Histopathology of fenestration operation for otosclerosis.

The author describes the histopathology of fenestration operation in five temporal bones of the Massachusetts Eye and Ear Infirmary collection. The four subjects underwent the intervention more than 18 years before their death. One of the patients was operated bilaterally by Lempert himself in 1950. Special attention is given to the effect on hearing results of the otosclerotic disease; the condition of the middle ear, mastoidectomy cavity, and fenestra; and the lateral and posterior membranous semicircular canals. Also the patency of the three fenestrae (the oval, the round, and the new fenestra) in respect to the resulting hearing function, is considered.

A functional measure of brain activity: brain stem transmission time.

Surface-recorded auditory nerve and brain stem responses are being used routinely for diagnostic purposes in man. When interest is in auditory diagnosis, the electric response threshold is of primary importance. However, when used in neurological diagnosis, the wave form of the response is important. As a measure of one aspect of response wave form, this paper suggests the use of brain stem transmission time (BTT), defined as the time interval between the first earlobe-negative wave (response of the auditory nerve--the 'input' to the brain stem) and the earlobe-positive wave from the region of the inferior colliculus (the 'output' of the brain stem). The paper shows that BTT is longest in neonates, approaches adult values at the age of about 3 years, is relatively independent of click intensity, conductive hearing loss (middle ear lesion), click rate (except for high rates) and click frequency (filtered clicks). The finding that in a given age group, BTT is generally independent of most stimulus conditions, makes it a useful functional test of brain stem activity.

Factors associated with climacteric symptoms in women around menopause attending menopause clinics in Italy.

OBJECTIVE: To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS: Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS: The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS: This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

The increasing burden of infectious diseases on hospital services at St Mary's Hospital Lacor, Gulu, Uganda.

To evaluate the impact of infectious diseases on hospital services in Northern Uganda, a retrospective analysis of discharge records concerning 70,304 inpatients admitted to the Lacor Hospital (Gulu, Uganda) during the period 1992-1997 was performed. Children less than five years old represented 46.5% of the admissions, and the burden of infectious diseases on pediatric admissions increased over time, especially due to malaria and measles. Infectious diseases accounted for 7 of the 10 leading causes of admission. The most frequent cause was malaria (21.8% of total). The second leading infectious disease resulting in admission was respiratory tuberculosis (6.2%); given the long hospital stay, this is the most important disease in terms of hospital bed days (24.6%). Infectious diseases have represented a progressively heavy burden on hospital services, mostly due to pediatric admissions. Respiratory tuberculosis and malaria represent nearly one-third of the overall burden in terms of hospital bed days.

Impact of insecurity, the AIDS epidemic, and poverty on population health: disease patterns and trends in Northern Uganda.

A retrospective analysis of the discharge records of 186,131 inpatients admitted to six Ugandan hospitals during 1992-1998 was performed to describe the disease patterns and trends among the population of Northern Uganda. In all hospitals, malaria was the leading cause of admission and the frequency of admissions for malaria showed the greatest increase. Other conditions, such as malnutrition and injuries, mainly increased in the sites affected by civil conflict and massive population displacement. Tuberculosis accounted for the highest burden on hospital services (approximately one-fourth of the total bed-days), though it showed a stable trend over time. A stable trend was also observed for acquired immunodeficiency syndrome (AIDS), which is in contrast to the hypothesis that AIDS patients have displaced other patients in recent years. In conclusion, preventable and/or treatable communicable diseases, mainly those related to poverty and poor hygiene, represent the leading causes of admission and death, reflecting the socioeconomic disruption in Northern Uganda.

[HACCP (Hazard Analysis and Critical Point system) at a catering service: checklist and microbiological examination].

The A.A. carried out a survey on the functioning of a catering service in Rome. The risk analysis assessment was performed by means of a checklist that collected systematically all data about the environment, the operators, the food and the catering process. The results of the microbiological analysis carried out in the CCP's showed that the general conditions of the catering service were not satisfactory, but also indicated the validity of the checklist used. This method showed a high predictive value and can be useful to improve the application of the HACCP system.

[Arterial pressure in a student population in Aquila].

The preliminary data of a policentric study for the determination of the blood-pressure values and the risk factors associated with high-blood pressure are reported for establishing a prospective epidemiological research. This research involved 258 children attending nursery, elementary-school in the city of L'Aquila. The weight, height, the triceps-skin-fold, the heart-rate, the blood systolic and diastolic-pressure (fifth tone) were examined. Furthermore the parent who accompanied the child was given a questionnaire in order to define: the general information concerning the child and the parent, the family anamnesis concerning the pathology correlated to the high-blood pressure, the child's weight at birth and the thronging-index (the ratio of the number of the family members with respect to the number of the habitable rooms). The obtained data show, especially for the PAS, higher blood-pressure values (greater than 95. percentile) in the grown-up subjects and in the male. The close correlation between the heart rate, BMI and the weight at birth, on one hand, and the blood-pressure values on the other hand, show, besides the validity of the research carried out, the connection between the blood-pressure situation and other physiological, paraphysiological and pathological factors. In conclusion, observing that a considerably high percentage of subjects with high-blood pressure values (5.8%' is one important result, especially for the future implications that it can have; it has to be defined if this situation has a prediction value of constant high-blood pressure in the adult and in what measure the examined factors influence the stabilization of this pathological situation.

[Traumatic auricular hematoma in athletes: histopathological findings and clinical considerations on the experimental animal model].

Surgical procedures used for treating hematomas of the auricle are often defective in obtaining complete cosmetic and functional results when hematoma is of traumatic origin, as often thickening and deformity may outcome. Up to date, some pathological aspects of the healing process of auricular hematomas due to blunt trauma are still unclear as only lesions similar to spontaneous hematoma were experimentally produced and studied. Traumatic auricular hematoma, a lesion caused by rough contacts in sports like rugby, judo, karate, wrestling, etc., was never experimentally induced and studied. The Authors have induced both traumatic and non-traumatic hematomas in a group of 5 rabbits, injecting blood clots under the perichondral layer of one auricle while in the contralateral the hematoma was induced by compressive friction as it is caused during the above referred sports activities. Lesions of both sides were examined after 15 and 60 days and it was found that histopathological picture and clinical course of traumatic hematoma deeply differ from those of non-traumatic one. Non-traumatic lesion appears like a circumscribed hematoma without any evident sign of surrounding reaction, while traumatic lesion shows clear signs of polymorphic and marked reaction. In traumatic hematoma reactive and reparative processes are observed which alter the cartilage and perichondrium associated with many which alter the microhematomas in the subcutaneous connective tissue, close to the main hematoma. Histological findings found 2 months after injury show cartilaginous auricolar skeleton to be thickened by chondrocytes grouped in islands or fingered from perichondrium.(ABSTRACT TRUNCATED AT 250 WORDS)
