[Relations between the site of insulin absorption and hypoglycemia in insulin-dependent diabetes during physical exertion].

Hypoglycaemia often arises after physical exercise in insulin-dependent diabetics treated with fast and slow acting insulin. Some claim that this is closely linked to the injection site for fast insulin and related to altered mobilization of the hormone from its subcutaneous deposit. Others attribute it to hyperinsulinism. The hypoglycaemia occurring after physical exercise in subjects treated with slow insulin and that arising even hours after physical exercise are then considered.

[Oral contraceptives and diabetes].

Some remarks are presented on the contraceptive treatment of diabetic females with emphasis on the relationship between progestins and estrogenic progestins and the metabolism of carbohydrates and fats, haemostasis and arterial hypertension. Indications for the contraceptive treatment of insulin dependent and non-insulin dependent diabetics and subjects exposed to diabetic risk are supplied.

[Phase II trial of an ambulatory treatment with 5-fluorouracil administered by continuous perfusion combined with vindesine and cyclophosphamide].

Thirty-four patients with histologically proven metastatic breast cancer, were included in this phase II trial. They were given a five day course of continuous infusion of 5 fluoro-uracile at the daily dose of 500 mg/m2 associated with vindesine 2 mg/m2 on day 2 and 5, cyclophosphamide 300 mg/m2 on day 2 and 5. The treatment was repeated every 15 days after haematological and biological assay. Twenty-six patients were objectively resistant to previous cytotoxic chemotherapy including adriamycin and 6 patients had recently received adjuvant therapy. A major objective response was observed in 17 cases (50%) (4 complete responses, 13 responses greater than 50%) and a response less than 50% in 7 cases. The median duration of response is over 8 months. After 8 months of median follow-up, the median of survival is not defined.

Peridural administration of morphine, with or without adrenalin, for postoperative analgesia.

Morphine was administered by the peridural route, in doses of 2 to 6 mg, without and with adrenalin 1/200,000, to 197 patients undergoing various surgical procedures under peridural anesthesia, with a view to lower post-operative pain. The evaluation of the effect was estimated subjectively, with the aid of a visual analogue scale and was also based on the demand for analgesia. The latency time appeared to be long: 1 hour. The overall mean duration of action is 16.25 h. The efficiency is related to the type of operation: 2-3 mg doses injected after the operation are effective in only about 50% of laparotomies and orthopedic procedures, whereas with soft tissue surgery or ano-perineal and gynecological surgery efficiency is higher (83.33% and 88.23% respectively). The injection given during the operation is more efficient, but urinary retention also seems to increase. No respiratory, circulatory or motor depression has been observed. The most untoward effects are nausea and vomiting (32.99%), urinary retention (25.40%), itching (20.30%). Combination with adrenalin does not improve the efficiency of the analgesia and secondary effects tend to be enhanced, especially urinary retention (53.57%). The spinal mediation of the effects of morphine and its access to supraspinal structures are discussed.

[Prevention of paralytic ileus after colonic surgery by continuous peridural sympathetic block. Preliminary report].

Gastrointestinal atony must be considered as a reflex response to surgical intervention, due to activation of sympathetic nerves. Following colonic surgery, the duration of this response may be notably extended and the ensuing intestinal distension becomes the paramount factor of a persisting ileus. Blocking the sympathetic fibers prevents intestinal distension. Introducing appropriate catheter into the peridural space up to the level of the 10th--11th thoracic vertebra makes it possible to realize a segmental continuous block between T6--L2, where gastrointestinal sympathetic innervation actually originates. Using bupivacaine at low concentration (0.125% with epinephrine 1/400,000) reduces untoward effect on circulation and may even prove beneficial if volaemia is effective. In addition, it produces a selective block on the visceromotor fibers which allows a differential diagnosis of the ileus, without risking to pass over a perforation of the gut whenever a coexisting mechanical factor is suspected.

Demonstration of communication between alveolus and interstitium in persistent interstitial pulmonary emphysema: case report.

Persistent interstitial pulmonary emphysema (PIPE) is an uncommon complication of premature infants suffering from hyaline membrane disease who have been treated with mechanical ventilation. The presumed mechanism for the development of the disease is via a break in the bronchioalveolar system that allows air to escape into the interstitium. We report a case of a 9-week-old child who developed the localized form of the disease and underwent a lobectomy. Immunohistochemical stains helped to demonstrate the communication between the airway system and interstitium. This report strengthens the theory that the disease develops from airway rupture at the alveolar level.

Proliferative activity of adrenal glands with adrenocortical cytomegaly measured by MIB-1 labeling index.

To investigate the proliferative activity of cytomegalic cells in the fetal adrenal cortex, we studied adrenal glands with cytomegaly by immunohistochemistry using the nuclear proliferation maker MIB-1. The percentage of positively stained nuclei was quantified using the SAMBA 4000 image analysis system. Only one case showed occasional positively stained cytomegalic cell nuclei. The permanent cortices showed proliferative activity that decreased with increasing gestational age. No proliferative activity was seen in normal fetal cortices except in one case that received corticosteroid therapy and had a maternal history of diabetes. The near absence of proliferative activity of the cytomegalic cells supports the previously proposed theory of cellular exhaustion following hyperactivity. The high proliferative activity in the fetal cortex of the infant receiving corticosteroid therapy may provide insight into the stimulus causing the hypermetabolic state.
