or hyperplasia was detected. The results document that in patients with pheochromocytoma, with Cushing's syndrome, and in those with primary aldosteronism, computed tomography is a simple and valid method for localization and classification of the adrenal lesion(s). In primary aldosteronism with a normal adrenal computed tomography, however, differentiation between a small unilateral adrenal adenoma and bilateral adrenal hyperplasia remains problematic.

[Changes in serum lipoproteins in cerebrovascular disease (author's transl)].

The determination of serum cholesterol, triglycerides and lipoprotein cholesterol (VLDL, LDL and HDL) in 96 patients suffering from cerebral vascular disease showed that some parameters were significantly different from those recorded in a control population of corresponding age HDL values below 40 mg/dl were found in 50% of cases. Pathological LDH/HDL and VLDL/HDL ratios were found in 30.9% and 37.7%, respectively, of patients with cerebrovascular disease, whose serum cholesterol level was below 250 mg/dl and triglyceride level was 175 mg/dl. Only 56% of all patients had elevated serum cholesterol and/triglyceride and triglyceride alone does not detect the risk. The difference in the VLDL/HDL ratio in patients with cerebrovascular disease and that of the control population is highly significant. The HDL values and the VLDL/HDL ratio differ significantly, also. On average, the HDL levels in women are higher than those in men, but the decrease faster with increasing age. In both sexes a decrease in HDL level occurs between the age of 40 and 80. Low HDL levels and unfavourable LDL/HDL and VLDL/HDL quotients are important risk indicators for cerebrovascular disease.

[Carotid auscultation and Doppler sonography of the A. supratrochlearis in cerebral ischemias].

72 patients (117 angiographies ) suffering from transient ischemic attacks or prolonged reversible ischemic neurological deficits have been examined to study the diagnostic accuracy of the Doppler-sonography of the supratrochlear artery and of neck bruits. The specificity and sensitivity of the Doppler examination were 91 and 81%. The specificity and sensitivity of carotid bruits were 80 and 60%. The sensitivity increased to 94% when either a neck bruit or a pathological Doppler-test were interpreted to be indicative of carotid occlusive disease.

Oestrogens, gonadotropins and prolactin after intra-vaginal administration of oestriol in post-menopausal women.

Serum total oestrone, 17 beta-oestradiol and oestriol concentrations and FSH, LH and prolactin values were measured radioimmunologically in post-menopausal women before and after intra-vaginal application of 0.5 mg oestriol. While oestrone and oestradiol were not altered, there was a 3100% increase in the mean oestriol values within 1 or 2 h; pre-treatment levels were again reached 8 h later. Both gonadotropins were moderately decreased, the serum prolactin values appeared to be slightly elevated. Repeated intra-vaginal application of oestriol resulted in a significant rise of the mean serum oestriol levels while the other oestrogens remained unchanged. The same was true for FSH and LH, a considerable negative feedback was therefore excluded. Again there seemed to be a slight rise of the prolactin secretion. It was concluded that intra-vaginal administration of oestriol is a most suitable local and systemic oestrogen replacement therapy, which is more effective than the oral regimen.

Acute hepatic ischaemia in the pig- the changes in plasma hormones, amino acids and brain biochemistry.

Acute hepatic ischaemia was induced in pigs by means of a portacaval shunt with hepatic artery ligation after 24 hours. Despite significant elevation in blood ammonia, fatty acids, aspartate aminotransferase, cerebrospinal fluid glutamine and ammonia, and brain tissue glutamine, ammonia and tryptophan, the experimental animals remained awake and alert and indistinguishable from sham-operated controls. The molar ratio of branched-chain to aromatic amino acids fell sharply in the arterial blood, but showed a terminal attempt at compensation in muscle venous samples. Portal and muscle venous insulin levels were elevated, and glucagon values rose in all circulation segments in the experimental group. The failure to induce coma in these pigs, despite the presence of many of the classical biochemical features, suggests that the syndrome of encephalopathy comprises several stages, and that the pig may be an important model in which to define these.
