Successful pregnancy renal transplantation.

Pregnancy in a renal homograft recipient resulted in the delivery of a 2770-g male fetus by classic cesarean section. Renal function remained stable throughout the gestation. The fetoplacental unit was monitored by the concurrent assay of 24-hour urinary estrogen excretion and human placental lactogen (hPL) in light of the spuriously low estrogen levels often found in these patients on chronic glucocorticoid immunosuppression.

Congenital release thrombocytopathy: pathophysiology and management.

Congenital release thrombocytopathy must be included in the differential diagnosis of bleeding diatheses in women. A review of the coagulation profiles of 7 patients with congenital release thrombocytopathy suggests that more sophisticated platelet aggregation studies must be performed when routine screening procedures fail to determine the cause of a clinically suspected bleeding disorder. Establishing the diagnosis helps to avoid the use of medications which cause an acquired platelet dysfunction, and contributes to adequate prophylaxis against surgical and obstetric hemorrhage.

Congenital absence of the uterine cervix.

Surgical correction of congenital atresia of the uterine cervix by creation of a fistulous communication between the functioning endometrial cavity and the vagina with the use of the atretic cervix as a conduit has been performed in two patients. This technique, which permits egress of menstrual blood and the retention of the capacity to reproduce, should replace hysterectomy as the treatment of choice for this extremely unusual and formerly hopeless Müllerian duct anomaly.

Normal electro-oculograms in two patients with malignant melanoma of the choroid.

The electro-oculogram has been applied in the diagnostic evaluation of eyes with malignant uveal melanoma. Typically, a marked reduction in the light peak to dark trough ratio has been reported. We studied two patients with histologically confirmed uveal malignant melanomas in whom the preenucleation electro-oculograms were normal. The electro-oculogram should be evaluated within the framework of the clinical examination and other ancillary tests in the diagnostic evaluation of a suspected uveal malignant melanoma.

Iridocyclitis masquerade syndrome.

A 52-year-old man presented with blurred vision and a red, painful eye. There was a marked anterior uveitis associated with a solid white lesion on the iris. The differential diagnosis and appropriate diagnostic workup are discussed. The diagnosis of adenocarcinoma of the lung with metastasis to the iris was made.

The diagnostic evaluation and therapy of secondary amenorrhea.

One of the greatest challenges to health-care providers for women is the diagnostic evaluation and treatment of secondary amenorrhea in an efficient, cost-effective, and timely fashion. Categories and compartments have been presented in order to assist one with the myriad of possible diagnostic tests and therapeutic modalities. The etiology and treatment of this common gynecologic symptom is reviewed in order to provide a working knowledge base for the nurse. In addition, the role of the nurse in the care of these clients is explored.

Herpes zoster ophthalmicus with orbital pseudotumor syndrome complicated by optic nerve infarction and cerebral granulomatous angiitis: MR-pathologic correlation.

The authors describe a 41-year-old woman with herpes zoster ophthalmicus and extensive intracranial and orbital involvement as documented by MR and pathologically. MR showed all of the lesions that led to the ophthalmoplegia and pseudotumor syndrome, the periaxial infarct of the distal optic nerve, pontine infarcts, and granulomatous angiitis of the meningeal vessels. MR is useful in both detection and monitoring of the disease.
