The importance of symptom duration in relation to prognosis of carcinoma of the large intestine.

Between 1971 and 1982, 162 patients underwent surgical treatment for carcinoma of the large intestine; follow-up data was available for 94 per cent of the patients. Tumor stage distribution was homogeneous and did not differ significantly with increasing duration of symptoms. The duration of symptoms was not determinant of survival, whereas stage influenced survival. These results show that patients with carcinoma of the large intestine with a short symptomatic period do not have either less advanced tumors or better survival prospects.

Telescopic anastomosis of the common bile duct in the rat.

A telescopic technique for the end-to-end anastomosis of the common bile duct in the rat is proposed as a way of reducing the number of sutures without prejudicing the impermeability of the union. Serial histological examinations of the scar process after 10, 20, and 30 days showed that patency was maintained.

Involvement of mitochondrial protein synthesis in sporulation: effects of erythromycin on macromolecular synthesis, meiosis, and ascospore formation in Saccharomyces cerevisiae.

Cells of strain Z270 (MAT alpha/MAT alpha) of Saccharomyces cerevisiae did not undergo ascospore formation in buffered or unbuffered acetate sporulation medium in the presence of erythromycin. The drug inhibited sporulation when added within the first 6 to 8 h and affected to different extents some of the metabolic and sporulation-specific events that normally occur during this period. In sporulation medium, protein synthesis was highly sensitive to erythromycin, whereas RNA synthesis was unaffected and premeiotic DNA synthesis was partially inhibited. Intragenic recombination occurred at normal rates for the various heteroallelic loci tested, but rates of intergenic recombination were markedly reduced, and commitment to haploidization did not occur; hence, development was evidently arrested between intragenic and intergenic recombination. Cells kept for 8 h in acetate sporulation medium that were ready for sporulation in water without erythromycin failed to sporulate in water containing the drug, indicating that erythromycin can inhibit sporulation independent of acetate utilization.

Ds and Cs: do's and don'ts.

Curettage of the uterus was introduced by Recamier in 1843 to remove fungosities. After a dubious reception the procedure fell into disrepute, to be reintroduced in France by Simon in 1872. Now it is one of the most frequently performed diagnostic procedures and has attained a ritualistic status in the management of diseases of women. Of the 125,000 curettages on the insured population of Australia in the year ending 31 August 1982, 25,000 were performed by general practitioners (Willett L. Personal communication; June 1983).

[Use of hyperbaric oxygen therapy in a case of inguino-genital gangrene].

A case of inguinogenital gangrene in a 63 years old man is reported. The patient was admitted to the hyperbaric medicine centre in a serious general condition. The use of HOT, antibiotics and selective surgery as treatment of metabolic disturbances effectively improved the patient's general condition, and HOT also proved effective in the treatment of hepatic insufficiency.

Pseudo-elastica in alcoholic hepatic fibrosis: histochemical and ultrastructural aspects.

During the early phases of alcoholic hepatic fibrosis, histochemical studies show positivity to both specific elastin and the specific collagen stains at the level of the centrolobular vein, the portal triad and portacaval anastomosis. Ultrastructural investigation in the portacaval anastomosis demonstrates the presence of collagen fibrils and afibrillar material. Treatment with tannic acid increases the contrast of these structures. The possible correlations between histochemical and ultrastructural findings are discussed.

A new hypothesis on memory - a possible role of local circuits in the formation of the memory trace.

An hypothesis is proposed arguing that local circuits might work as biochemical units involved in synthesis of macromolecules related to the memory processes. Specific sequences in the activation of local circuits should act as a "template" for the synthesis of particular memory molecules, which, in turn, may facilitate the repetition of the original sequences of bioelectrical signals (short-term memory). The resynthesis of the local memory molecules in cell body areas, their flow to terminal areas via axoplasmic transport and their possible passage to other nerve cells via transynaptic transport might be the basis of long term memory. The role of diffuse neuronal systems (e.g. noradrenaline systems) is discussed in the frame of memory recall and consolidation. The suggested hypothesis can be represented in a very effective way by means of the graph theory.

Cochrane Drugs and Alcohol Group: the development of systematic reviews of treatment outcome.

- The aim of the Cochrane Collaboration is to promote review processes which address all aspects of health care and which can be viewed by clinicians to guide their day-to-day clinical practice. Recently, a Cochrane review group on drugs and alcohol has been developed. The Cochrane Review Group Editorial base is in Rome, Italy. There is an international editorial board with editors in the UK, Italy, France, Australia and the USA. So far, the group has published five reviews addressing treatment for opioid, cocaine and alcohol dependence. Additional reviews and protocols are in progress. A growing number of titles are registered with the group. Standing to lying heart rate variation. A new simple test in the diagnosis of diabetic autonomic neuropathy.

We have examined the immediate heart-rate response to standing to lying (S-L) in 83 male insulin-dependent diabetic subjects aged 40 +/- 11 years (mean +/-S.D.) who underwent five other cardiovascular autonomic tests. Using a specially devised scoring system, the patients were divided into 3 groups: 54 subjects without autonomic neuropathy; 17 'borderlines'; 12 with autonomic neuropathy. The results were compared with those of 50 sex and age matched controls. We evaluated: S-L1 = ratio between R-R mean before lying and R-R minimum over the first 5 beats after lying; S-L2 = ratio between R-R maximum between the 20th to 25th beat and R-R minimum over the first 5 beats after lying. In controls S-L1 was 1.23 +/- 0.098 (mean +/- S.D.), S-L2 1.56 +/- 0.2. In diabetic subjects without autonomic neuropathy S-L1 was 1.18 +/- 0.096 (p less than 0.01), S-L2 1.50 +/- 0.23. In the autonomic group S-L1 was 1.03 +/- 0.01 (p less than 0.001), S-L2 1.16 +/- 0.086 (p less than 0.001). We propose that the lowest normal and highest abnormal limits of S-L1 are 1.10 and 1.07, respectively, and that normal and highest abnormal limits of S-L2 are 1.23 and 1.41, respectively. We suggest the use of S-L1 as a pure parasympathetic test and S-L2 as a mixed but predominantly sympathetic test in the diagnosis of autonomic neuropathy.

The surgical treatment of nonunion of the proximal metaphysis of the femur.

Nonunion of the proximal femur is a severe pathology, often provoked by the inopportune or improper use of a therapeutic aid: nonsurgical (primary nonunion), surgical (secondary nonunion). Surgical treatment of this nonunion may thus be characterized by different degrees of difficulty, depending on whether or not it is the sequela of surgery or of nonsurgical treatment. In lax nonunion, with atrophy of the segments and regions of necrotic bone interposed, modeling resection is required to correct the functional axes. The condylar blade-plate may be opposed by a cortical graft--to improve stability of the assembly--protecting the medial wall and providing the screws with excellent hold.

Aseptic nonunion of the humeral shaft.

A total of 78 cases of aseptic nonunion of the humeral shaft treated at the third division of the Rizzoli Orthopaedic Institute between 1980 and 1997 were evaluated. There was a mean time of 19 months between trauma and definitive treatment. Surgical treatment consisted in osteosynthesis with a plate and bone grafting in 75 cases, and synthesis with screwed bone grafting alone in 3 cases. Consolidation was obtained in 96% of cases. The bone graft used (homo- or autoplastic) encountered complete osteointegration in a maximum time of 8 months. Postoperative motility of the shoulder and elbow were comparable to the preoperative findings.
