[Clinical and risk records assessment].

In the collaboration between the Hospital Occupational Health Department and Public Occupational Service of Bergamo, it has been elaborated a plan finalised to the verification of the quality and of the appropriateness in the compilation of the Clinical and Risk Record in the firm. The study has been lead by evaluation of 409 clinical and risk records (containing the first visit and, in 277 records, a periodic visit) collected from different productive areas (wood, building, health, agriculture). The analysis of the clinical and risk records have shown many critical aspects in their compilation, for this reason it is necessary to improve vigilance and attention from Public Occupational Service, Hospital Occupational Health Department and the Scientific Associations.

[Health surveillance in noise exposure].

The Occupational Health Department of Bergamo Ospedali Riuniti and Occupational Health Public Services set up a project in order to check the health surveillance quality in noise exposed workers. 887 hearing threshold tests, performed between 1998 and 2002, were examined; the audiograms concerned 304 workers exposed to industrial noise higher than 85dB(A), employed in 22 textile, mechanical, wood, stone, building and paper industries. After examining the 887 audiograms we noticed that the health surveillance of noise exposed workers up to now, 14 years after the issuing of the Legislative Decree 277/91, reveals many critical situations, showing an inadequate accuracy and possibility of interpretation of the audiometric tests.

[Occupational risk and prevention in the biotechnology industry: a review].

BACKGROUND: The biotechnology industry has expanded greatly in the last 20-30 years and has led to a number of applications in different sectors of work, i.e., medical and pharmaceutical, agricultural, chemical, energetic and others. Nowadays hundreds of thousands of workers worldwide are employed in biotechnology plants. Health and safety issues related to such working activities are considered as relevant to workers as well as to the general public. In particular, when compared to traditional biotechnology, modern methods of processing microrganisms have given rise to public concern that they might generate hazards to human beings and to the environment. OBJECTIVES AND METHODS: After summarizing the most important products and fields of application, the paper sets out to detail potential adverse effects for the health of biotechnology workers; in addition, an analysis of the literature highlights the various concepts of primary and secondary prevention. RESULTS AND CONCLUSION: Along with occupational risk factors common to other working activities (i.e. the well-known physical and chemical hazards), the peculiarity of handling microrganisms and/or different biologic systems may induce infections, immunological alterations or non-infective and non-immunologic toxic reactions in the workers involved The need is emphasized for an accurate risk assessment, careful control by means of the current monitoring strategies and implementation of the confinement measures, taking into account the criteria set by Italian legislation for occupational biological risk. Lastly, attention is focussed on examinations for the medical surveillance of workers at risk.

Cement thickness and microleakage under Dicor crowns: an in vivo investigation.

This in vivo study recorded the microleakage and marginal discrepancies of cast glass-ceramic cemented crowns compared to gold onlays and a metal ceramic restoration. Within the limits of the small number of samples and the variables inherent in an in vivo study, it was found that (1) dentin bonding agents are not strong enough to produce a gap-free cementation, (2) the marginal adaptation of beveled metal restorations is better than that of glass-ceramic crowns, and (3) the marginal adaptation of cast glass-ceramic crowns is clinically acceptable.

Synthesis and immunomodulating activity of condensed N-aryl-2-cyano-3-oxo-3-pyrazolyl-propanamides.

A series of condensed N-aryl-2-cyano-3-oxo-3-pyrazolyl-propanamides were synthesized and evaluated for immunomodulating activity following intraperitoneal administration. These new molecules were found to enhance macrophage cytotoxicity and stimulate host mediated antibacterial defences in mice. The compound 3-cyano-3-(1,4-dihydro-1-phenyl-[1]-benzothiopyrano[4,3-c]pyrazol- 3-yl]-3-oxo-N-phenyl-propanamide, chosen for wider pharmacological investigation, proved effective in preventing adjuvant-induced arthritis development in rats.

Influence of gastric pH changes on pharmacokinetic of a sustained-release formulation of theophylline.

The effect of magnesium-aluminum-hydroxide antacid (Maalox) on the oral absorption of theophylline anhydrous time-release capsules Somophyllin CRT was studied in eight healthy volunteers. The subjects were given 500 mg of theophylline alone or with 30 ml of antacid in a complete crossover design. Drug serum concentrations were determined by an immunoenzymatic method within 24 hours at frequent intervals. Antacid induced a significant reduction of theophylline serum levels and of the fraction absorbed at 1 and 1.5 hours of the monitoring period (less than 0.05), but no significant differences were found at later sampling time. Maalox did not influence the area under the curve from 0 to 24 hours and the peak concentrations; the time to peak was slightly but significantly longer with antacid (p less than 0.05). Our results indicate that physicochemical factors such as gastrointestinal fluid pH do not influence substantially absorption of the slow-release formulation Somophyllin.

Association of intracranial meningiomas and aneurysms. Report of three cases and review of the literature.

The Authors report three personal cases of association between meningioma and arterial aneurysm, both intracranial. Twenty-three similar cases are referred in the literature, bringing the total to 26. Of these 26, the overall average age at diagnosis was 48.3 years: 17 were females and 9 males. The predominant site for the meningioma was the cerebral convexity and for the aneurysm the anterior communicating artery. Meningiomas were multiple in 5 cases and the aneurysms were multiple in the same number of cases. In 21 cases the meningioma was removed: in 11 of these the aneurysm was treated simultaneously. In the majority of cases the aneurysm was an accidental finding during neuroradiological investigation to document the meningioma; panangiography is reported to have been performed only in a few cases. Greater use of the latter would probably have revealed a higher incidence of this rare association. The greater frequency found of the association of aneurysm and multiple meningiomas makes it advisable to perform panangiography in all such cases. The best long-term results were obtained in the cases where the meningioma and aneurysm were operated simultaneously.

Retrospective comparison of freeze and non-freeze myopic epikeratophakia.

We conducted a retrospective study of 20 eyes of 20 selected patients with preoperative myopia of -14 to -28 diopters who were treated with myopic epikeratophakia and followed for 8 weeks. Seven eyes were treated with frozen-lyophilized donor lenticules by the Kaufman-McDonald technique and 13 eyes were treated with freshly prepared donor lenticules using the Barraquer-Krumeich-Swinger (BKS) technique. All lenticules were sutured into a circular keratotomy without keratectomy. Postoperatively, all eyes had a refraction between -4.00 and +2.00 D, and there was no meaningful difference in the refractive outcome of the two groups. The freeze-lyophilized technique showed a greater overall loss of best spectacle corrected visual acuity, although no eyes in the entire series lost more than two lines of Snellen visual acuity compared to preoperative values. Uncorrected visual acuity after surgery was 20/40 or better in 5% of each group. The rate of postoperative epithelialization was slower in the freeze-lyophilized group, but all eyes were re-epithalialized by 3 weeks after surgery. No lenticules had to be removed for complications, but there was one case of epithelial ingrowth into the interface that required a second operative procedure. Our clinical experience with this small group of eyes shows that both techniques of myopic epikeratophakia can safely and effectively reduce high amounts of myopia, but that the refractive outcome is less predictable than desired. The technical requirements for the non-freeze (BKS) technique are high, but the rate of visual recovery is faster.

[Differential diagnosis of multiple gliomas, multicentric gliomas and the other main endocranial multifocal lesions. Presentation of a case].

Multifocal gliomas represent an uncommon lesion, particularly if the supra-tentorial and infra-tentorial spaces are involved. The Authors describe a particular case and they outline the main differences between multiple gliomas and multicentric gliomas. Then, they consider the various possibilities of diagnosis and differential diagnosis of multiple endocranial lesions.

[Familial cerebral aneurysms: case report and review of the literature].

Familial occurrence of cerebral aneurysms is rare. About 80 families affected by these lesions have been reported in the international literature to date. The authors describe three members of a family presenting intracranial aneurysms without other congenital associated anomalies. Clinico-pathological features, with regard to the possibility of an early and a more effective diagnostic and therapeutic approach, are presented and discussed in the light of the available literature on this topic.

[Multicentric glioma: presentation of a case].

A case of multiformis glioblastoma with a parieto-temporal and brainstem localization is presented. Multicentric gliomas are unfrequently reported in the literature, and very often rise diagnostic problems with other multiple lesions of the Central Nervous System, either of neoplastic nature or not. After briefly reviewing the pathogenetic hypothesis concerning this lesion, indication and limits of clinical and radiological investigations are discussed.
