[Laryngeal infection by Rhodococcus equi in patient with AIDS].

The Rhodococcus equi is an aerobic gram positive pleomorphic bacillus, that was isolated for the first time like a producer of bronchopneumonia in young horses. Every time more often, it is being recognized as a pathogen in humans, mainly in the immunodepressed population. We described a case, until now exceptional, of laryngeal infection by Rhodococcus equi in a patient with positive serology for the virus of the human immunodeficiency (HIV), and we reviewed some clinical and epidemiological characteristics of the infections by this germ. The treatment is riphampicine and/or erythromycin, being the prognosis bad, because usually they are immunodepressed patients.

[Primary malignant melanoma of the oral cavity. Report of a case. Review of the literature].

Malignant melanomas of the oral mucous membrane seems an infrequent pathology. They are characterized by a late diagnosis, bad prognosis and a disappointing treatment, because of the difficulties encountered for its total removal owing to complicated accessibility. We report one case of mucous membrane of the mouth in a 56 year-old man, diagnosed and operated in the General Hospital of Specialties "Ciudad de Jaen". The article end with a careful revision of the literature.

[Intra-parotid facial plexiform neurofibroma. Review of the literature].

It is being tumor developed from Schwann cells of peripheral nerves sheath. In its plexiform type it is a rare growth presenting solitary or in the context of von Recklinghausen's disease. The aim of this article is to present one plexiform neurofibroma localized in the parotid gland. At the same time we make a perusal on the literature of this kind of tumor.

[Complications of thyroid surgery. Report of 683 thyroidectomies].

Most important complications of thyroid gland surgery are recurrent paralysis (which usually results from section, stretching or devascularization of this neural branch) and parathyroid lesions resulting from unnoticed removal of more than two of these glands or through necrosis because deficient blood supply. We present a study about the complications of thyroid surgery based on the thyroid surgery done in our ENT Service during 1979-1998 term. An amount of 683 cases make the series. The 86% were female and 14% male, with a mean age of 40-50. Surgical techniques realized were: hemithyroidectomy (15.3%) and isthmectomy (2.9%). About 7.4% of patients were reoperated. Stated complications were: local (serohematoma 4.1%, infection 1.2%, postoperative hemorrhage 1.2%) and recurrent paralysis (unilateral transitory 2.6% and unilateral definitive 1.6%; there wasn't any case of bilateral palsy) and superior laryngeal nerve paralysis (1.4% as well as hypoparathyroidism (transitory 22.7% and definitive 5.3%). These complications were most frequent in reoperated cases and in total and subtotal thyroidectomies. In most of these series the incidence of recurrent palsy range between 1.5% and increases more than 9.5% in surgical check and the hypoparathyroidism incidence ranges over 25%.

[Magnetic resonance and metastatic neck adenopathies. Usefulness of the technique for cervical staging].

25 patients with epidermoid carcinoma of the larynx underwent staging of cervical adenopathies by magnetic resonance imaging (MRI). All patients subsequently underwent neck dissection. The MRI results were compared with the histopathological findings ans those gained through previous palpation. MRI correctly staged 54.1% of patients with a sensitivity of 66% and a specificity of 74%.

[Tumoral collision: cancer metastases within a second neoplasm].

A case report of epithelial carcinoma of the inferior lip which spread in cervical lymph nodes previously affected by a chronic lymphocitic leukaemia, is presented. The coexistence of both histological findings in some of the nodes (collision tumor) is remarked. We have made a review of the scanty literature about metastases on second tumors and compared and commented those features.

[Supraglottic carcinoma].

We present the follow-up and end results of 98 horizontal supraglottic laryngectomies done for treatment of supraglottic carcinomata. Both functional and oncologic outcome as well the complications and 5-year survival of these laryngectomies are considered. We conclude that this surgical technique is a suitable one for the larger part of supraglottic carcinomas.

[Otolaryngological complaints in tongue amyloidosis: a case report].

We present a case of Amyloidosis of the oral cavity associated to multiple mieloma, with otolaryngological symptom. Review of structural characterization of the disease, its pathogenesis and clinical disorders when displayed in thyroid, oral cavity and upper respiratory tract.

[Symptomatic malformations of the first branchial arch].

Congenital malformations of the branchial arches are not infrequent. Among them first branchial cleft anomalies are common and they are to remain asymptomatic. The case reported is one of symptomatic fistula auris, which at the beginning led us to the initial diagnosis of an orbital cellulitis, secondary to ethmoidal sinusitis. We have made a review of the literature dealing with these anomalies and we expose in the paper an update perusal of the clinical features and the management of this rare complication.

[Pneumocephalus as a complication of mastoid surgery].

Non-traumatic otologic diseases are a rare cause of Pneumocephalus. Among them few reports have been published whereas it presented following oto-surgery. The AA. communicate one of those events in a patient who presented with CSF-otorrhea, headache and disorientation after recent radical mastoidectomy. The definitive diagnosis was done by CT-scan imaging. Survey of the scarce literature on the subject, description of the clinical and diagnostic features, its possible physiopathology and surgical management.

[Pleomorphic adenoma of the palate].

Furthermore major salivary glands salivary tissue is localized in minor salivary glands. The AA. present a case of pleomorphic adenoma arising in soft palate.

[The value of the anamnesis and physical examination in surgical pathology of the thyroid gland. Report on 423 thyroidectomies].

The purpose of this paper is to present a retrospective study of 423 cases of surgical pathology of the thyroid gland. All the cases underwent surgery during a period of 11 years. The data gained through these ways were statistically studied and compared with those provided by histological diagnosis, with the aim of searching any clinic situations that can lead to the suspicion of oncological pathology of the gland.

[The value of the computer tomography (CT) and magnetic resonance (MR) in tumors of the ear].

The first case report is an undifferentiated squamous cell carcinoma of the ear. This patient complained with pain and discharging ear, which led us to practice a TC followed by an attico-antrotomy. The pathologist could detect in the removed tissue the neoplasm in its early evolutive stages. In the other case the earache and the suppuration suggested at first a carcinoma. In spite of the TAC and MR exams the surgery that followed was not confirmative of the tumor presence.

[Histiocytoid hemangioma].

Angiolymphoid hyperplasia with eosinophilia is a multisystemic disease characterized mainly by cutaneous nodules, vascular proliferation with atypical histiocytoid endothelial cells and numerous eosinophiles, often with blood eosinophilia. It use to recur "in situ" and persist after surgical excision. The AA. review the literature about this rare tumor and present the clinical and histopathological features of a case of Angiolymphoid hyperplasia with eosinophilia of the tongue.

[Auricular pseudocyst].

Pseudocyst of the auricle is a spontaneous intracartilaginous collection of serous fluid in the anterior aspect of the auricle. These cysts occur frequently in young healthy Chinese males. Engel (1966) was the first to report a series of pseudocysts of the auricle in Chinese. Its etiology is unknown, treatment is difficult and recurrence frequent. In the paper a case report is presented and a review of the literature of this rare condition is done.
