Prevention of cystic fibrosis in Italian families by DNA studies.

We performed a feasibility study of prenatal diagnosis in the Italian population by DNA analysis utilizing the probes: metD, metH, J3.11, KM.19 and XV-2c. With these probes, 118 out of 126 families (93.7%) were fully informative. We also tested part of the families with new additional tightly linked probes, E.9 and W3D1.4. With all the probes feasibility was complete in 44 out of 45 families (97.8%). In particular with a set of only 3 tightly linked probes, KM.19 + XV-2c + W3D1.4, 71% of the families were fully informative. We carried out 8 prenatal diagnoses, 4 of which with the polymerase chain reaction (PCR) for KM.19. These data show that these probes are highly informative and can be used for feasibility studies and prenatal diagnosis of CF in the Italian population.

A further study on relationships between herpes simplex virus and Bovid herpesvirus-2.

Calves exposed by intravenous or intradermal inoculation with Herpes simplex virus (HSV), types 1 and 2, remained clinically normal and HSV was not recovered from nasal secretion nor blood samples. However, the clinical response of calves pre-inoculated with HSV, to Bovid herpesvirus-2 (BH-2) challenge infection was much milder than that in the challenge control calves, and the titer of BHV-2 by skin titration underwent a significant (2-2.5 log units) reduction in the HSV pre-inoculated calves. Inoculation of calves with live HSV provided a much greater protection against BHV-2 challenge infection compared with the results of previous experiments in which a Triton X100-inactivated HSV antigen was used. It was speculated that the possibility of HSV replicating in cattle must still be considered an open question.

The lapinized Chinese strain of hog cholera virus (HCV) previously adapted in minipig kidney (MPK) cell line cultures, would also protect pigs to experimental infection with virulent HCV [corrected].

Two-month old piglets previously inoculated with different dilutions of the Lapinized Chinese (LC) strain of Hog Cholera Virus (HCV), adapted in a minipig kidney (MPK) cell line, resisted challenge infection with virulent HCV. All the animals remained healthy and the challenge virus was never recovered from any of them. In contrast, the pigs which served as controls for the challenging virus underwent the clinically lethal form of the disease and HCV was consistently recovered from their tissues.

Bullet emboli to the systemic and venous circulation.

The rarity of bullet emboli leads to frequent delays in diagnosis and inadequate early management. Our recent experience with this entity is described, and 153 cases reported in the English-language literature are reviewed and summarized. The majority of cases occurs as a consequence of civilian violence among men in their 20s and 30s. Most bullet emboli follow the direction of blood flow, although 15% of venous bullets cause embolization in a retrograde manner. One in 10 arterial emboli follow a right-heart or venous injury. Arterial bullets are symptomatic in 80% of cases, venous bullets in only one third. The choice of surgical management must be individualized according to the symptoms caused by the bullet and its location in the vascular system. In general, arterial bullet emboli are removed because of symptoms or findings of acute peripheral ischemia. Arterial and venous emboli not causing symptoms should be removed according to the risk of possible displacement and further embolization. Selective intraoperative angiograms and phlebograms can precisely localize the migrating bullet and permit appropriate placement of incisions before removal.

The influence of acidosis on the myocardial uptake and electrocardiographic effects of disopyramide.

The time course for the ECG effects and myocardial uptake of disopyramide was studied in isolated perfused guinea pig hearts under different pH conditions. At pH 7.46 the drug depressed the overall AV conduction time (PR) by 16.64%, the His-ventriculum conduction time (HV interval) by 30.46% and delayed the ventricular repolarization (QT interval) by 8.08%, on average. The maximum intraventricular pressure (Pmax) was also depressed by 35.6%. The maximum effect on the QT interval (constant rate: 0.609 min-1) was reached faster than the maximum effect on the PR and HV intervals (constant rates: 0.399 and 0.400 min-1, respectively), while the myocardium uptake process was complete before any ECG parameter reached a steady state (uptake constant: 1.58 min-1). Under conditions of extracellular acidosis (pH 6.92), the disopyramide disposition parameters (uptake rate constant and myocardial concentration) were not modified. However, the drug exerted significantly smaller effects on the HV and QT intervals and on myocardial contractility. These results are in contrast with those obtained previously with lidocaine and quinidine, and indicate that the influence of acidosis on class 1 antiarrhythmic agents may also depend on the characteristics of the individual drug.

Determination of cerebral venous hemoglobin saturation by derivative near infrared spectroscopy.

An "in vivo" method for non-invasive determination of cerebral venous hemoglobin O2 saturation (SvO2) was developed. A specially designed spectrophotometer recorded the Td near IR spectra of transilluminated brain tissue surrounding the SS. The accuracy of the method, based on the principle of DNIRS was tested on eight pentobarbital anesthetized dogs during hypoxic hypoxia (inspired O2 6-21%). Spectral data were transformed into first derivative for correlation with SvO2 data measured from the SS. Linear regression analyses were applied using data from 5 dogs, with SvO2 ranging from 1.5%-70%, to build a 3 wavelength algorithm for predicting brain SvO2. In three dogs, this regression equation was employed to predict SvO2 in 31 separate spectra of varying HH intensity. The standard deviation of differences between SvO2 and predicted values was 3.2%. The predicted values, when regressed against the sampled SvO2, yielded an r value of 0.97. The results demonstrate that it is possible to noninvasively quantify SvO2 utilizing IR spectroscopy.

Near infrared determined cerebral transit time and oxy- and deoxyhemoglobin relationships during hemorrhagic hypotension in the dog.

The goal of the present study was to investigate the relationships among CBF, CMRO2, MTT and tissue/superior sagittal sinus hemoglobin saturations over a wide range of CPP using a canine model of hypotension. Microsphere-determined CBF was autoregulated in all tested cerebral regions over the 40-130 mmHg CPP range, but it decreased 50% around 30 mmHg. MTT, calculated by an indicator-dilution technique, progressively and proportionately increased over the 40-130 mmHg CPP range. The HbO2 content, measured in the same area by an optical method, parallely decreased. Around 30 mmHg CPP, MTT disproportionately lengthened (700% of control). CMRO2 maintenance was accompanied by a progressive decrease of tissue HbO2 and sagittal sinus hemoglobin saturation. In this framework, MTT measurement and tissue HbO2 monitoring by NIRS might give a sensitive evaluation of the hemodynamic reserve than CBF measurement in anesthesia and intensive care.

Immunization against bovine rotaviral infection.

Calves fed with colostrum from cows vaccinated with an inactivated rotavirus vaccine were refractory to experimental infection with strain 81/36F of bovine rotavirus. In the field study, 458 pregnant cows from 26 herds were involved. In each herd, cows were selected and randomly subdivided in two groups. Cows in one group (248 head in total) were vaccinated, whereas cows in the other group (210 head in total) were left as unvaccinated controls. At calving, colostrum was collected from each cow and stored at -30 degrees C until used for feeding calves. The newborn calves, beginning the second day of life and for the next 7-10 days, were each fed a daily supplement of 400 ml of colostrum from its dam. The diarrhea occurred in 86 (40.9%) calves that had received colostrum from unvaccinated dams (normal colostrum), and in 7 (2.8%) calves which were fed colostrum from vaccinated dams (immune colostrum). The disease was very severe in the normal colostrum-fed calves and 52 of them died. By contrast, the 7 immune colostrum-fed calves displayed a rather mild enteric condition, and all recovered without any sequela being observed.

FCE20696, a new synthetic immunomodulator: immunopharmacological profile.

A preliminary characterization of the immunopharmacological profile of the new immunomodulating agent FCE20696 (6H,6[2-(dimethylamino)-ethoxycarbonyl]-dibenzo[b,d]pyran-HCl) was performed in mice. Single i.p. doses of this chemical, concomitantly given with the antigen, increased the antibody response and decreased the delayed hypersensitivity reaction to a suboptimal dose of SRBC, the active doses ranging from 6.25 to 50 mg/kg. No activity was observed using a full antigen dose. Macrophage cytotoxic activity was enhanced 2-4 days after a single i.p. treatment with 50 and 10 mg/kg. Spleen cell proliferation to T and B mitogens was inhibited by a single dose of 30 mg/kg given i.p. 6 days before the test, or by 10 mg/kg x 3 days ending one day before the test. Finally, generation of suppressor cells was enhanced by the compound, given p.o. biweekly for at least 7 weeks, at doses ranging from 0.1 to 10 mg/kg. Collectively taken, these data suggest that FCE20696 has a broad range of immunomodulating activities and that macrophages and suppressor cells are presumed to be the main targets of its pharmacological activity.

Evaluation of a chemical etching solution for nickel-chromium-beryllium and chromium-cobalt alloys.

Two chemical etching solutions were capable of providing micromechanical retention in two nickel-chromium-beryllium alloys and in a chromium-cobalt alloy. A resin matrix was used to verify the quality of etching on the metal surfaces. The chemical etching solutions created high microretentive surfaces in nickel-chromium-beryllium alloy but the chromium-cobalt alloy surfaces after etching were less retentive. Improved chemical etching technique should encourage expanded use of the resin-bonded retainers.

Further studies on passive immunization of newborn calves against rotaviral infection.

An inactivated rotavirus vaccine was prepared with an adjuvant which gives a water-in-oil emulsion. The vaccine when injected into cows in their last month of pregnancy, proved to be as effective as the traditional vaccine, prepared with the Freud's incomplete adjuvant, in enhancing the lactogenic immunity. Thus, feeding experimentally infected calves with colostrum and first milk from vaccinated cows prevented diarrhea and reduced significantly rotavirus shedding. Because of its low grade viscosity the water-in-oil emulsifier facilitates inoculation of the vaccine and is therefore recommended as an adjuvant in the preparation of inactivated rotavirus vaccine.

Haplotypes in cystic fibrosis patients with or without pancreatic insufficiency from four European populations.

We examined the allele and haplotype frequencies of five polymorphic DNA markers in 355 European cystic fibrosis (CF) patients (from Belgium, the German Democratic Republic, Greece, and Italy) who were divided into two groups according to whether they were or not taking supplementary pancreatic enzymes. The level of linkage disequilibrium between each polymorphism and the CF mutation varied among the different populations; there was no significant association between KM.19 and CF in the Greek population. The distributions of alleles and haplotypes derived from the polymorphisms revealed by probes KM.19 and XV.2c were always different in patients with or without pancreatic insufficiency (PI) in all the populations studied. In particular, among 32 patients without PI, only 9 (or 28%) were homozygous for the KM.19-XV.2c = 2-1 haplotype (which was present in 73% of all the CF chromosomes in our sample) compared to 162 of 252 patients (or 64%) with PI. These findings are consistent with the hypothesis that pancreatic insufficiency or sufficiency may be determined by different mutations at the CF locus.

Prostate specific antigen in the diagnosis and treatment of adenocarcinoma of the prostate. III. Radiation treated patients.

Serum prostate specific antigen was determined (Yang polyclonal radioimmunoassay) in 183 men after radiation therapy for adenocarcinoma of the prostate. A total of 163 men had received 7,000 rad external beam radiotherapy and 20 had been implanted with 125iodine seeds. Only 11 per cent of these 183 patients had undetectable prostate specific antigen levels at a mean interval of 5 years since completion of radiotherapy. Prostate specific antigen levels after radiotherapy were directly related to initial clinical stage and Gleason score before treatment. Multiple prostate specific antigen determinations were performed with time in 124 of 183 patients. During year 1 after radiotherapy prostate specific antigen levels were decreasing in 82 per cent of the patients but only 8 per cent continued to decrease beyond year 1. Of 80 patients observed greater than 1 year after completion of radiotherapy 51 per cent had increasing values and 41 per cent had stable values. Increasing prostate specific antigen values after radiotherapy were correlated with progression to metastastic disease and residual cancer on prostate biopsy. Total serum acid phosphatase levels were poorly related to prostate specific antigen levels, were less effective in discriminating patients with metastatic disease and provided no additional information beyond that provided by prostate specific antigen.
